Health care in the United States is provided by many distinct organizations. Health care facilities are largely owned and operated by private sector businesses. 62% of the hospitals are non-profit, 20% are government owned, 18% are for-profit. 60--65% of healthcare provision and spending comes from programs such as Medicare, Medicaid, TRICARE, the Children's Health Insurance Program, and the Veterans Health Administration. Most of the population under 67 is insured by their or a family member's employer, some buy health insurance on their own, and the remainder are uninsured. Health insurance for public sector employees is primarily provided by the government. The United States life expectancy of 78.4 years at birth, up from 75.2 years in 1990, ranks it 50th among 221 nations, and 27th out of the 34 industrialized OECD countries, down from 20th in 1990. Of 17 high-income countries studied by the National Institutes of Health in 2013, the United States had the highest or near-highest prevalence of infant mortality, heart and lung disease, sexually transmitted infections, adolescent pregnancies, injuries, homicides, and disability. Together, such issues place the U.S. at the bottom of the list for life expectancy. On average, a U.S. male can be expected to live almost four fewer years than those in the top-ranked country. According to the World Health Organization (WHO), the United States spent more on health care per capita ($8,608), and more on health care as percentage of its GDP (17.9%), than any other nation in 2011. The Commonwealth Fund ranked the United States last in the quality of health care among similar countries, and notes U.S. care costs the most. In a 2013 Bloomberg ranking of nations with the most efficient health care systems, the United States ranks 46th among the 48 countries included in the study. The U.S. Census Bureau reported that 49.9 million residents, 16.3% of the population, were uninsured in 2010 (up from 49.0 million residents, 16.1% of the population, in 2009). A 2004 Institute of Medicine (IOM) report said: "The United States is among the few industrialized nations in the world that does not guarantee access to health care for its population." A 2004 OECD report said: "With the exception of Mexico, Turkey, and the United States, all OECD countries had achieved universal or near-universal (at least 98.4% insured) coverage of their populations by 1990." Recent evidence demonstrates that lack of health insurance causes some 45,000 to 48,000 unnecessary deaths every year in the United States. In 2007, 62.1% of filers for bankruptcies claimed high medical expenses. A 2013 study found that about 25% of all senior citizens declare bankruptcy due to medical expenses, and 43% are forced to mortgage or sell their primary residence. On March 23, 2010, the Patient Protection and Affordable Care Act (PPACA) became law, providing for major changes in health insurance. The Patient Protection and Affordable Care Act (Public Law 111-148) is a health care reform bill that was signed into law in the United States by President Barack Obama on March 23, 2010. Along with the Health Care and Education Reconciliation Act of 2010 (passed March 25), the Act is a product of the health care reform agenda of the Democratic 111th Congress and the Obama administration. The law includes a large number of health-related provisions to take effect over the next four years, including expanding Medicaid eligibility for people making up to 133% of FPL,[174] subsidizing insurance premiums for peoples making up to 400% of FPL ($88,000 for family of 4) so their maximum "out-of-pocket" pay will be from 2% to 9.8% of income for annual premium,[175][176] providing incentives for businesses to provide health care benefits, prohibiting denial of coverage and denial of claims based on pre-existing conditions, establishing health insurance exchanges, prohibiting insurers from establishing annual spending caps and support for medical research. The costs of these provisions are offset by a variety of taxes, fees, and cost-saving measures, such as new Medicare taxes for high-income brackets, taxes on indoor tanning, cuts to the Medicare Advantage program in favor of traditional Medicare, and fees on medical devices and pharmaceutical companies;[177] there is also a tax penalty for citizens who do not obtain health insurance (unless they are exempt due to low income or other reasons).[178] The Congressional Budget Office estimates that the net effect (including the reconciliation act) will be a reduction in the federal deficit by $143 billion over the first decade. http://en.wikipedia.org/wiki/Health_care_in_the_united_states

Hillary Clinton did not announce a 2016 U.S. presidential run on Saturday (March 22) but she did hint at its possibility. Speaking at a Clinton Global Initiative University event in Tempe Arizona event, moderated by comedian Jimmy Kimmel and with husband Bill and daughter Chelsea at her side, Clinton said "I'm obviously thinking about all kinds of decisions." With Kimmel acting as a host of the session, the talk took on a mostly light-hearted tone. "When you do run for president will you re-use the old Clinton campaign lawn signs?," said Kimmel. Despite the general levity of the session, Hillary stuck a somewhat serious tone when she said, "I am very much concerned about the direction of our country and it's not just who runs for office, but what they do when they get there and how we bring people together and particularly empower young people so that we can tackle some of these hard decisions we've just been talking about," said Clinton. Kimmel responded by prodding her further, asking what a future Clinton White House would look like. "Should you wind up back in the White House, will Chelsea get her old room back or will you convert that into a home gym?," asked the comedian. Early polls indicate that Clinton easily would be the favorite to win the 2016 nomination if she chose to run. But recently there have been signs of frustration within the party over what some call the "inevitability" of her nomination. Clinton has said that she will not decide whether to run for president before the end of this year. Until then, she will continue the speaking tour, and promote a new memoir that focuses on her time as a diplomat. Clinton has also said that an assault by militants on a U.S. mission in Benghazi, Libya, that killed four Americans in 2012 was her greatest regret as secretary of state, and Republicans continue to try to use the episode to portray her as incompetent on foreign policy. The summer release of her book will give her opportunities to talk about her time as secretary of state in carefully staged appearances, while continuing to give lucrative speeches. Transcript -- I think the most important thing that I have learned is that there's more to learn. That we should -- that we should all be hungry for a lifetime. I mean, for example, at my next birthday I'll be 68. All the great scientific discoveries made by all the great geniuses were largely made when they were in their 20s and 30s. And yet I became, about two years ago, obsessed with particle physics and I was determined to understand it before I died. I could not have done that if I hadn't learned to read when I was young. If I hadn't had the opportunity to study science courses in my high school, and I lived in the second poorest state in the United States, which most people my age in my state did not have. I happened to go to a bigger high school with people who understood we had to get good science and math teachers there. And if I hadn't gone to, in my case, Georgetown University, which was a Jesuit University, and I hadn't been subject to the kind of rigors that the Jesuits imposed which made me realize that however much I thought I knew and however smart I was I didn't know very much and I wasn't very smart. I had a lot to learn. So that's the most important thing I learned that your brain is a gift. And we now know that people well into their late 60s and 70s can form new neural networks. So that even though your brain begins to shrink in your 30s, and does throughout your life, since none of us ever use even close to half of our brainpower we got a lot left and we will on our last day on earth we'll have a lot left. So, the idea that we now know, as a scientific measure because of all the brain scanning technology that we can form these networks and that we form them best, we're most likely to form new neural networks later in life by learning something new. So if -- I said I was interested in particle physics and also in astrophysics and I'm trying to figure out what it means that we've located 20 planets outside our solar system in the last five years that seem to have enough density and be far enough away from their sons that they might be able to support life. That may be the answer to the Russia Ukraine problem; an attack from outer space will immediately unite us all. Members of Congress in the U.S. will immediately start hugging each other and singing Kumbaya. But anyway, I can form new neural networks doing that because I don't know anything about it, or I didn't when I started. A theoretical physicist would do better going to Suzuki piano lessons with his grandchild or her grandchild and just playing if you knew nothing about music. But this is an incredible thing that the most important thing I learned is that it's important to keep on learning. That you should stay hungry and that the greatest gift can be even as your body begins to fail if your minds still working you need to use it. Produced by Jonathan Fowler
