Low Birth Weight, Childhood Infections May Be Associated With Later Development Of Ankylosing Spondylitis.
Nancy Walsh writing for MedPage Today reported that research indicated that “low birth weight and childhood infections were associated with the later development of ankylosing spondylitis.” The study also indicated that “individuals who were diagnosed with ankylosing spondylitis more often had older siblings, which is often considered a proxy for greater exposure to childhood infections...said” Ulf Lindstrom, PhD, at “a press conference at the annual meeting of the European League Against Rheumatism.”

Prednisone May Contribute To Herpes Zoster Risk In Patients With RA.
Nancy Walsh writing in MedPage Today reported that research suggests that individuals “with rheumatoid arthritis (RA) have an elevated risk for herpes zoster, particularly with increasing age, but the only immunomodulating medication that contributes to the risk is prednisone.”
The findings were published in Arthritis Care and Research.

Significant Weight Loss After Bariatric Surgery May Be Linked To Improved Disease Control, Lower Markers Of Inflammation In Patients With RA.
Frieda Wiley writing in MedPage Today reported that research published in Arthritis Care and Research indicated that “significant weight loss” after “bariatric surgery was associated with improved disease control, lower markers of inflammation, and a decrease in the need for medication among patients with rheumatoid arthritis.” Investigators found that one “year after the surgery, patients’ average weight loss was 41 kg, which was a mean loss of 70% excess weight, and RA disease activity had significantly improved.”
Comment: Fat cells produce cytokines, inflammation drivers. Removal of fat takes these cytokines away.

If a patient is in clinical remission from their RA, don’t taper their biologics until you do this one test…
Don't taper RA meds before checking Doppler for synovitis
Reported in MedPage Today, Doppler-detected synovitis is strongly associated with failure of biological therapy (BT) in patients with rheumatoid arthritis (RA) who are in sustained clinical remission, a new prospective observational study has found. Baseline clinical remission as determined by the Disease Activity Scale in 28 joints (DAS28) and the global score of Doppler synovitis were independent predictors of BT tapering failure at 6 and 12 months, with the presence of Doppler synovitis being the strongest predictor for tapering failure, according to Esperanza Naredo, department of rheumatology, faculty of medicine, Universidad Complutense in Madrid, Spain.
Comment: Doppler ultrasound is the rheumatologist’s stethoscope.

Anakinra an effective option in complex gout
Reported in MedPage Today, anakinra (Kineret) is an effective treatment option for acute gout in critically ill patients, who traditionally are difficult to treat because of contraindications to standard therapy. In 13 critically ill hospitalized patients treated with anakinra for 20 episodes of acute gouty arthritis, half had responses within 24 hours, another 40% had responses by 48 hours, and the remaining 10% by 72 hours, report researchers from Minnesota Medical School, St. Paul, in Seminars in Arthritis and Rheumatism.
Comment: Every so often, a patient needs this treatment. Glad it’s available.

Genetic Factors May Play Role In Development Of Knee OA-Related Pain.
Nancy Walsh writing in MedPage Today reported that according to the results of a 219-patient study published in the Annals of the Rheumatic Diseases, “genetic factors appear to play a role in the development of osteoarthritis (OA)-related knee pain, with higher rates of worsening knee pain being identified in the offspring of parents who had knee arthroplasty.” Meaning knee replacement.
Comment: Not a surprising finding since knee OA does tend to run in families.

Is shingles vaccine safe for arthritis patients on biologic therapy?
No Extra Risk With Zoster Vax in RA Patients on Biologics
Kate Johnson writing for MedPage Today reported herpes zoster vaccination appears to be safe in patients treated with biologic therapies for rheumatoid arthritis (RA), psoriatic arthritis (PsA) and ankylosing spondylitis (AS), despite widespread recommendations against this practice, a new study suggests.
Among 176 patients vaccinated against HZ while on biologic therapy, a protocol of withholding one dose of biologic therapy and replacing it with the vaccine has resulted in no complications of disseminated HZ disease and no increased incidence of HZ in the 6-week, post-vaccination window.
The practice "appears safe using this protocol," said Stephen Lindsay, MD, Chief of Rheumatology at the Ochsner Clinic.

Gout May Protect Against Alzheimer's Disease
Kate Johnson writing for MedPage Today reported new research suggests that patients diagnosed with gout face a lower risk of developing Alzheimer's disease (AD) compared with people without gout.
"Our findings provide the first population-based evidence for the potential protective effect of gout on the risk of AD, and support the neuroprotective role of uric acid," said Hyon Choi, MD, the study's senior investigator, and director of epidemiology at Massachusetts General Hospital.

