Basal-cell carcinoma (BCC) is a very common type of skin cancer. It rarely metastasizes or kills. However, because it can cause significant destruction and disfigurement by invading surrounding tissues, it is still considered malignant.
Statistically, approximately 3 out of 10 Caucasians may develop a basal-cell cancer within their lifetime. In 80 percent of all cases, basal-cell cancers are found on the head and neck. There appears to be an increase in the incidence of basal-cell cancer of the trunk (torso) in recent years.
Basal-cell carcinomas may be divided into the following types:

Patients present with a shiny, pearly nodule. However, superficial basal-cell cancer can present as a red patch like eczema. Infiltrative or morpheaform basal-cell cancers can present as a skin thickening or scar tissue – making diagnosis difficult without using tactile sensation and a skin biopsy. It is often difficult to distinguish basal-cell cancer from acne scar, actinic elastosis, and recent cryodestructioninflammation.
The stratum basale (basal layer, sometimes referred to as stratum germinativum) is the deepest layer of the five layers of the epidermis, which is the outer covering of skin in mammals.
The stratum basale is a continuous layer of cells. It is often described as one cell thick, though it may in fact be two to three cells thick in glabrous (hairless) skin and hyperproliferative epidermis (from a skin disease).
The stratum basale is primarily made up of basal keratinocyte cells, which can be considered the stem cells of the epidermis. They divide to form the keratinocytes of the stratum spinosum, which migrate superficially. Other types of cells found within the stratum basale are melanocytes (pigment-producing cells), Langerhans cells (immune cells), and Merkel cells (touch receptors).

In nodular BCC, the cancer presents itself as pearly, skin-colored or pink bumps, with tiny blood vessels often appearing on their surface. As the lesion grows, it may ulcerate or bleed with minor trauma. Infiltrating lesions often appear scar-like or depressed. They may often be shiny and have sores or scabs on their surface. Superficial BCC presents itself as pink or red scaly skin, often dry appearing. Lesions will slowly grow and the border of the lesion may become raised. This can be mistaken as a chronic rash such as ringworm or just dryness. This subtype is generally found on the trunk, arms, and legs. While BCC can affect any age or ethnic group, it most commonly appears in elderly fair complected caucasians. Chronic sun exposure is virtually always noted as a causative factor. There are also syndromes they convey a markedly elevated risk of BCC, such as nevoid Basal cell carcinoma Syndrome and Xeroderma Pigmentosa.

*Surgical removal is the most commonly employed treatment
While self-examination is recommended to identify lesions, they must be treated by health care professionals. Once diagnosis is confirmed via biopsy, treatment will be depend on several factors. These may include size and location of the cancer, as well as the general health of the patient.
In the case of nodular BCC, treatment options include electrodessication and curettage, surgical excision, Mohs Micrographic surgery, and radiation treatment of the affected area. Radiation is usually only used in cases where the affected person isn't a good candidate for surgery due to other health problems.

Perhaps a simple liquid nitrogen procedure could have removed a new cancer, but instead because I procrastinated for years, and didn't see the signs that my body was giving me, I had to go through this entire mess.
Hopefully this will serve as a wake up call to some of my friends, and whomever views this, to get to a dermatologist IMMEDIATELY if you see something that doesn't look normal. If you think a dermatologist visit is too expensive, ask yourself this: would you rather have your face chopped up to the tune of $30,000 bucks, or would you rather cough up a hundred bucks to go in for a biopsy. I pays to be "chatty". It takes practice to be able to carry on a conversation while doing surgery.. as you can see I use an electrocautery to cauterize bleeding vessels, and there is an instance where I had to tie off an arteriole to diminish bleeding.
Deep vertical mattress sutures, followed by running subcuticular suture were used to close the wound.

This avoids creating pulling of the tissue in noticeable ways, such as avoiding pulling up of the corner of his right lip, or distorting the shape of the right side of his nose.

