It is not yet clear whether CT urography is having any durable beneficial effect on patient morbidity or mortality (with respect to survival with urothelial cancer). Nonetheless, we believe that several observations validate the utility of CTU. First, CTU can be used to survey the upper tracts prior to cystoscopy, to determine whether or not ureteroscopy will also be needed. Second, CTU prior to cystoscopy can identify some bladder cancers. Patients in whom bladder cancers have been detected in this fashion can proceed directly to rigid cystoscopy with attempted trans-urethral resection, rather than having a preliminary diagnostic flexible cystoscopy first. Third and finally, on some occasions CTU can lead to early aggressive intervention, perhaps leading to treatment of some urothelial neoplasms before they have become invasive.
