HUMAN ENDOCRINE SYSTEM :PANCREAS :Pancreas is a mixed gland -- A portion of pancreas acts like exocrine gland and another portion acts like an endocrine gland. The cells of endocrine pancreas are called Islets of Langerhangs representing 1 to 2% of pancreatic tissue. They secrete 2 hormones -- Insulin and Glucagon.Insulin : The hormone is produced by β-cells of Islet of Langerhangs. It is a peptide hormone, which plays a major role in the regulation of glucose homeostasis. Insulin promotes the conversion of glucose to glycogen in liver and muscle that is stored in these tissue for future use. Insulin is liberated when the blood glucose levels are high, usually when food is digested and absorbed. If sufficient insulin is not produced glucose level in blood increases and it is excreted through urine. This is called DIABETES MELLITUS. Diabetes is identified by the presence of sugar in urine. ∴ Diabetic patients take insulin injections regularly. Insulin also affects the metabolism of fats and proteins. Unattended diabetis results in wasting of body tissue, reduced healing power, ketone bodies, blurred vision and kidney problems. They may end up with gangrene and coma.Glucagon is a peptide hormone secreted by α cells of islets of Langerhans. Glucagon acts on liver cells and stimulates glyconeolysis resulting in an increased blood sugar -- hyperglycemia. This hormone stimulates the process of gluconeogenesis which also contributes to hyperglycemia. Glucagon reduces the cellular glucose uptake and utilization. Thus glucagon is hyperglycemic hormone.

The Pancreas
The pancreas is shown here with the foreground highly magnified to reveal its inner anatomy. Ninety-nine percent of pancreatic tissue is composed of acinar glands, which secrete an alkaline digestive juice into the duodenum via the pancreatic duct to help digest food. The endocrine areas of the pancreas, known as the islets of Langerhans, are composed of two major types of cell. The alpha cells secrete the hormone glucagon, and the beta cells secrete insulin, into the bloodstream. These hormones work in opposition to control the blood glucose level. Insulin promotes the uptake and usage of glucose in the tissues, particularly skeletal muscle and fat, and reduces glucose production in the liver. Glucagon has an anti-insulin effect in the liver, increasing glucose release. The anatomy of the pancreas also includes: the ampulla of the common bile duct: the pancreatic duct, which carries digestive juice containing enzymes to the duodenum; the acini, the exocrine portions of the pancreas which secrete digestive juice into the pancreas; the splenic arteries which conduct oxygenated blood into the pancreas; and the mesenteric veins, which carry deoxygenated blood containing insulin and glucagon away from the pancreas.

Pancreatic islets
The pancreatic islets or islets of Langerhans are the regions of the pancreas that contain its endocrine (i.e., hormone-producing) cells. Discovered in 1869 by German pathological anatomist Paul Langerhans. New studies show that the pancreatic islets constitute approximately 4.5% of the pancreas volume and receive 10-15% of its blood flow. The islets of Langerhans are arranged in density routes throughout the healthy human pancreas.
Structure
There are about 3 million islets distributed in the form of density routes throughout the pancreas of a healthy adult human, each of which measures an average of about 0.1 mm (109 µm) in diameter.:914 Each is separated from the surrounding pancreatic tissue by a thin fibrous connective tissue capsule which is continuous with the fibrous connective tissue that is interwoven throughout the rest of the pancreas.:914 The combined mass of the islets is 2 grams. Islets of Langerhans can also form super structures called Islet clusters which are composed of small islets that surround large blood vessels. The roundness of islets along the pancreas has also been quantified through the Index of Sphericity method. Thus, the islets closest to the spherical form are mainly found in the pancreas tail whereas the islets farthest from the spherical form are found in the pancreas neck.