They are due to the development of abnormal cells that have the ability to invade or spread to other parts of the body.[1] There are three main types: basal cell cancer (BCC), squamous cell cancer (SCC) and melanoma.[2] The first two together along with a number of less common skin cancers are known as nonmelanoma skin cancer (NMSC).[3][4] Basal cell cancer grows slowly and can damage the tissue around it but is unlikely to spread to distant areas or result in death.[3] It often appears as a painless raised area of skin, that may be shiny with small blood vessel running over it or may present as a raised area with an ulcer.[2] Squamous cell cancer is more likely to spread.[3] It usually presents as a hard lump with a scaly top but may also form an ulcer.[5] Melanomas are the most aggressive. Signs include a mole that has changed in size, shape, color, has irregular edges, has more than one color, is itchy or bleeds.[6]
Greater than 90% of cases are caused by exposure to ultraviolet radiation from the Sun.[7] This exposure increases the risk of all three main types of skin cancer.[7] Exposure has increased partly due to a thinner ozone layer.[3] Tanning beds are becoming another common source of ultraviolet radiation.[7] For melanomas and basal cell cancers exposure during childhood is particularly harmful.[8] For squamous cell cancers total exposure, irrespective of when it occurs, is more important.[7] Between 20% and 30% of melanomas develop from moles.[8] People with light skin are at higher risk [2] as are those with poor immune function such as from medications or HIV/AIDS.[3][9] Diagnosis is by biopsy.[6]
Decreasing exposure to ultraviolet radiation and the use of sunscreen appears to be effective methods of preventing melanoma and squamous cell cancer.[8][10] It is not clear if sunscreen affects the risk of basal cell cancer.[10] Nonmelanoma skin cancer is usually curable.[3] Treatment is generally by surgical removal but may less commonly involve radiation therapy or topical medications such as fluorouracil.[2] Treatment of melanoma may involve some combination of surgery, chemotherapy, radiation therapy, and targeted therapy.[6] In those people whose disease has spread to other areas of their bodies, palliative care may be used to improve quality of life.[6] Melanoma has one of the higher survival rates among cancers, with over 86% of people in the UK and more than 90% in the United States surviving more than 5 years.[11][12]
Skin cancer is the most common form of cancer, globally accounting for at least 40% of cases.[3][13] It is especially common among people with light skin.[14] The most common type is nonmelanoma skin cancer, which occurs in at least 2-3 million people per year.[8][15] This is a rough estimate, however, as good statistics are not kept.[2] Of nonmelanoma skin cancers, about 80% are basal cell cancers and 20% squamous cell cancers.[4] Basal cell and squamous cell cancers rarely result in death.[8] In the United States they were the cause of less than 0.1% of all cancer deaths.[2] Globally in 2012 melanoma occurred in 232,000 people, and resulted in 55,000 deaths.[8] Australia and New Zealand have the highest rates of melanoma in the world.[8] The three main types of skin cancer have become more common in the last 20 to 40 years, especially in those areas which are mostly Caucasian.

Cancer is not just one disease but many diseases. There are more than 100 different types of cancer. -Lymphoma and myeloma - cancers that begin in the cells of the immune system.
Central nervous system cancers - cancers that begin in the tissues of the brain and spinal cord.

Obesity is associated with an aggressive subtype of breast cancer called basal-like breast cancer (BBC). Using C3(1)-TAg mice, a genetically engineered mouse model that resembles human BBC, we demonstrated that mice displayed increased tumor aggressiveness when fed obesogenic diets. Obesity induced in the adult window of susceptibility triggered early latency and elevated mammary gland expression and activation of hepatocyte growth factor (HGF)/c-Met compared to lean controls, a pro-tumorigenic pathway associated with BBC in patients. HGF secretion was also increased from primary mammary fibroblasts isolated from normal mammary glands and tumors of obese mice compared to lean. Conditioned media from primary tumor fibroblasts from obese mice drove tumor cell proliferation compared to lean controls. In co-culture, neutralization of secreted HGF blunted tumor cell migration, further linking obesity-mediated HGF-dependent effects to in vitro measures of tumor aggressiveness. Using a more severe diet model, we exposed mice to a lifelong diet intervention - C3(1)-TAg mice were weaned onto and maintained on an obesogenic high fat diet. Obese mice displayed significant elevations in tumor progression. Epidemiologic studies estimate that weight loss could prevent a large proportion of BBC. We sought to investigate whether weight loss in adulthood prior to tumor onset would protect mice from the accelerated tumorigenesis observed in obese mice. Indeed, tumor progression was significantly reversed when obese mice were induced to lose weight by switching to a control low fat diet prior to tumor onset compared to mice maintained on obesogenic diet. Obesity-elevated HGF/c-Met expression in normal mammary glands and c-Met in tumors was significantly reversed with weight loss. Other mediators associated with oncogenesis such as hyperinsulinemia and a high leptin/adiponectin ratio were also elevated by obesity and reduced with weight loss. In sum, weight loss significantly blunted the obesity-responsive pro-tumorigenic HGF/c-Met pathway and improved several metabolic risk factors associated with BBC, which together may have contributed to the dramatic reversal of obesity-driven tumor progression. Future research aims to evaluate the role of obesity, the mammary microenvironment, and the HGF/c-Met pathway in basal-like cancer progression.

