The following information courtesy Wikipedia: The SL-1, or Stationary Low-Power Reactor Number One, was a United States Army experimental nuclear power reactor which underwent a steam explosion and meltdown on January 3, 1961, killing its three operators. The direct cause was the improper withdrawal of the central control rod, responsible for absorbing neutrons in the reactor core. The event is the only known fatal reactor accident in the United States. The accident released about 80 curies (3.0 TBq) of Iodine-131, which was not considered significant due to its location in a remote desert of Idaho. About 1,100 curies (41 TBq) of fission products were released into the atmosphere. The facility, located at the National Reactor Testing Station approximately 40 miles (64 km) west of Idaho Falls, Idaho, was part of the Army Nuclear Power Program and was known as the Argonne Low Power Reactor (ALPR) during its design and build phase. It was intended to provide electrical power and heat for small, remote military facilities, such as radar sites near the Arctic Circle, and those in the DEW Line. The design power was 3 MW (thermal). Operating power was 200 kW electrical and 400 kW thermal for space heating. In the accident, the core power level reached nearly 20 GW in just four milliseconds, precipitating the reactor accident and steam explosion. Documentary Films: The History of Nuclear Power How do nuclear power plants work to ensure absolute safety? To answer this question constantly tossed up in environmental forums, it may be best to understand what goes behind doors of a nuclear power plant. In many developing nations, nuclear energy is a major source of electricity as it claims to be safe whilst producing sufficient levels to meet their needs. Since coal and oil are considered depleting resources, the drive to explore nuclear energy as an alternative source is more evident than ever. The mere mention of a nuclear power plant normally draws mixed reactions. Perhaps due to historical events which are still rearing ugly heads, pun intended, in this current day and age, many may be hesitant to support the building of one in their neighborhood. Whether it is the fear of seeing over-sized farm animals, glow-in-the-dark residents or setting off Geiger meters, nuclear energy is becoming more of a reality. To put fluttering hearts at ease, strict rules and procedures are imposed in the running of a nuclear power plant. In ensuring safety within the plant as well as surrounding areas, it is considered to be a safer technology as compared to conventional methods. How do nuclear power plants work to generate the electricity supply? Leaving a lump of radioactive material on its own is not the solution as it breaks down and emits rays, not electrical charges. Other than endangering those in close proximity, this is obviously not the solution sought. When radioactive material breaks down, it is known as the fission process. When this happens, a tremendous amount of heat is released. To manage the heating process, water is used to control the temperature hike. As the water is heated, it is converted into steam. This is then pumped into a turbine which turns like a waterwheel placed atop a flowing river. Electricity is thus the final outcome and sent to a power grid or plant. Although the above covers the gist of the matter, it is not as simple in actual fact. Many levels and mechanisms of control need to be implemented to ensure smooth and safe operations at all times. Since radioactive material is the mainstay of the process, careful control over its behavior is crucial. If too much heat is generated, there must be methods to slow down the fission process. This is normally done via control rods which are lowered into the core to absorb the neutrons. By pumping pressurized water into the reactor to cool it down, the process is also slowed down. Uncontrolled overheating can result in reactor meltdown, an unfortunate incident in the dark annals of history.

An explosion and fire released large quantities of radioactive particles into the atmosphere, which spread over much of the western USSR and Europe. The Chernobyl disaster is the worst nuclear power plant accident in history in terms of cost and resulting deaths, and is one of only two classified as a level 7 event (the maximum classification) on the International Nuclear Event Scale (the other being the Fukushima Daiichi nuclear disaster in 2011). The battle to contain the contamination and avert a greater catastrophe ultimately involved over 500,000 workers and cost an estimated 18 billion rubles. The burning graphite moderator increased the emission of radioactive particles, carried by the smoke, as the reactor had not been encased by any kind of hard containment vessel. The accident occurred during an experiment scheduled to test a potential safety emergency core cooling feature, which took place during a normal shutdown procedure.

The finding supports the result of a simulation suggesting most of the molten fuel penetrated the core's base. Nuclear fuel in 3 of the plant's 6 reactors melted down in the March 2011 nuclear accident at the plant. But extremely high radiation levels have prevented experts from locating and determining the state of the molten fuel. Experts from the High Energy Accelerator Research Organization and other institutions have succeeded in taking X-ray-like photos of the plant's No.1 reactor. Since February, they have been using a type of elementary particle called the muon to get a peek inside the reactor, instead of using X-ray technology. The particles are created when cosmic rays collide with the Earth's atmosphere. The experts found nuclear fuel inside a storage pool located beside the No.1 reactor. But they did not find fuel inside the core of the reactor where the meltdown took place. The finding confirms the result from an earlier computer simulation that suggested most of the fuel in the reactor core likely melted and fell through the bottom into the containment vessel housing the core. Experts say the finding that most of the fuel had leaked out of the reactor core underlines the difficulties faced in scrapping the reactor. New tests show almost all of the fuel inside one of the Fukushima plant's reactors has melted, its operator said Thursday, the latest step in the clean up after Japan's worst ever nuclear crisis. Tokyo Electric Power Co. said the technology, which uses elementary particles called muon to create x-ray style images, gave the most concrete evidence yet the fuel had dropped to the bottom of the first reactor. The data, though largely expected, should help TEPCO as it continues its effort to decommission the plant four years after an earthquake and tsunami caused one of the worst nuclear meltdowns in living memory. Engineers have not been able to develop a machine to directly see the exact location of the molten fuel, hampered by extremely high levels of radiation in and around the reactors. While our previous analysis have already strongly suggested that fuel rods had melted down, the latest study provided further data that we like to regard as a progress in our effort to determine the exact locations of the debris, said a TEPCO spokesman. TEPCO plans to eventually use robots to locate the fuel debris as part of the decommissioning process, which is expected to take three to four decades to complete. Last month the International Atomic Energy Agency said Japan had made significant progress in its cleanup efforts but warned the situation remains very complex due to the growing amounts of contaminated water generated by the process. When a large fissile atomic nucleus such as uranium-235 or plutonium-239 absorbs a neutron, it may undergo nuclear fission. The heavy nucleus splits into two or more lighter nuclei (the fission products), releasing kinetic energy, gamma radiation and free neutrons. A portion of these neutrons may later be absorbed by other fissile atoms and trigger further fission events, which release more neutrons, and so on. This is known as a nuclear chain reaction.
