In this video I'm going to talk about what is thiamine and how it's beneficial. If someone doesn't have enough of this vitamin it can cause something known as beriberi.
Beriberi can lead to an increased heart rate or heart failure. It can also negatively affect nerves and muscles leading to a bunch of problems.
A gentleman named William Fletcher in 1905 discovered something in the husks of rice that prevented this disease and can you guess what that was? Yes it was thiamine and that was how it was uncovered. You might know it by the name of vitamin B1.
In 1912 a gentleman named Casimir Funk took Fetchers findings as well as evidence that oranges prevented scurvy and created the term vitamin. B1 being it was the first B discovered.
Vitamin B1 is a water-based vitamin and this means we need a continuous supply of it because it gets flushed out of our system daily. A large way the body uses this substance is to convert carbohydrates into energy. It also plays a role in creating hydrochloric acid for your digestive system. This acid is so strong that if it wasn't for mucous membranes that protect the stomach lining it would digest the stomach.
Next up this vitamin assists in controlling the flow of electrolytes in and out of the cells of the nerves and muscles. This is why if someone doesn't consume enough of this vitamin it can negatively affect the heart rate and lead to fatigue, trouble with thinking and even breathing difficulties.
Apart from needing this vitamin just to function normally, it can also provide some nifty health benefits like helping with depression. Scientist at the Shanghai Institutes for Biological Sciences looked at 1,500 men and women between the ages of 50-70. Then they used questionnaires to determine whether the participants had depressive tendencies and measured the amount of thiamine in their diet. The result was that the participants with the lowest thiamine intake were one and a half times more likely to suffer from depression.
If that wasn't enough there is some evidence that it can prevent cataracts. According to the U.S. National Library of Medicine a study of 2,9000 Australian men and women discovered that those who where to the top one fifth of thiamin intake were 40% less likely to have nuclear cataracts than the one fifth of participants that took the least.
To sum everything thiamine is a water-based vitamin that converts carbohydrates into energy, helps digestion, assists with the health of nerves, prevents depression and cataracts.

Then a little later in time this substance received the name vitamin b1 or thiamine. Do you know why this vitamin was called B1? It is because it was the very first B vitamin discovered.
You might be wondered what is so bad about beriberi right? Beriberi can either affect the cardiovascular system known as wet beriberi or it can attack the nervous system and that is known as dry beribery.
If beribery affects the nervous system all kinds of problems can develop. According to Healthgrades.com There could be problems with walking, coordination, slurred speech, confusion or memory problems. Next a person could feel tingling in the hands or feet, uncontrolled eye movement and possibly vomiting.

Hi, I'm Dr. Jared Nielsen. I'd like to talk today about the importance of a vitamin called thiamine or B1. B1 is very, very important for liver function, especially for people who have a history of alcoholism. It's quickly depleted in the system and is essential for that person to be repleted, if you will, their B1 vitamin.
There are a lot of people that don't think, "I am really an alcoholic or I don't really have a drinking problem." What's interesting is they'll start to manifest some of the symptomatology of B1 deficiency, peripheral neuropathies, sensitivity or hyper‑sensitivity in the legs and feet.
These are very commonly overlapped with diabetics. Diabetics also would get that type of symptomatology as the liver becomes congested or a fatty infiltrated liver.
When we look at blood labs, a lot patients will actually say, when we see cholesterol levels going up and triglyceride levels matching or even exceeding that they don't drink alcohol.
What we see a lot of times is that they're chronic soda pop abusers. Without naming specifically different brands or types of soda drinks, what we find is people who have a daily soda pop actually will start to also have fatty infiltrated livers and their thiamine levels can start to go down.
Equally is important is when we have patients that have eating disorders that begin to experience metallic taste in their mouth is associated with the gall bladder but it also as an indication of thiamine or B1 deficiency...

Thiamine (Vitamin B1) deficiencies are primarily seen in alcoholic patients. This scenario is so common on Step 1 that anytime you see alcoholism in a vitamin question, your first thought should be thiamine deficiency. Thiamine is found in a lot of different foods, so you usually only see deficiencies in people who are malnourished or getting a majority of their calories from alcohol. Deficiencies of Thiamine primarily affect 3 different areas; the CNS (Wernicke-Korsakoff Syndrome), PNS (Dry Beri-Beri), and the cardiac system (Wet Beri-Beri). Damage done to the cardiac system and the PNS can be reversed with treatment, but damage done to the CNS is permanent.
Thiamine is an important cofactor in multiple processes that generate ATP from glucose sugar. Therefore, deficiencies are worsened by sugar intake (in the absence of thiamine administration) because the little bit of Thiamine left in the system is used up to metabolize the sugar.
Wernicke-Korsakoff Syndrome is the collection of CNS damage that results from Thiamine deficiency. It includes amnesia (memory loss), confusion, ataxia (unsteady gait), and visual disturbances due to uncoordinated eye movement. The confusion and memory loss can lead to confabulation in which the patient has bizarre explanations for things like how they got injured. Wernicke-Korsakof syndrome characteristically affects the Mammillary Bodies and the surrounding structures (3rd/4th ventricle & aqueduct). Infarcts or hemorrhages in these areas can be seen on radiologic imaging.
Dry BeriBeri is damage to the peripheral nervous system (PNS) that results from Thiamine deficiency. It causes muscle wasting, weakness, and diminished peripheral sensation. Wet BeriBeri is the cardiac effects of Thiamine deficiency. It leads to arrhythmias and/or heart failure by affecting the electrical system of the heart. Edema, dyspnea, increased cardiac output, and cardiac dilation can result.
Niacin (Vitamin B3) is part of NAD+ and NADP+ which carry electrons in redox reactions. Vitamin B3 is made from tryptophan using a vitamin B6 cofactor. Therefore, Niacin deficiency can be the result of Vitamin B6 deficiency or tryptophan deficiency (Hartnup Disease or Carcinoid Syndrome).
Niacin deficiency is called Pellagra. Pellagra is characterized by the "4 Ds" Diarrhea, Dementia, Dermatitis, and Death. Niacin is one way to treat high cholesterol. Niacin toxicity can result if a patient receives too high of a dose. This causes the side effect of Facial Flushing (red face with the sensation of warmth).
Pyridoxine (Vitamin B6) is an essential coenzyme in many metabolic reactions. Deficiencies can cause peripheral neuropathy (numbness & tingling), seizures, anemia & irritability. Deficiencies of Vitamin B6 can also lead to deficiencies of Vitamin B3, because B6 is a cofactor in the pathway that converts Tryptophan to Vitamin B3. The most common cause of Vitamin B3 deficiency is Isoniazid (TB drug) Treatment. Therefore, some patients receiving Isoniazid are given B6 vitamins prophylactically.
Vitamin C (Ascorbic Acid) is involved in the hydroxylation of certain amino acids in collagen that are important for crosslinking individual pro-collagen chains into a triple helix. Without this hydroxylation the collagen is very weak. Therefore, deficiencies of Vitamin C cause poor wound healing, gum abnormalities (pain, bleeding & loose/missing teeth) & bruising (purpura, ecchymosis, perifollicular hemorrhages). Scurvy results from not eating enough fruit and other fresh foods.