Abstract
Tzank Test is an effective method to complete a definitive diagnosis in Dermatology. It can be used to inflammatory disease, metabolic and cutaneous tumors. It is a very cheap exam and useful to many dermatological disease. It is not limited to Herpes virus infections. Cytology in general was discovered by Erlich in 1870 and in 1928 Papanicolaou found its application in diagnosis of uterine cancer. In Dermatology the pioneer author was Tzanck in France in 1948. In Brazil Dr. Estevam Almeida Neto completed his doutorate in Dermatology in 1960 with a thesis about Cytology of Leprosy. I was his disciple in 1984 when I was a resident in Dermatology at University of Sao Paulo in Brazil. For many years I have work hard using this wonderful practical test to help in diagnosis of many tumoral, inflammatory and metabolic cutaneous diseases. We can do a rapid diagnosis of a skin tumor and begin the treatment immediately. I have used two type of smear stain (GIEMSA and PAPANICOLAOU). We can differentiate between Pigmented Basal Cell Carcinoma to Melanoma or Pigmented Bowen Disease using Tzanck Test. It is not overcome skin biopsy but has a complementary effect. I was useful in Dermatologic Surgery when we can examine a deeper margin of a surgical ulcer before stop the surgery and seek atypical cells in that place. I would like to show at the Congress how to do this practical method and some case can be showed to demonstration usefulness of this test.
Biography
Thomas Paulo MD, has completed his graduate course in Medicine at the age of 25 years from Federal University of State of Rio Grande do Norte and completed (one year) Residency in Internal Medicine in Rio de Janeiro and Dermatology at University of Sao Paulo - Brazil (USP) for two years (1984 and 1985). He concluded his Board in Dermatology in 1986 by Brazilian Society of Dermatology. He is Assistant Professor of Dermatology since 1994 at Department of Dermatology of the Rio Grande do Norte University in Northeast of Brazil. He has a Board in Hansen Disease at Brazilian Association of Leprosy. He is a teacher in Dermatopathology to Residents Doctors at Department of Dermatology. He has made a short Atlas of Cutaneous Cytology in 2009 with support from Galderma Laboratories and had published some articles about Cutaneous Cytology and Skin Cancer on Dermatology Online Journal in March 2007 about Diffuse Melanosis in Malignant Melanoma. He has written half of chapters at two more famous books of Dermatology in Brazil in sections about Diagnosis Tests in General Dermatology.

