Richard Cook
Royal Institute of Technology, Stockholm
Dr. Richard Cook is the Professor of Healthcare Systems Safety and Chairman of the Department of Patient Safety at the Kungliga Techniska Hogskolan (the Royal Institute of Technology) in Stockholm, Sweden. He is a practicing physician, researcher and educator.
Dr. Cook graduated with honors from Lawrence University in Appleton, Wisconsin where he was a Scholar of the University. He worked in the computer industry in supercomputer system design and engineering applications. He received the MD degree from the University of Cincinnati in 1986 where he was a General Surgery intern. Between 1987 and 1991 he was researcher on expert human performance in Anesthesiology and Industrial and Systems Engineering at The Ohio State University. He completed an Anesthesiology residency at Ohio State in 1994. From November 1994 until April 2012 he was faculty in the Department of Anesthesia and Intensive Care of the University of Chicago.
Dr. Cook was a member of the Board of the National Patient Safety Foundation from its inception until 2007. He is internationally recognized as a leading expert on medical accidents, complex system failures, and human performance at the sharp end of these systems. He has investigated a variety of problems in such diverse areas as urban mass transportation, semiconductor manufacturing, and military software systems. He is often a consultant for not-for-profit organizations, government agencies, and academic groups. He does not do any expert witness testimony for litigation.

Resilience In Complex Adaptive Systems: Operating At The Edge Of Failure
Systems seem to run at the very edge of failure much of the time. The combination of high workload, limited resources, pressure for additional features and capability, and inherent software, hardware, and network fragility is a noxious kettle of stuff always about to boil over in the form of outages, degraded response, or functional breakdowns. For insiders the surprising thing about our systems is not that they fail so often but that they fail so rarely! This good performance in the face of adverse conditions is called resilience. An important conclusion from resilience studies is that it depends critically on human operators and their ability to anticipate and monitor the system, react to threats, and sacrifice some goals to protect others. This talk will introduce resilience and a model of system dynamics useful in analyzing failed and successful event management and offer an explanation for why our systems run at the edge of failure.

New York’s most iconic buildings, the early 20th-Century high rises, were designed as aspirational symbols of urban life with carefully sculpted forms that mediate between the great height of the structure and life at street level. These stepped skyscrapers created a “fifth façade” of roofs, terraces, and balconies that took advantage of daylight, natural ventilation, and preserved views. Inspired by biophilic design, New York City architects are designing new urban highrises as modern interpretations of the materiality and language of these early buildings, repurposing iconic sculpted forms to integrate nature into the built environment with a fifth façade of planted space. This presentation discusses architectural precedents and their modern interpretation as environmentally responsive New York high rises.

The New Yorker
The New Yorker is an American magazine of reportage, commentary, criticism, essays, fiction, satire, cartoons, and poetry. It is published by Condé Nast. Started as a weekly in 1925, the magazine is now published 47 times annually, with five of these issues covering two-week spans.
Although its reviews and events listings often focus on the cultural life of New York City, The New Yorker has a wide audience outside of New York. It is well known for its illustrated and often topical covers, its commentaries on popular culture and eccentric Americana, its attention to modern fiction by the inclusion of short stories and literary reviews, its rigorous fact checking and copyediting, its journalism on politics and social issues, and its single-panel cartoons sprinkled throughout each issue.
History
The New Yorker debuted on February 21, 1925. It was founded by Harold Ross and his wife, Jane Grant, a New York Times reporter. Ross wanted to create a sophisticated humor magazine that would be different from perceivably "corny" humor publications such as Judge, where he had worked, or Life. Ross partnered with entrepreneur Raoul H. Fleischmann (who founded the General Baking Company) to establish the F-R Publishing Company and established the magazine's first offices at 25 West 45th Street in Manhattan. Ross edited the magazine until his death in 1951. During the early, occasionally precarious years of its existence, the magazine prided itself on its cosmopolitan sophistication. Ross famously declared in a 1925 prospectus for the magazine: "It has announced that it is not edited for the old lady in Dubuque."

Dr. Cook graduated with honors from Lawrence University in Appleton, Wisconsin where he was a Scholar of the University. He worked in the computer industry in supercomputer system design and engineering applications. He received the MD degree from the University of Cincinnati in 1986 where he was a General Surgery intern. Between 1987 and 1991 he was researcher on expert human performance in Anesthesiology and Industrial and Systems Engineering at The Ohio State University. He completed an Anesthesiology residency at Ohio State in 1994. From November 1994 until April 2012 he was faculty in the Department of Anesthesia and Intensive Care of the University of Chicago.
Dr. Cook was a member of the Board of the National Patient Safety Foundation from its inception until 2007. He is internationally recognized as a leading expert on medical accidents, complex system failures, and human performance at the sharp end of these systems. He has investigated a variety of problems in such diverse areas as urban mass transportation, semiconductor manufacturing, and military software systems. He is often a consultant for not-for-profit organizations, government agencies, and academic groups. He does not do any expert witness testimony for litigation.