Ninety-nine percent of pancreatic tissue is composed of acinar glands, which secrete an alkaline digestive juice into the duodenum via the pancreatic duct to help digest food. The endocrine areas of the pancreas, known as the islets of Langerhans, are composed of two major types of cell. The alpha cells secrete the hormone glucagon, and the beta cells secrete insulin, into the bloodstream. These hormones work in opposition to control the blood glucose level. Insulin promotes the uptake and usage of glucose in the tissues, particularly skeletal muscle and fat, and reduces glucose production in the liver. Glucagon has an anti-insulin effect in the liver, increasing glucose release. The anatomy of the pancreas also includes: the ampulla of the common bile duct: the pancreatic duct, which carries digestive juice containing enzymes to the duodenum; the acini, the exocrine portions of the pancreas which secrete digestive juice into the pancreas; the splenic arteries which conduct oxygenated blood into the pancreas; and the mesenteric veins, which carry deoxygenated blood containing insulin and glucagon away from the pancreas.

The cells of endocrine pancreas are called Islets of Langerhangs representing 1 to 2% of pancreatic tissue. They secrete 2 hormones -- Insulin and Glucagon.Insulin : The hormone is produced by β-cells of Islet of Langerhangs. It is a peptide hormone, which plays a major role in the regulation of glucose homeostasis. Insulin promotes the conversion of glucose to glycogen in liver and muscle that is stored in these tissue for future use. Insulin is liberated when the blood glucose levels are high, usually when food is digested and absorbed. If sufficient insulin is not produced glucose level in blood increases and it is excreted through urine. This is called DIABETES MELLITUS. Diabetes is identified by the presence of sugar in urine. ∴ Diabetic patients take insulin injections regularly. Insulin also affects the metabolism of fats and proteins. Unattended diabetis results in wasting of body tissue, reduced healing power, ketone bodies, blurred vision and kidney problems. They may end up with gangrene and coma.Glucagon is a peptide hormone secreted by α cells of islets of Langerhans. Glucagon acts on liver cells and stimulates glyconeolysis resulting in an increased blood sugar -- hyperglycemia. This hormone stimulates the process of gluconeogenesis which also contributes to hyperglycemia. Glucagon reduces the cellular glucose uptake and utilization. Thus glucagon is hyperglycemic hormone.

Pancreatitis: acute pancreatitis and chronic pancreatitis nursing NCLEX review lecture on symptoms, treatment, pathophysiology, diet, and nursing interventions. Pancreatitis is inflammation of the pancreas that can lead to the digestion of the pancreas by its own enzymes and/or irreversible structural damage to the organ.
There are two types of pancreatitis: acute pancreatitis and chronic pancreatitis. In acute pancreatitis, there is sudden inflammation of the pancreas due to something that has triggered the digestive enzymes to become activated inside the organ (high amylase and lipase levels will be found in the blood) and the pancreas starts to digest itself and swell. Two common causes of acute pancreatitis include: gallstones obstruction and heavy consumption of alcohol. Typically, acute pancreatitis is reversible if the patient is treated promptly.
Chronic pancreatitis is chronic inflammation of the pancreas (can be caused by repeated episodes of acute pancreatitis but most common cause is years of alcohol abuse) that has led to irreversible damage to the structure of pancreas. There will be loss of the function of the endocrine and exocrine cells (acinar and islet of langerhans cells), damaged to the pancreatic duct etc.
Symptoms of pancreatitis include: sudden, very painful mid-epigastric or left upper quadrant which can be felt in the back as well, fever, increased heart rate, decreased blood pressure, Cullen's sign, Grey-Turner's sign, hyperglycemia, dark-urine, jaundice, steatorrhea (oily/fatty stools).
What is Cullen and Grey-Turner's Sign? Cullen's Sign is a bluish discoloration around the belly button, whereas Grey-Turner's Sign is bluish discoloration found on the flanks of the abdomen. They are due to retroperitoneal bleeding. Watch the video for an easy way to remember how to tell them apart.
Nursing interventions for pancreatitis include: maintain NPO status, inserting/monitor NG tube, controlling pain, administering pancreatic enzymes and stomach acid reducers per MD order, assessing urine, hydration status, diet education etc.