Abstract
Tzank Test is an effective method to complete a definitive diagnosis in Dermatology. It can be used to inflammatory disease, metabolic and cutaneous tumors. It is a very cheap exam and useful to many dermatological disease. It is not limited to Herpes virus infections. Cytology in general was discovered by Erlich in 1870 and in 1928 Papanicolaou found its application in diagnosis of uterine cancer. In Dermatology the pioneer author was Tzanck in France in 1948. In Brazil Dr. Estevam Almeida Neto completed his doutorate in Dermatology in 1960 with a thesis about Cytology of Leprosy. I was his disciple in 1984 when I was a resident in Dermatology at University of Sao Paulo in Brazil. For many years I have work hard using this wonderful practical test to help in diagnosis of many tumoral, inflammatory and metabolic cutaneous diseases. We can do a rapid diagnosis of a skin tumor and begin the treatment immediately. I have used two type of smear stain (GIEMSA and PAPANICOLAOU). We can differentiate between Pigmented Basal Cell Carcinoma to Melanoma or Pigmented Bowen Disease using Tzanck Test. It is not overcome skin biopsy but has a complementary effect. I was useful in Dermatologic Surgery when we can examine a deeper margin of a surgical ulcer before stop the surgery and seek atypical cells in that place. I would like to show at the Congress how to do this practical method and some case can be showed to demonstration usefulness of this test.
Biography
Thomas Paulo MD, has completed his graduate course in Medicine at the age of 25 years from Federal University of State of Rio Grande do Norte and completed (one year) Residency in Internal Medicine in Rio de Janeiro and Dermatology at University of Sao Paulo - Brazil (USP) for two years (1984 and 1985). He concluded his Board in Dermatology in 1986 by Brazilian Society of Dermatology. He is Assistant Professor of Dermatology since 1994 at Department of Dermatology of the Rio Grande do Norte University in Northeast of Brazil. He has a Board in Hansen Disease at Brazilian Association of Leprosy. He is a teacher in Dermatopathology to Residents Doctors at Department of Dermatology. He has made a short Atlas of Cutaneous Cytology in 2009 with support from Galderma Laboratories and had published some articles about Cutaneous Cytology and Skin Cancer on Dermatology Online Journal in March 2007 about Diffuse Melanosis in Malignant Melanoma. He has written half of chapters at two more famous books of Dermatology in Brazil in sections about Diagnosis Tests in General Dermatology.

The N Engl J Med image of the week shows a young woman’s scalp with a large, pedunculated lesion without hair growth. It has been present since birth. You are offered: cerebriform nevus sebaceous, verruca vulgaris, basal cell carcinoma of the scalp, dermal melanosis, and seborrheic keratosis. We go over all of these lesions. Chronic obstructive pulmonary disease (COPD) is thought to result from an accelerated decline in forced expiratory volume in 1 second (FEV1) over time. Yet it is possible that a normal decline in FEV1 could also lead to COPD in persons whose maximally attained FEV1 is less than population norms. We review data from Framhingham, the Copenhagen City Study, and the Lovelace Smokers Cohort to address this question. Patients with advanced squamous-cell non–small-cell lung cancer (NSCLC) who have disease progression during or after first-line chemotherapy have limited treatment options. A randomized, open-label, international, phase 3 study was performed that evaluated the efficacy and safety of nivolumab, a fully human IgG4 programmed death 1 (PD-1) immune-checkpoint–inhibitor antibody, as compared with docetaxel in this patient population. An increase in overall survival is claimed. The treatment of peripheral artery disease with percutaneous transluminal angioplasty is limited by the occurrence of vessel recoil and restenosis. Drug-coated angioplasty balloons deliver antiproliferative agents directly to the artery, potentially improving vessel patency by reducing restenosis. In this single-blind, randomized trial conducted at 54 sites, investigators assigned, in a 2:1 ratio, 476 patients with symptomatic intermittent claudication or ischemic pain while at rest and angiographically significant atherosclerotic lesions to angioplasty with a paclitaxel-coated balloon or to standard angioplasty. Looks like we had better coat our balloons. Between 2011 and 2013, three breeders of variegated squirrels (Sciurus variegatoides) from Saxony-Anhalt had encephalitis with similar clinical signs and died 2 to 4 months after onset of the clinical symptoms. With the use of a metagenomic approach that incorporated next-generation sequencing and real-time reverse-transcriptase quantitative polymerase chain reaction (RT-qPCR), the presence of a previously unknown bornavirus was detected in a contact squirrel and in brain samples from the three patients. Watch out for those variegated squirrels. Mast cells, which are present in most tissues, mature in situ from hematopoietic progenitors and acquire unique features of local effector cells. These features vary, depending on the tissue microenvironment. A N Engl J Med review provides an overview of recent developments concerning the physiology and pathobiology of mast cells. The case of the week concerns a 37 year-old resident of Vietnam who develops fever, bacteremia, and abdominal pain days after a fish dinner. In the Lancet we review a remarkable feasibility study of investigating transfusion practices in persons with gastrointestinal hemorrhage. Is the 10 g/dL or the 8 g/dL hemoglobin threshold better? Yes, a study would be feasible but is it worth it? Information and behavior can spread through interpersonal ties. By targeting influential individuals, health interventions that harness the distributive properties of social networks could be made more effective and efficient than those that do not. Investigators aimed to assess which targeting methods produce the greatest cascades or spillover effects and hence maximize population-level behavior change. The interventions were a multivitamin preparation and chlorine tablets for drinking water. If you are in to social networking, this study is for you. Comprehensive long-term data on atrial fibrillation trends in men and women are scant. Framingham investigators aimed to provide such data through analysis of the Framingham cohort over 50 years. They investigated trends in incidence, prevalence, and risk factors for atrial fibrillation and its association with stroke and mortality after onset in 9511 participants enrolled in the Framingham Heart Study between 1958 and 2007. Atrial fibrillation remains a burning problem. We then learn that life expectancy in England and Wales is likely to increase between now and 2030, so better keep those pension funds afloat. The review is on how to defeat AIDS; more platitudes are presented. We close with a case of a patient with multiple factures after polytrauma, who goes on to develop petechiae, respiratory, and cerebral failure.