New York’s most iconic buildings, the early 20th-Century high rises, were designed as aspirational symbols of urban life with carefully sculpted forms that mediate between the great height of the structure and life at street level. These stepped skyscrapers created a “fifth façade” of roofs, terraces, and balconies that took advantage of daylight, natural ventilation, and preserved views. Inspired by biophilic design, New York City architects are designing new urban highrises as modern interpretations of the materiality and language of these early buildings, repurposing iconic sculpted forms to integrate nature into the built environment with a fifth façade of planted space. This presentation discusses architectural precedents and their modern interpretation as environmentally responsive New York high rises.

Mr. Cook discussed the early start of JPL in the 1930's and how the Explorer One Mission helped fortify JPLs' legacy. Richard gave insight to future space exploration plans including a second visit to Mars and Europa moon of Jupiter. Dr. Richard Cook is the Professor of Healthcare Systems Safety and Chairman of the Department of Patient Safety at the Kungliga Techniska Hogskolan (the Royal Institute of Technology) in Stockholm, Sweden. He is a practicing physician, researcher and educator.
Dr. Cook graduated with honors from Lawrence University in Appleton, Wisconsin where he was a Scholar of the University. He worked in the computer industry in supercomputer system design and engineering applications. He received the MD degree from the University of Cincinnati in 1986 where he was a General Surgery intern. Between 1987 and 1991 he was researcher on expert human performance in Anesthesiology and Industrial and Systems Engineering at The Ohio State University. He completed an Anesthesiology residency at Ohio State in 1994. From November 1994 until April 2012 he was faculty in the Department of Anesthesia and Intensive Care of the University of Chicago.
Dr. Cook was a member of the Board of the National Patient Safety Foundation from its inception until 2007. He is internationally recognized as a leading expert on medical accidents, complex system failures, and human performance at the sharp end of these systems. He has investigated a variety of problems in such diverse areas as urban mass transportation, semiconductor manufacturing, and military software systems. He is often a consultant for not-for-profit organizations, government agencies, and academic groups. He does not do any expert witness testimony for litigation.

Resilience In Complex Adaptive Systems: Operating At The Edge Of Failure
Systems seem to run at the very edge of failure much of the time. The combination of high workload, limited resources, pressure for additional features and capability, and inherent software, hardware, and network fragility is a noxious kettle of stuff always about to boil over in the form of outages, degraded response, or functional breakdowns. For insiders the surprising thing about our systems is not that they fail so often but that they fail so rarely! This good performance in the face of adverse conditions is called resilience. An important conclusion from resilience studies is that it depends critically on human operators and their ability to anticipate and monitor the system, react to threats, and sacrifice some goals to protect others. This talk will introduce resilience and a model of system dynamics useful in analyzing failed and successful event management and offer an explanation for why our systems run at the edge of failure.

Dr. Richard Cook is the Professor of Healthcare Systems Safety and Chairman of the Department of Patient Safety at the Kungliga Techniska Hogskolan (the Royal Institute of Technology) in Stockholm, Sweden. He is a practicing physician, researcher and educator.
Dr. Cook graduated with honors from Lawrence University in Appleton, Wisconsin where he was a Scholar of the University. He worked in the computer industry in supercomputer system design and engineering applications. He received the MD degree from the University of Cincinnati in 1986 where he was a General Surgery intern. Between 1987 and 1991 he was researcher on expert human performance in Anesthesiology and Industrial and Systems Engineering at The Ohio State University. He completed an Anesthesiology residency at Ohio State in 1994. From November 1994 until April 2012 he was faculty in the Department of Anesthesia and Intensive Care of the University of Chicago.
Dr. Cook was a member of the Board of the National Patient Safety Foundation from its inception until 2007. He is internationally recognized as a leading expert on medical accidents, complex system failures, and human performance at the sharp end of these systems. He has investigated a variety of problems in such diverse areas as urban mass transportation, semiconductor manufacturing, and military software systems. He is often a consultant for not-for-profit organizations, government agencies, and academic groups. He does not do any expert witness testimony for litigation.
Dr. Cook's most often cited publications are "Gaps in the continuity of patient care and progress in patient safety", "Operating at the Sharp End: The complexity of human error", "Adapting to New Technology in the Operating Room", and the report "A Tale of Two Stories: Contrasting Views of Patient Safety"

Rick Cook discusses the issues surrounding the creation of environmentally responsible, high-performance buildings and explore the holistic definition of building performance criteria. Moving to the next level of high-performance building, we should consider not only "how we build" but also, "how we can make a difference" when we build. PlaNYC reports that buildings contribute to 80% of NYC's vast carbon footprint- demanding immediate action to transform the urban landscape for an energy efficient future while preserving another resource: the vitality and health of urban citizens.
Rick Cook is a Partner at Cook+Fox Architects, a firm devoted to creating environmentally responsible, high-performance buildings. Over the past 25 years as a New York City architect, he has built a reputation for innovative, award-winning architectural design. Rick cultivated a broad portfolio ranging from master planning to various commercial and residential projects. With special expertise in working in historic Landmarks districts, the firm received numerous honors and awards for integrating contemporary design with sensitive, contextual architecture.
In 2003, Rick combined his established firm with Bob Fox to form Cook+Fox Architects based on a transformative vision: beautiful design guided by high standards of sustainability, and a commitment to excellence enriched by the collaborative process. Its work includes three LEED Platinum projects in New York City: the Bank of America Tower at One Bryant Park, Skanska USA's office in the Empire State Building, and Cook+Fox's own office interior.