ระบบต่อมไร้ท่อ เป็นระบบที่สำคัญระบบหนึ่งของร่างกาย ทำหน้าที่ควบคุมอวัยวะภายใน ร่างกายให้ทำงานประสานกัน โดยอาศัยสารเคมีที่เรียกว่า ฮอร์โมน ซึ่งฮอร์โมนจะถูกขนส่งไปสู่ อวัยวะทั่วร่างกาย แต่จะออกฤทธิ์หรือมีผลต่ออวัยวะและเซลล์เป้าหมาย
การแบ่งประเภทต่อมไร้ท่อก็จะแบ่งได้เป็น
1. ต่อมใต้สมอง มี 2 ส่วน ดังนี้
1.1 ต่อมใต้สมองส่วนหน้า (anterior pituitary gland) ผลิตฮอร์โมนดังนี้
1. FSH (follicle stimulating hormone)
2. LH (luteinizing hormone)
3. PRl (prolactin)
4. TSH (thyroid stimulating hormone)
5. ACTH (adrenocorticotropin hormone)
6. endorphin
7. GH (growth hormone)
1.2 ต่อมใต้สมองส่วนหลัง(posterior pituitary gland) ผลิตฮอร์โมนดังนี้
1. Oxytocin
2. ADH (antidiuretic hormone or vasopressin
2. ต่อมไพเนียล (pineal gland)
3. ต่อมไทมัส (thymus gland) ทำหน้าที่สร้างฮอร์โมน Thymosin ไปกระตุ้นต่อมไทมัสให้พัฒนาเม็ดเลือดขาวที่ชื่อ T-cell
4. ต่อมไทรอยด์ (thyroid gland) อยู่บริเวณคอสร้างฮอร์โมนดังนี้
4.1 Thyroxin
4.2 Calcitonin
5. ต่อมพาราไทรอยด์ (parathyroid gland)
6. ต่อมหมวกไต (adrenal gland) มี 2 ชั้น
6.1 ต่อมหมวกไตชั้นนอก (adrenal cortex)
1. Glucocorticoid เช่น cortisol
2. Mineralocorticoid เช่น Aldosterone
3. sex hormone พวกฮอร์โมนเพศชาย (androgen) และเพศหญิง (estrogen)
6.2 ต่อมหมวกไตชั้นใน (adrenal medulla)
7. ตับอ่อน (Pancreas) มีทั้งส่วนที่เป็นต่อมมีท่อและต่อมไร้ท่อ ส่วนที่เป็นต่อมไร้ท่อเรียก Islet of Langerhans ซึ่งสร้างฮอร์โมน 2 ชนิด คือ
1. Insulin ทำหน้าที่ลดระดับน้ำตาลในเลือดโดยทำให้น้ำตาลเปลี่ยนเป็นไกลโคลเจนเก็บสะสมในตับ และเอาน้ำตาลเข้าสู่เซลล์ หากขาดฮอร์โมนนี้จะเป็นโรคเบาหวาน (diabete mellitus)
2. Glucagon ทำหน้าที่เพิ่มระดับน้ำตาลในเลือดโดยทำให้ไกลโคลเจนในตับสลายเป็นกลูโคส
8. ไต (kidney) สร้างฮอร์โมน Erytropoietin ทำหน้าที่กระตุ้นให้สร้างเซลล์เม็ดเลือดแดงซึ่งฮอร์โมนนี้จะมีบทบาทสำคัญเวลาเราไปอยู่ในที่สูง ซึ่งมีระดับออกซิเจนที่ต่ำกว่าตอนอยู่ด้านล่าง
9. ฮอร์โมนจากต่อมเพศ (Gonad sex hormone)
1. ฮอร์โมนเพศชาย ได้แก่ เทสโทสเตอโรน (Testosterone)ทำหน้าที่ควบคุมลักษณะเพศชาย เช่น เสียงที่แตกพร่า การมีกล้ามเนื้อ
2. ฮอร์โมนเพศหญิง ได้แก่ ฮอร์โมน เอสโตรเจน(Estrogen) ทำหน้าที่ซ่อมแซมผนังมดลูกหลังมีประจำเดือนและควบคุมลักษณะของเพศหญิง เช่น การมีหน้าอก การที่เอวคอด และฮอร์โมน โปรเจสเตอโรน(Progesterone) ทำหน้าที่ทำให้ผนังมดลูกหนาเหมาะแก่การฝังตัวของตัวอ่อน
