Ganglion cysts are fluid filled cysts that are benign (not cancerous) and will not spread. Ganglion cysts may vary in size from time to time, and it may grow, but it usually does not cause any harm, and the patient is usually more concerned about how the cyst looks. When these cysts pressure a nerve, an artery, or a tendon it can cause a problem. Ganglion cyst of the wrist is usually classic. This type of cyst transilluminates, therefore an MRI is usually not needed. If an MRI is done for whatever reason, it will usually show a fluid signal inside the cyst. The pterygopalatine ganglion is one of four parasympathetic ganglia of the head and neck and largest of the peripheral parasympathetic ganglia. It receives parasympathetic fibres from the facial nerve.

Although the actual causes are unknown, several risk factors have been associated with the development of ganglion cysts:
First of all, they are more frequent in women aged 20-30.
Also, they are more frequently associated with osteoarthritis, as well as with previous injuries or the joints or tendons.
They seem to occur over existing tears – whether caused by significant trauma, or just micro-tears that happen during the normal use of the foot. Although they are not dangerous, they can be scary and uncomfortable. Watch as Dr. Carter drains a patient's ganglion cyst!

Ganglion cysts may vary in size from time to time, and it may grow, but it usually does not cause any harm, and the patient is usually more concerned about how the cyst looks. When these cysts pressure a nerve, an artery, or a tendon it can cause a problem. Ganglion cyst of the wrist is usually classic. This type of cyst transilluminates, therefore an MRI is usually not needed. If an MRI is done for whatever reason, it will usually show a fluid signal inside the cyst. Ganglion cysts of the wrist can be either dorsal or volar. The dorsal type of ganglion cyst is common. The volar type is rare. The volar ganglion could compress the radial artery this might compromise the circulation in the hand. The dorsal type arises from the scapholunate articulation. The volar type arises from the radiocarpal joint, which is the wrist joint itself. A bump or a mass that is well defined, localized, smooth, and not attached to the skin. The dorsal ganglion cyst is more obvious with flexion of the wrist. The volar ganglion cyst is more obvious with extension of the wrist. This ganglion cyst of the wrist is usually asymptomatic, and the patient is usually more concerned about the looks and the cosmetics. When there is a volar ganglia of the wrist, check the nerves of the hand and do the Allen’s test to see if this cyst is compromising the circulation. Ultrasound is a good study to see the relationship between the cyst and the radial artery in volar ganglion cyst. Ultrasound may be needed during aspiration of the ganglion cyst because the ganglion is near the artery. Surgery is usually done for the ganglion cyst if there are severe symptoms, neurovascular compromise, or if the patient complains of pain. High recurrence rate with excision of the volar ganglion cyst (about 20%). Do the Allen’s test to check the blood flow of the hand before doing excision of the volar ganglion cyst.

The smooth functioning of the human body depends on coordination between somatic and visceral parts of the nervous system. Visceral part of the nervous system is commonly known as the autonomic nervous system. It has two components parasympathetic and sympathetic. Visceral efferent has at least two sets of neurons, i.e. preganglionic and postganglionic neurons.
Fibers of cranial parasympathetic ganglion originate from brain stem nuclei. After leaving the brainstem, these fiber synapses in the peripheral parasympathetic ganglion. There are four peripheral parasympathetic ganglia (ciliary ganglion, pterygopalatine ganglion, otic ganglion, and submandibular ganglion) situated in the head and neck region. Postganglionic parasympathetic fibers provide parasympathetic innervation to the target organ.
Ciliary ganglion is a peripheral parasympathetic ganglion. It is situated near the apex of orbit between the optic nerve and lateral rectus muscle. It is related medially to the ophthalmic artery and laterally to the lateral rectus muscle.

Generally, an MRI is used to attempt to determine an appropriate diagnosis, but one must take into account that MRI is only effective about 50% of the time. MRI can be useful in finding other related problems, however, such as cartilage and ligament tears.
Some may aspirate a ganglion cyst, but recurrence rates are high, so surgical excision is the best bet. A small incision is made in the wrist above the cyst, and the whole thing, including the trunk, is removed from the scapholunate ligament in the joint of the wrist. The whole procedure takes only 15 minutes, and after a few days of light splinting, activity may be resumed, and should be to avoid stiffness developing. In some cases, arthroscopic surgery may be necessary to determine if damage to the ligament or cartilage, and in this case recovery is slightly longer, up to six weeks. Ganglions may recur in about 5% of cases, but surgical removal keeps this number very low.

Although the actual causes are unknown, several risk factors have been associated with the development of ganglion cysts:
First of all, they are more frequent in women aged 20-30.
Also, they are more frequently associated with osteoarthritis, as well as with previous injuries or the joints or tendons.
They seem to occur over existing tears – whether caused by significant trauma, or just micro-tears that happen during the normal use of the foot. Although they are not dangerous, they can be scary and uncomfortable. Watch as Dr. Carter drains a patient's ganglion cyst!