In this tutorial, dermatologist Eric Stewart, MD, discusses the diagnosis and treatment options for basal cell carcinoma.
Key Points

*Consists of bumps or growths which can be pearly or waxy, white, light pink, flesh-colored, or brown, sometimes slightly raised. Commonly, BCC will scab or bleed intermittently
*Frequently occurs in sun-damaged skin
*Does not usually metastasize. These are locally destructive cancers
Basal cell carcinoma (BCC) is the most common form of skin cancer. It usually presents in areas of the skin exposed to sun, including the face, neck, head, chest and upper back, though it can appear anywhere. There are several subtypes, including nodular, infiltrating, morpheic, and superficial.
In nodular BCC, the cancer presents itself as pearly, skin-colored or pink bumps, with tiny blood vessels often appearing on their surface. As the lesion grows, it may ulcerate or bleed with minor trauma. Infiltrating lesions often appear scar-like or depressed. They may often be shiny and have sores or scabs on their surface. Superficial BCC presents itself as pink or red scaly skin, often dry appearing. Lesions will slowly grow and the border of the lesion may become raised. This can be mistaken as a chronic rash such as ringworm or just dryness. This subtype is generally found on the trunk, arms, and legs. While BCC can affect any age or ethnic group, it most commonly appears in elderly fair complected caucasians. Chronic sun exposure is virtually always noted as a causative factor. There are also syndromes they convey a markedly elevated risk of BCC, such as nevoid Basal cell carcinoma Syndrome and Xeroderma Pigmentosa.

Key Points
*Diagnosis based on skin appearance
*Skin biopsy performed to confirm diagnosis and rule out other conditions
*Biopsy results indicate which treatment course is appropriate
BCC is generally diagnosed based on appearance, then further classified by sub-type. Health care professionals will then perform a biopsy to confirm diagnosis and plot out treatment course.
Treatment
*Self-examination and evaluation by a dermatologist is key in identifying lesions early
*Surgical removal is the most commonly employed treatment
While self-examination is recommended to identify lesions, they must be treated by health care professionals. Once diagnosis is confirmed via biopsy, treatment will be depend on several factors. These may include size and location of the cancer, as well as the general health of the patient.
In the case of nodular BCC, treatment options include electrodessication and curettage, surgical excision, Mohs Micrographic surgery, and radiation treatment of the affected area. Radiation is usually only used in cases where the affected person isn't a good candidate for surgery due to other health problems.
With infiltrating BCC or morpheic BCC, treatment is more limited due to the aggressive and invasive nature of the disease. Mohs Micrographic surgery is usually the treatment of choice in this situation.
Aggressive treatments are generally not required for instances of superficial BCC, as it is slow-growing and thin. In addition to conventional treatments such as cryosurgery, electrodessication and radiation treatment, topical options such as Imiquimod (which encourages the immune system to attack the BCC), Photodynamic therapy, and laser treatments can be used to combat the condition.

