Once you get the hang of it, you'll find it a really useful skill to have. Wash your hands before you start. Find a suitable wide-rimmed container for your milk, such as a jug. Either sterilise the container in a steriliser, or wash it well and scald it with boiling water. Make yourself comfortable in a warm room, where you won't be interrupted. Put the container within reach. Start by massaging your breasts, stroking downwards, towards the nipple, for a minute or so. Place your little finger under your breast, against your ribs, and spread your other fingers slightly, to support your breast. Your thumb needs to be on top, about 3cm (1.2in) or 4cm (1.6in) back from the base of your nipple. Adjust your fingers, if you need to, so that your first finger and thumb are opposite each other, making a big C-shape around your breast. Your milk comes from deep within your breast, so your finger and thumb need to be well away from your nipple. Place them towards the edge of the darker skin surrounding your nipple (areola). Feel for a change in texture from the softness of your nipple and areola to a slight ridge. Then put your thumb and fingers behind this ridge, and start expressing. If you have a large areola, you may need to bring your fingers inside it. Gently but firmly squeeze your thumb and first finger together and then release, but keep your finger and thumb in the same position. Repeat the squeezing and releasing again and again, building up a rhythm. Keep the rhythm up, and at some point, you'll see drops of colostrum or breastmilk appearing at your nipple. Be patient, as it may take a minute or two to appear. Keep on squeezing and releasing. You may just get a few more drops, or your milk may start to flow freely. Sometimes, it takes practice and time to get a good flow going. Once you've spotted some drops of milk, adjust your position, so you can collect it. If the flow slows, rotate your hand slightly, as if moving around a clock-face, and try a different section of your breast. Try to work around all areas of your breast before you change breasts. You can keep on moving your hand around your breast to squeeze a different section, and keep swapping from one breast to another. Do this until the milk stops, or you've got as much as you want. Some women find that hand-expressing works better if they press their whole hand back and in towards their rib cage, just before they squeeze. Here are some more tips to help you: Avoid sliding your thumb or fingers over the skin of your breast, as this won't stimulate the flow of milk and will squeeze the breast tissue inside. You may need to move your thumb and fingers further towards, or away from, the nipple. We're all different shapes and sizes, so find what works best for you. If your fingers feel tired, use your other hand, or try your other breast. Once you’ve had a bit of practice, your milk may spray out. If this is the case, try collecting your milk using a funnel inside the container. Scald the funnel first with boiling water, though. It takes time to learn how to express by hand, but keep going. It costs nothing, and is especially useful if breastfeeding is challenging in the early days, perhaps because your baby was premature or is unwell. In this case, the earlier you start expressing, the greater the volume of milk you'll be able to produce.
