SpaceShipTwo Crash: Co-Pilot Triggered Failure, NTSB Says | Video
Incident investigators find that SpaceShipTwo’s re-entry feather system was manually unlocked too early in the flight causing extreme aerodynamic loading leading to catastrophic structural failure.

Builder Scaled Composites could have added safeguards to prevent the failure. Complicating the situation, the FAA had determined that SS2 did not meet requirements for human and software errors, but granted a waiver anyway, even though no waiver had been requested by Scaled Composites.

The chairman of the National Transportation Safety Board (N-T-S-B) concluded a week long hearing into the TWA Flight 800 crash by promising that their investigation will continue.
Chairman James Hall said they will spare no effort to find out what really happened.
All 230 people aboard the aircraft were killed when the plane's centre fuel tank exploded last year just minutes after taking off from New York City.
The N-T-S-B isn't expected to release a probable cause until late next year.
The men, women, and children whose family members were killed in the crash of T-W-A Flight 800 won a promise from the National

USA: NTSB DISCUSS NEED FOR AIRCRAFT COCKPIT VIDEO
English/Nat
The U-S National Transportation Safety Board (N-T-S-B) wants another set of eyes in the cockpit.
N-T-S-B Chairman Jim Hall says the board is recommending that the Federal Aviation Authority (F-A-A) require video recorders in the cockpits of commercial jetliners.
That recommendation has been strongly criticised by pilots associations.
Speaking before a House of Representative panel, Hall said the cameras would be able to see the cockpit displays and other information that isn't recorded by the current equipment.
Hall says the N-T-S-B has been discussing the need for cockpit video recording since 1989.

Two people were killed aboard a small private plane after it collided in midair with an Air Force F-16 fighter jet Tuesday morning over South Carolina, officials said.
The Federal Aviation Administration said the fighter jet collided with a Cessna C150 around 11 a.m. ET, about 11 miles north of Charleston. Officials at the Shaw Air Force Base, east of Columbia, confirmed that the jet was one of their F-16 Fighting Falcons and that its pilot — Maj. Aaron Johnson of the 55th Fighter Squadron — survived.
"The F-16 pilot safely ejected and was taken to Joint Base Charleston for a health assessment," officials said in a statement. "Our thoughts