When I was first diagnosed with this form of cancer, basal cell carcinoma, I was scared and shocked. I thought it would mean the end of my face as I knew it.
With the help of a number of talented professionals in my area, I was able to have the cancer removed, and a bang up reconstructive job done. Thank you Dr. Algarin for the removal, Dr. Yeh for the reconstruction.
I get asked frequently what tipped me off to my cancer. What made me go in and get it checked? The fact is, I had a little blemish on my nose, like a tiny scab. It would fall off on the shower, and be gone for a few days, then a new scab would form. It started out small. It just looked like dry skin at first. I thought it was just exema or something. I thought nothing of it. Over time, as the scab started to replace the dried skin, the scabs started to get bigger, and darker.
After LITERALLY years of this, I finally got off my butt and went in to see my dermatologist. One biopsy later, and it was determined that I had cancer.
Had I gone to see him earlier, I feel very confidently that all of this could have been avoided. Perhaps a simple liquid nitrogen procedure could have removed a new cancer, but instead because I procrastinated for years, and didn't see the signs that my body was giving me, I had to go through this entire mess.
Hopefully this will serve as a wake up call to some of my friends, and whomever views this, to get to a dermatologist IMMEDIATELY if you see something that doesn't look normal. If you think a dermatologist visit is too expensive, ask yourself this: would you rather have your face chopped up to the tune of $30,000 bucks, or would you rather cough up a hundred bucks to go in for a biopsy. I hope you'll chose the former.

Skin cancer surgery: Double Advancement Flap on upper lip
Skin cancer surgery: Double Advancement Flap on upper lip
This patient had Mohs Micrographic Skin Cancer Surgery to remove a basal cell carcinoma, a type of skin cancer on his upper lip. Although at this size, this type of skin cancer is not life threatening, however, it is locally destructive, so needs to be removed, and Mohs Surgery is the ideal technique that removes as little as skin as possible, leaving as small a possible secondary defect (wound), and therefore, likely the smallest scar possible.
Due to the location of the wound (area of skin removed after Mohs surgery), I opted to create a double advancement flap, a type of flap that advances tissue from two different directions, and hides the scar well within natural creases in the face (in this case, the junction between the nasal ala and the upper lip and cheek). This avoids creating pulling of the tissue in noticeable ways, such as avoiding pulling up of the corner of his right lip, or distorting the shape of the right side of his nose.
This is a procedure done in the office under local anesthesia, and is something I do at least a couple days a week. Thank you for watching!!

I pays to be "chatty". It takes practice to be able to carry on a conversation while doing surgery.. as you can see I use an electrocautery to cauterize bleeding vessels, and there is an instance where I had to tie off an arteriole to diminish bleeding.
Deep vertical mattress sutures, followed by running subcuticular suture were used to close the wound.

Megan Miller Shares Her Experience with Skin Cancer
At age 22, Megan Miller was diagnosed with basal cell carcinoma, a common form of skin cancer that can be easily treated. But a year later, she is diagnosed with melanoma.
"Looking back, I knew this was a deadly form of skin cancer," she says.
Melanoma has become the most common form of cancer for young adults age 25 to 29 years old and the second most common for 15-29 years old.
"I was the typical 22 year old who did the tanning bed and sunbathing,"she says. She now shares her story with other young adults in the hopes of reducing their risk of skin cancer. Many of her friends have stopped tanning and have increased their sunscreen use, she says.
"I was lucky enough that I caught my melanoma in time," she says."Early detection saved my life."

