Brief presentation about defining health policy with examples from public health. Introduces one way of thinking about health policy: big P and little p.
The United States health care system is a $3 trillion enterprise, the largest in the developed world. Yet Americans often experience more severe access and quality problems, and spend much more for the same procedures and medications, than patients in other countries. Projections of the future cost of health care are unsustainable, yet many well-intended cost-control efforts have been ineffective. This forum features two renowned experts who will discuss the causes of and potential solutions to the extraordinary cost of American health care. Physician, journalist and Stanford alum Elisabeth Rosenthal has drawn national attention to the issue through her widely praised “Paying Till It Hurts” series in the New York Times. She will be joined by Professor Doug Owens, the director of Stanford’s Center for Health Policy and an expert in health care cost-effectiveness research.

About this Course
Many Americans complain that they do not understand the U.S. health reform law called the Affordable Care Act (or Obamacare). They are right, and the main reason is because most Americans do not understand the basics of the U.S. health care system and U.S. health policy. Featuring some of the nation's foremost teachers and thought leaders, this course provides students with a basic and thorough understanding of the U.S. health care system focusing on access, quality of care, and costs. Students will learn how the system is structured, how care is organized, delivered, and financed, and how the Affordable Care Act will influence the future of the system. Students will understand the U.S. health policy making structure process at the federal, state, and local levels. Students who complete this course will be able to interpret current controversies around US health policy and develop informed opinions on future policy developments.

Health care reform in the US is well underway despite political and legal challenges. Experiments in expanding access to healthcare while simultaneously bending the cost curve are underway. Drs. Phillips and Bazemore will discuss some of these, including Accountable Care Organizations and Patient Centered Medical Homes, and review some of the evidence about improved quality and reduced costs. They will also demonstrate tools that the Robert Graham Center has developed for federal and state governments to turn data into information about where care is delivered, where to put new resources more effectively, where disparities reduce health, and where resources are needed to reduce costs as new people receive health insurance. They will also review related collaborations with the Australian Primary Health Care Research Institute.
Bob Phillips became Director of the Robert Graham Center for Policy Studies in Family Medicine and Primary Care in 2004. He trained in medicine at the University of Florida and completed family medicine training and an MSPH in health services research at the University of Missouri. He is a Professor at Georgetown University and a Clinical Professor at Virginia Commonwealth University. Between 2006 and 2010 he was Vice Chair of the US Council on Graduate Medical Education and continues as a technical and policy advisor to the US Department of Health and Human Services. He was elected to the US Institute of Medicine of the National Academies of Science in 2010.
Andrew Bazemore became Medical Director for Health Policy Research at the Robert Graham Center in 2005. He trained in medicine at the University of North Carolina, completed vocational training as a family physician at the University of Cincinnati, and an MPH at the Harvard School of Public Health. He is an Associate Professor at both Georgetown and Virginia Commonwealth Universities. He co-directs the Health Policy Research Fellowship with Georgetown University and oversees a successful research portfolio that includes funding from federal and philanthropic sources.
The Robert Graham Center aims to improve individual and population health by enhancing the delivery of primary care. The Center is a division of the American Academy of Family Physicians, and is tasked with conducting research and analysis to inform deliberations of the Academy in its public policy work and to provide a family medicine perspective to policy deliberations in Washington, D.C.

Health policy can be defined as the "decisions, plans, and actions that are undertaken to achieve specific health care goals within a society." According to the World Health Organization, an explicit health policy can achieve several things: it defines a vision for the future; it outlines priorities and the expected roles of different groups; and it builds consensus and informs people.
There are many categories of health policies, including personal health care policy, pharmaceutical policy, and policies related to public health such as vaccination policy, tobacco control policy or breastfeeding promotion policy. They may cover topics of financing and delivery of health care, access to care, quality of care, and health equity.
There are also many topics in the politics and evidence that can influence the decision of a government, private sector business or other group to adopt a specific policy. Evidence-based policy relies on the use of science and rigorous studies such as randomized controlled trials to identify programs and practices capable of improving policy relevant outcomes. Most political debates surround personal health care policies, especially those that seek to reform health care delivery, and can typically be categorized as either philosophical or economic. Philosophical debates center around questions about individual rights, ethics and government authority, while economic topics include how to maximize the efficiency of health care delivery and minimize costs.

