OK, this is a GREAT pilar cyst excision on the scalp of one of my longstanding patients who is so dear to my heart!
She has had this bump on her scalp for many years now, and would love it removed, but wasn't sure what was involved in this process, so she really left it along for many years.
She was happy to learn that it is a pretty simple process, and is just a procedure under local anesthesia.

Multiple Pilar Cysts - #TBT
This is a lovely patient of mine who travelled from a little distance to see me. She knew I did segments for The Doctors TV show, but this was before DrPimplePopper got big. She had a couple big bumps on her scalp, which are pilar cysts.
Pilar cysts tend to occur in families, and many people can have more than one. They most commonly occur on the scalp and have a thicker wall than an epidermoid

She is so sweet to let us film the removal of her pilar cysts. She has a few of them, and this one is the biggest. They are benign, but they are bumpy and can be embarrassing. They are fun to remove because they are a challenge to pop out whole but usually behave in general and don't cause too many problems. And they always look bigger then you think when you take them out! Enjoy!

Two GREAT pilar cysts excised by Dr Rebish!
PIlar cysts are benign growths most commonly seen on the scalp. They are a little different than epidermoid cysts which are more often found on the body. Pilar cysts usually have thicker walls, so they are more easily popped out whole.
As you can see, Dr Rebish and I have different methods for removing cysts, and this is expected since we trained at different dermatology residency programs.

This particular pilar cyst excision will go down on My Favorites short list. This gentleman travelled from Hollywood to see me, and he was dressed to the nines, but very, very nervous. He admits he gets "white coat syndrome" and hates to see doctors. However, he saw me on The Doctors TV show, and he felt that I might be the one to remove this bump on his scalp.
As you can see he is pretty nervous but my favorite part was seeing his face right after I removed this cyst. He was smiling to himself and I could tell that he still couldn't believe that he went through with the surgery. He actually only had an appointment scheduled for a consult, but I could sense that he really wanted this bump removed and would probably chicken out and never come back, so luckily I had time enough to do it right then and there!
Also, there are parts of this video that are muted, in order to protect his privacy and the privacy of others.
Interestingly, I used a "suspension suture" to help me to extract this pilar cyst. The walls of a pilar cyst are usually very smooth, reminds me of an olive. I couldn't grasp the cyst with my forceps, so I decided to place a suture into it so I could pull it out.
As you know, pilar cysts are benign growths that occur predominantly on the scalp, and usually have thicker walls than the more common epidermoid cysts. This makes them easier to pop out whole. This bump was very noticeable on his scalp so I think the removal will make him very happy, indeed.

In this video, I point out a distinctive sound that can be heard when doing surgery on the skin of the scalp. Under the skin and fat, just before you get to the bone of the skull, there is a thin area of tissue called the galea aponeurosis. When you tease and undermine away in this surgical plane, the tissue makes a distinctive noise.. see if you can hear it. Thought you may find this interesting. Also, this is the ideal surgical plane for closure. Although the scalp is very vascular and often bleeds, blood vessels don’t course through this surgical plane, so there is very low risk of bleeding when I am undermining the skin at this level.

They tend to be more mobile and firmer than epidermoid cysts, and are derived from a different part of the hair follicle/skin. The internal content tend to be more “homogenous”/ smooth (looks different under the microscope), but it is also a benign growth. It is not uncommon to have multiple pilar cysts on the scalp and to have other family members to have these cysts (tend to run in families). Treatment is for cosmetic reasons usually (people don’t like the appearance of them), and excision with removal of the cyst and it’s contents is curative.

OK, this is a GREAT pilar cyst excision on the scalp of one of my longstanding patients who is so dear to my heart!
She has had this bump on her scalp for many years now, and would love it removed, but wasn't sure what was involved in this process, so she really left it along for many years.
She was happy to learn that it is a pretty simple process, and is just a procedure under local anesthesia. I was quite surprised and excited to see that this pilar cyst just popped out whole with just a little pressure and a squeeze!
I was happy to do this procedure for her, she was so happy to get it removed, and I'm sure all of you will be thrilled to watch this cyst extraction!

OK, this is a GREAT pilar cyst excision on the scalp of one of my longstanding patients who is so dear to my heart!
She has had this bump on her scalp for many years now, and would love it removed, but wasn't sure what was involved in this process, so she really left it along for many years.
She was happy to learn that it is a pretty simple process, and is just a procedure under local anesthesia. I was quite surprised and excited to see that this pilar cyst just popped out whole with just a little pressure and a squeeze!
I was happy to do this procedure for her, she was so happy to get it removed, and I'm sure all of you will be thrilled to watch this cyst extraction!

This is a lovely patient of mine who travelled from a little distance to see me. She knew I did segments for The Doctors TV show, but this was before DrPimplePopper got big. She had a couple big bumps on her scalp, which are pilar cysts.
Pilar cysts tend to occur in families, and many people can have more than one. They most commonly occur on the scalp and have a thicker wall than an epidermoid cyst so the pilar cyst wall is easier to fully enucleate compared to an epidermoid cyst.
These are not harmful, and don't need to be removed. However, many people do seek removal, because the bump may be noticeable or it becomes bothersome to them, especially when they comb their hair.
Here I excise two areas on her head and remove these cysts. The second one is one of my favorite pilar cyst removals. I am often shocked by the size of a cyst when I remove it.. it is often bigger than it initially appears, and this one proved this to be true once again!

They are fun to remove because they are a challenge to pop out whole but usually behave in general and don't cause too many problems. And they always look bigger then you think when you take them out! Enjoy!

PIlar cysts are benign growths most commonly seen on the scalp. They are a little different than epidermoid cysts which are more often found on the body. Pilar cysts usually have thicker walls, so they are more easily popped out whole.
As you can see, Dr Rebish and I have different methods for removing cysts, and this is expected since we trained at different dermatology residency programs.
He prefers staples in the scalp because these are less likely to snag hairs which can increase chances for local infection.
Pilar cysts tend to run in the family.. often more than one family member has them, and it's not uncommon to have multiple cysts.