Skin cancers are cancers that arise from the skin. They are due to the development of abnormal cells that have the ability to invade or spread to other parts of the body.[1] There are three main types: basal cell cancer (BCC), squamous cell cancer (SCC) and melanoma.[2] The first two together along with a number of less common skin cancers are known as nonmelanoma skin cancer (NMSC).[3][4] Basal cell cancer grows slowly and can damage the tissue around it but is unlikely to spread to distant areas or result in death.[3] It often appears as a painless raised area of skin, that may be shiny with small blood vessel running over it or may present as a raised area with an ulcer.[2] Squamous cell cancer is more likely to spread.[3] It usually presents as a hard lump with a scaly top but may also form an ulcer.[5] Melanomas are the most aggressive. Signs include a mole that has changed in size, shape, color, has irregular edges, has more than one color, is itchy or bleeds.[6]
Greater than 90% of cases are caused by exposure to ultraviolet radiation from the Sun.[7] This exposure increases the risk of all three main types of skin cancer.[7] Exposure has increased partly due to a thinner ozone layer.[3] Tanning beds are becoming another common source of ultraviolet radiation.[7] For melanomas and basal cell cancers exposure during childhood is particularly harmful.[8] For squamous cell cancers total exposure, irrespective of when it occurs, is more important.[7] Between 20% and 30% of melanomas develop from moles.[8] People with light skin are at higher risk [2] as are those with poor immune function such as from medications or HIV/AIDS.[3][9] Diagnosis is by biopsy.[6]
Decreasing exposure to ultraviolet radiation and the use of sunscreen appears to be effective methods of preventing melanoma and squamous cell cancer.[8][10] It is not clear if sunscreen affects the risk of basal cell cancer.[10] Nonmelanoma skin cancer is usually curable.[3] Treatment is generally by surgical removal but may less commonly involve radiation therapy or topical medications such as fluorouracil.[2] Treatment of melanoma may involve some combination of surgery, chemotherapy, radiation therapy, and targeted therapy.[6] In those people whose disease has spread to other areas of their bodies, palliative care may be used to improve quality of life.[6] Melanoma has one of the higher survival rates among cancers, with over 86% of people in the UK and more than 90% in the United States surviving more than 5 years.[11][12]
Skin cancer is the most common form of cancer, globally accounting for at least 40% of cases.[3][13] It is especially common among people with light skin.[14] The most common type is nonmelanoma skin cancer, which occurs in at least 2-3 million people per year.[8][15] This is a rough estimate, however, as good statistics are not kept.[2] Of nonmelanoma skin cancers, about 80% are basal cell cancers and 20% squamous cell cancers.[4] Basal cell and squamous cell cancers rarely result in death.[8] In the United States they were the cause of less than 0.1% of all cancer deaths.[2] Globally in 2012 melanoma occurred in 232,000 people, and resulted in 55,000 deaths.[8] Australia and New Zealand have the highest rates of melanoma in the world.[8] The three main types of skin cancer have become more common in the last 20 to 40 years, especially in those areas which are mostly Caucasian.

Cancer is a term used for diseases in which abnormal cells divide without control and are able to invade other tissues. Cancer cells can spread to other parts of the body through the blood and lymph systems.
Cancer is not just one disease but many diseases. There are more than 100 different types of cancer. Most cancers are named for the organ or type of cell in which they start - for example, cancer that begins in the colon is called colon cancer; cancer that begins in melanocytes of the skin is called melanoma.
Cancer types can be grouped into broader categories. The main categories of cancer include:
-Carcinoma - cancer that begins in the skin or in tissues that line or cover internal organs. There are a number of subtypes of carcinoma, including adenocarcinoma, basal cell carcinoma, squamous cell carcinoma, and transitional cell carcinoma.
-Sarcoma - cancer that begins in bone, cartilage, fat, muscle, blood vessels, or other connective or supportive tissue.
-Leukemia - cancer that starts in blood-forming tissue such as the bone marrow and causes large numbers of abnormal blood cells to be produced and enter the blood.
-Lymphoma and myeloma - cancers that begin in the cells of the immune system.
Central nervous system cancers - cancers that begin in the tissues of the brain and spinal cord.
What causes cancer?
Cancer is ultimately the result of cells that uncontrollably grow and do not die. Normal cells in the body follow an orderly path of growth, division, and death. Programmed cell death is called apoptosis, and when this process breaks down, cancer begins to form. Unlike regular cells, cancer cells do not experience programmatic death and instead continue to grow and divide. This leads to a mass of abnormal cells that grows out of control.