Stefan Basil Molyneux (born 24 September 1966) is a blogger, essayist, author, and host of the Freedomain Radio series of podcasts, living in Mississauga, Canada. He has written numerous articles and smaller essays which have been published on libertarian websites such as LewRockwell.com, antiwar.com and Strike The Root, recorded over 2000 podcasts, produced over 800 videos, and written several books which are self-published except for his first, which was published by Publish America. In 2006 Molyneux quit his previous job in the field of computer software and works full-time on Freedomain Radio, a philosophical community website which is funded through donations. He self-identifies as a full-time parent and philosopher.
Freedomain Radio is primarily an online podcast, however Molyneux also uses essays, videos, books, articles, interviews, and public speaking to deliver his content. The regular content of the show covers topics such as anarcho-capitalism, ethics, Austrian economics, atheism, religion, education, family, philosophy and politics. Each Sunday Molyneux conducts a call-in show where listeners can talk with him about either a set topic of discussion, or their own. Freedomain Radio also has an online community message board for listeners to discuss Molyneux's content as well as related topics.

Health care reform in the US is well underway despite political and legal challenges. Experiments in expanding access to healthcare while simultaneously bending the cost curve are underway. Drs. Phillips and Bazemore will discuss some of these, including Accountable Care Organizations and Patient Centered Medical Homes, and review some of the evidence about improved quality and reduced costs. They will also demonstrate tools that the Robert Graham Center has developed for federal and state governments to turn data into information about where care is delivered, where to put new resources more effectively, where disparities reduce health, and where resources are needed to reduce costs as new people receive health insurance. They will also review related collaborations with the Australian Primary Health Care Research Institute.
Bob Phillips became Director of the Robert Graham Center for Policy Studies in Family Medicine and Primary Care in 2004. He trained in medicine at the University of Florida and completed family medicine training and an MSPH in health services research at the University of Missouri. He is a Professor at Georgetown University and a Clinical Professor at Virginia Commonwealth University. Between 2006 and 2010 he was Vice Chair of the US Council on Graduate Medical Education and continues as a technical and policy advisor to the US Department of Health and Human Services. He was elected to the US Institute of Medicine of the National Academies of Science in 2010.
Andrew Bazemore became Medical Director for Health Policy Research at the Robert Graham Center in 2005. He trained in medicine at the University of North Carolina, completed vocational training as a family physician at the University of Cincinnati, and an MPH at the Harvard School of Public Health. He is an Associate Professor at both Georgetown and Virginia Commonwealth Universities. He co-directs the Health Policy Research Fellowship with Georgetown University and oversees a successful research portfolio that includes funding from federal and philanthropic sources.
The Robert Graham Center aims to improve individual and population health by enhancing the delivery of primary care. The Center is a division of the American Academy of Family Physicians, and is tasked with conducting research and analysis to inform deliberations of the Academy in its public policy work and to provide a family medicine perspective to policy deliberations in Washington, D.C.

His policy research focuses on achieving large-scale change in global health, particularly related to infectious diseases of poverty and to reproductive, maternal, newborn, and child health. He teaches masters courses in global health policy at UCSF and at the London School of Hygiene and Tropical Medicine.
Gavin has been an External Advisor to the World Health Organization and to TDR, the Special Program for Research and Training in Tropical Diseases. He is a member of the Lancet Commission on Investing in Health, chaired by Lawrence Summers and co-chaired by Dean Jamison, which recently published the Global Health 2035 report, and he chairs the Economics and Finance Working Group of the Lancet Commission on Global Surgery.

