Cleft lip and palate can take make forms: bilateral or unilateral, complete or incomplete. Many cases of cleft lip are diagnosed before birth using advanced imaging tests, and the pediatric plastic surgeons in The Children’s Hospital of Philadelphia’s Cleft Lip and Palate Program do more than 1,000 surgeries related to cleft lip and palate each year. Oksana Jackson, M.D., Jesse Taylor, M.D., and Nahla Khalek, M.D., describe the treatments available and the role CHOP's Center for Fetal Diagnosis and Treatment plays in patients’ care.

This is a 10 months old baby with an isolated cleft of hard and soft palate. This is one of the milder versions of the palatal clefts. To close this type of cleft, an intraoral approach is done where in a vomerine flap is raised. The nasal layer is raised. Through the incision on the hard palate, a palatal flap is also raised. The greater palatine artery is exposed. At this juncture, the nasal mucosa is carefully separated. An osteotomy of the greater palataine foramen and canal is carried with great dexterity to prevent post operative tension that could lead to potential problems. The medial and lateral flaps of the mucogingival flaps are also carried out. Using a Howarth Elevator, the palatal mucosa is elevated. Care is exercised not to disturb the tooth buds. While operating in the opposite or the contra lateral side, the left hand is used for incising. Hence this surgery would be best performed by an ambidextrous surgeon. The nasal mucosa is sutured to create an ideal nasal cavity.
The muscle attachments are carefully identified. The leveator palati is detached and the correct attachment is performed. Throughout the procedure, the greater palantine vessels are carefully handled. The muscle sling is established. The greater palatine artery is moved away from the canal to aid in repositioning of the flap.
At the end of this procedure, suturing is commenced from Anterior to Posterior direction then towards the uvula. A horizontal or a vertical mattress type of suture is placed. Dead space between nasal and palatal mucosa should be eliminated. For this reinforcement bite is placed even from the nasal mucosa

Cleft lip and palate
Cleft lip and cleft palate, also known as orofacial cleft and cleft lip and palate, is a group of conditions that includes cleft lip (CL), cleft palate (CP), and both together (CLP). A cleft lip contains an opening in the upper lip that may extend into the nose. The opening may be on one side, both sides, or in the middle. A cleft palate is when the roof of the mouth contains an opening into the nose. These disorders can result in feeding problems, speech problems, hearing problems, and frequent ear infections. Less than half the time the condition is associated with other disorders.
Cleft lip and palate is due to tissues of the face not properly joining together during development. They are a type of birth defect. The cause in most cases is unknown. Risk factors include smoking during pregnancy, diabetes, an older mother, obesity, and certain medications such as some used to treat seizures. They can often be diagnosed during an ultrasound done during pregnancy.
A cleft lip or palate can be successfully treated with surgery. This is often done in the first few months of life for cleft lip and before eighteen months for cleft palate. Speech therapy and dental care may also be needed. With appropriate treatment outcomes are good.

Though they received unwelcome stares and even cruel comments via social media, the Buchanans remained positive, and became Internet sensations when they posted a video on YouTube to share their story. They say that while his disabilities may be inconvenient, they never want Christian — who they lovingly call "Superman" — to feel like he can't do what he wants to do.