Obesity is associated with an aggressive subtype of breast cancer called basal-like breast cancer (BBC). Using C3(1)-TAg mice, a genetically engineered mouse model that resembles human BBC, we demonstrated that mice displayed increased tumor aggressiveness when fed obesogenic diets. Obesity induced in the adult window of susceptibility triggered early latency and elevated mammary gland expression and activation of hepatocyte growth factor (HGF)/c-Met compared to lean controls, a pro-tumorigenic pathway associated with BBC in patients. HGF secretion was also increased from primary mammary fibroblasts isolated from normal mammary glands and tumors of obese mice compared to lean. Conditioned media from primary tumor fibroblasts from obese mice drove tumor cell proliferation compared to lean controls. In co-culture, neutralization of secreted HGF blunted tumor cell migration, further linking obesity-mediated HGF-dependent effects to in vitro measures of tumor aggressiveness. Using a more severe diet model, we exposed mice to a lifelong diet intervention - C3(1)-TAg mice were weaned onto and maintained on an obesogenic high fat diet. Obese mice displayed significant elevations in tumor progression. Epidemiologic studies estimate that weight loss could prevent a large proportion of BBC. We sought to investigate whether weight loss in adulthood prior to tumor onset would protect mice from the accelerated tumorigenesis observed in obese mice. Indeed, tumor progression was significantly reversed when obese mice were induced to lose weight by switching to a control low fat diet prior to tumor onset compared to mice maintained on obesogenic diet. Obesity-elevated HGF/c-Met expression in normal mammary glands and c-Met in tumors was significantly reversed with weight loss. Other mediators associated with oncogenesis such as hyperinsulinemia and a high leptin/adiponectin ratio were also elevated by obesity and reduced with weight loss. In sum, weight loss significantly blunted the obesity-responsive pro-tumorigenic HGF/c-Met pathway and improved several metabolic risk factors associated with BBC, which together may have contributed to the dramatic reversal of obesity-driven tumor progression. Future research aims to evaluate the role of obesity, the mammary microenvironment, and the HGF/c-Met pathway in basal-like cancer progression.

Abstract
Tzank Test is an effective method to complete a definitive diagnosis in Dermatology. It can be used to inflammatory disease, metabolic and cutaneous tumors. It is a very cheap exam and useful to many dermatological disease. It is not limited to Herpes virus infections. Cytology in general was discovered by Erlich in 1870 and in 1928 Papanicolaou found its application in diagnosis of uterine cancer. In Dermatology the pioneer author was Tzanck in France in 1948. In Brazil Dr. Estevam Almeida Neto completed his doutorate in Dermatology in 1960 with a thesis about Cytology of Leprosy. I was his disciple in 1984 when I was a resident in Dermatology at University of Sao Paulo in Brazil. For many years I have work hard using this wonderful practical test to help in diagnosis of many tumoral, inflammatory and metabolic cutaneous diseases. We can do a rapid diagnosis of a skin tumor and begin the treatment immediately. I have used two type of smear stain (GIEMSA and PAPANICOLAOU). We can differentiate between Pigmented Basal Cell Carcinoma to Melanoma or Pigmented Bowen Disease using Tzanck Test. It is not overcome skin biopsy but has a complementary effect. I was useful in Dermatologic Surgery when we can examine a deeper margin of a surgical ulcer before stop the surgery and seek atypical cells in that place. I would like to show at the Congress how to do this practical method and some case can be showed to demonstration usefulness of this test.
Biography
Thomas Paulo MD, has completed his graduate course in Medicine at the age of 25 years from Federal University of State of Rio Grande do Norte and completed (one year) Residency in Internal Medicine in Rio de Janeiro and Dermatology at University of Sao Paulo - Brazil (USP) for two years (1984 and 1985). He concluded his Board in Dermatology in 1986 by Brazilian Society of Dermatology. He is Assistant Professor of Dermatology since 1994 at Department of Dermatology of the Rio Grande do Norte University in Northeast of Brazil. He has a Board in Hansen Disease at Brazilian Association of Leprosy. He is a teacher in Dermatopathology to Residents Doctors at Department of Dermatology. He has made a short Atlas of Cutaneous Cytology in 2009 with support from Galderma Laboratories and had published some articles about Cutaneous Cytology and Skin Cancer on Dermatology Online Journal in March 2007 about Diffuse Melanosis in Malignant Melanoma. He has written half of chapters at two more famous books of Dermatology in Brazil in sections about Diagnosis Tests in General Dermatology.