Judith Healy is an Adjunct Associate Professor with the School of Regulation and Global Governance in the College of Asia and Pacific at the Australian National University. In this public seminar, Judith will examine different ways of comparing and analysing health systems, institutions, health policies and programs, drawing in part on the work of the Asia Pacific Observatory on Health Systems and Policies. Health policy makers often ask how does my country compare to others on measures of health systems and population health outcomes? We care about the reputation of our country but more importantly a comparative perspective offers an opportunity to learn from other countries. This is because while health systems face broadly similar problems (eg. constrained funds, shortages of nurses, sub-standard services) their responses may be very different - and so different solutions in one country may suggest better ways to do things for another country. Comparative health systems research now is a rapidly expanding field that encompasses multiple approaches and methods.

Health policy makers often ask how does my country compare to others on measures of health systems and population health outcomes? We care about the reputation of our country but more importantly a comparative perspective offers an opportunity to learn from other countries. This is because while health systems face broadly similar problems (eg. constrained funds, shortages of nurses, sub-standard services) their responses may be very different - and so different solutions in one country may suggest better ways to do things for another country. Comparative health systems research now is a rapidly expanding field that encompasses multiple approaches and methods.

Menzies Centre for Health Policy at The Australian National University conducts academically rigorous research and innovative capacity building programs that lay the foundation for more effective, equitable and efficient domestic and international health policy. The Centre's research addresses three themes that include social determinants of health and health equity, systems and translation, and integrated care.

We continue our tour of Washington, D.C., where we visit a monument and discuss the monumental issues impacting our economy and the business community.
This time we headed over to the Tidal Basin and the Jefferson Memorial with Katie Mahoney, executive director of Health Policy at the U.S. Chamber of Commerce. KEYSTONE is a collective initiative of several Indian health policy and systems research (HPSR) organizations to strengthen national capacity in HPSR towards addressing critical needs of health systems and policy development. KEYSTONE is convened by the Public Health Foundation of India in its role as Nodal Institute of the Alliance for Health Policy and Systems Research (AHPSR).
The inaugural KEYSTONE short course was conducted in New Delhi from 23 February – 5 March 2015. In the process of delivering the inaugural course, a suite of teaching and learning materials were developed under Creative Commons license, and are being made available as open access resources. The KEYSTONE teaching and learning resources include 38 videos and 43 slide presentations organized into 13 modules (link here for main index page). These materials cover foundational concepts, common approaches used in HPSR, and guidance for preparing a research plan.

Participatory action research (PAR) seeks to understand and improve the world by changing it, but does so in a manner that those affected by problems collectively act and produce change as a means to new knowledge. It specifically, recognizes the wealth of assets that community members bring to the processes of knowing, creating knowledge and acting on that knowledge to bring about change. This module focuses on the core concepts and frameworks of the PAR approach, its suitability to address health system problems and issues of rigour and ethics associated with this approach.

Module 9: Implementation research
Implementation research is the scientific study of the processes used in the implementation of initiatives as well as the contextual factors that affect these processes. It can address or explore any aspect of implementation, including the factors affecting implementation, the processes of implementation themselves, and the outcomes, or end products of the implementation under study. This module provides an overview of IR characteristics, application of IR to complex interventions, the role of IR in scaling up, and the differences between IR and Participatory Action Research.

Module 9: Ethnography
The ethnographic approach offers a unique research space to better understand context (political, social, institutional, and historical) of health policies and policy formulation; and how these policies are translated and come alive in health systems. This approach lends itself well for a nuanced analysis of the relationships between power, knowledge and practice in health systems. In this module the (a) roots of ethnographic approach, multiple traditions, key features, principles, strengths and limitations; (b) distinguishing ethnography per se from qualitative research; (c) ethnographic approach and health systems as social institutions & (d) ethical issues and rigour in the ethnographic approach are explored.

Evidence-Based Medicine (EBM) developed to ground clinical practice on scientific facts and reduce the idiosyncratic scope of different therapeutic approaches by basing clinical research on statistical methods, using Randomized Clinical Trials, and meta-analysis. Since its introduction in the 1990s EBM became one of the most influential concepts in the medical sphere. It engendered the development of Evidence-Based Public Health (EBPH) as an attempt to ground health policies and interventions on "sound facts." Professor Davidovitch’s talk presents a critical evaluation of EBM and EBPH, discussing their historical roots, limitations and influences on health policy making.