Silica and Smoking Synergistically Boost RA Risk
Kate Johnson writing for MedPage Today reported whereas smoking and silica exposure each significantly boosted the likelihood of developing anti-citrullinated protein/peptide antibody (ACPA) positive rheumatoid arthritis, the risk was turbo-boosted more than eight-fold when both factors are combined, a new study showed.
What's worse, the smoking-associated risk appeared hard to reverse in silica-exposed individuals, with increased rates of RA persisting up to 10 years after smoking cessation.
"Silica dust may remain in the lung permanently, and since it might take 10 years for the diminishment of the smoking-silica interaction and even longer for the main effect of smoking to return to baseline, it is yet another reason to advise silica-exposed persons not to start smoking, to smoke less, and to quit smoking as early as possible," said Xia Jiang, BSc, from the Karolinska Institute in Stockholm, Sweden.
Comment: Not good news at all.

Research Raises Questions About Innovations To Devices Used In Joint Replacement.
Nancy Walsh writing in MedPage Today reported that a review published in BMJ suggests that “innovations to devices used in joint replacement have been introduced without sufficient evidence that they are any better than existing implants.” Researchers found that “for one type of new device technology, hip replacements with modular femoral necks, the cumulative rates of revision surgery at 5 and 10 years were 7.4% and 10.8%, compared with rates of 3.7% and 6.4% for conventional prostheses with nonmodular necks.” This “represented a near doubling of the adjusted 10-year revision risk.
Comment: Not all progress is forward.

MedPageToday: Водещ американски учен, иска облекчен режим за регистрацията на Cytisine (Tabex) в САЩ.
Nancy A. Rigotti, MD, of the Tobacco Research and Treatment Center at Massachusetts General Hospital and Harvard in Boston, agreed in an editorial accompanying the NEJM paper.
However, getting the drug to the U.S. market could be a challenge, Rigotti told MedPage Today.
The manufacturer in Bulgaria never saw a market for the drug in the U.S. and so never tried for approval, she explained.
"We're sort of stuck here," she said in an interview. "We need to find a sponsor for the drug who can take it through the licensing process, but once it went through the licensing process that sponsor would probably make it more expensive. So we might end up with another drug that is just as expensive as others."
She called instead for "creative collaboration" among regulators, pharma, research funding agencies, and other stakeholders interested in the public health benefit of smoking cessation in order to find some novel pathway through the system for the drug.

While retractions from scientific journals are the "nuclear option" in self-correction, they are a compelling window into the process of peer review and publication. In a review of the first four years of Retraction Watch -- http://retractionwatch.com, which has been called "one of the best innovations in science in recent years" by former BMJ editor in chief Richard Smith -- speaker Ivan Oransky will discuss the reasons why papers are retracted, how often fraud occurs, whether journals are quick or slow to respond, and other issues related to scientific integrity.

Welcome.
I’m going to begin by this installment by reading an excerpt from a news report that’s posted on the website MedPage Today. It states…Young American Indians and Alaska Natives (AIAN) have much higher rates of suicide than those of other racial and ethnic groups, according to findings of a new federal report.
The findings, published by the Center for Disease Control and Prevention's Center for Health Statistics, found that young AIAN men were more than twice as likely to commit suicide as other gender or racial groups studied. Among AIAN young adults, there were an estimated 34.3 suicides per 100,000 population for males and 9.9 suicides for females.
The report noted that suicide rates for AIAN young adults are likely to be underestimated as a previous study found that deaths for the AIAN population were under-reported by 30%.
When researchers combined data from 2009 through 2013 for non-Hispanic black and non-Hispanic white young adults who committed suicide, firearms were the most common method used, followed by suffocation. For Hispanic, Asian/Pacific Islander, and AIAN young adults who committed suicide, suffocation was the most common method used, followed by the use of firearms.
Joining me to discuss the crisis in greater detail is Pastor Leon "Blunt Horn" Matthews, author of "Rez Ramblings: Living on the Pine Ridge as a 21st Century Injun". The book is a collection of the pastor’s newspaper columns and personal blogs spanning the past several years and provides an in-depth look at the stark realities of poverty, oppression, racism, and cultural struggles of Native Americans.
Pastor Matthews has appeared on the show on two occasions to reveal how alcoholism, suicides, and unemployment have decimated the Pine Ridge Indian Reservation in South Dakota.
The interview was conducted on October 8, 2015.
