Comair 191 (Delta 5191)
Comair Flight 191, marketed as Delta Connection Flight 5191, was a scheduled United States (US) domestic passenger flight from Lexington, Kentucky, to Atlanta, Georgia, operated on behalf of Delta Connection by Comair. On the morning of August 27, 2006, the Bombardier Canadair Regional Jet 100ER that was being used for the flight crashed while attempting to take off from Blue Grass Airport in Fayette County, Kentucky, four miles (6 kilometers) west of the central business district of the City of Lexington. The aircraft was assigned the airport's Runway 22 for the takeoff, but used Runway 26 instead. Runway 26 was too short for a safe takeoff, causing the aircraft to overrun the end of the runway before it could become airborne. It crashed just past the end of the runway, killing all 47 passengers and two of the three crew. The flight's first officer was the only survivor. In no way am I mocking the accident, as it was very tragic and my prayers go out to the families who lost loved ones.

The plane crashed into a house in Clarence Center, New York on February 12, 2009, at 10:17 pm The crash killed 50 people, including the two pilots, two flight attendants, 45 passengers (including an off-duty pilot), and a person in the house into which the plane crashed. It was the first fatal accident of a commercial airliner in the United States since the August 2006 crash of Comair Flight 191, and the most recent as of April 2012. A Bombardier DHC8-402 Q400 operating as Flight 3407 departed late from Newark on February 12, 2009, at 9:20 pm EST. Shortly after the last communication by the flight crew with approach control at 10:11 pm (03:11, February 13 UTC), the plane stalled less than a mile northeast of the Outer Marker (LOM) while on an ILS approach to Runway 23[1] and crashed into a house in the northeast Buffalo suburb of Clarence Center, a hamlet and census-designated place in the Town of Clarence, about 5 nautical miles (9.3 km) short of the runway threshold. A total of 50 people were killed including the two pilots, two flight attendants, 45 passengers (including one off-duty pilot), and one person in the house. It was the first fatal crash of a commercial airliner in the United States since the crash of Comair Flight 191 in August 2006 that killed 49.

This is the Cockpit voice recording with CVR transcript of the Delta Airlines Lockheed L10-11 TriStar that crashed on approach to Dallas Fortworth on August 2nd 1985. 137 poeple died including one person on the ground and two that died more than 30 days after the accident as a result of their injuries, only 27 poeple survived. A Delta Airlines Lockheed L-1011 Tristar crashes while approaching a runway at Dallas-Fort Worth International Airport. The disaster kills 133, including a motorist on the ground, although 30 aboard the plane survive. The survivors are in the tail section that breaks off from the rest of the plane, which explodes after skidding across the airport. The crash is attributed to wind shear.

Viewer discretion advised Delta Air Lines Flight 1141 was a flight that flew from Jackson to Dallas-Fort Worth International Airport to Salt Lake City International Airport in Salt Lake City, Utah. On August 31, 1988, Flight 1141 crashed after takeoff from Dallas-Fort Worth Airport. Two of the seven crew members and twelve of the 101 passengers on board lost their lives. On this occasion, the aircraft was a Boeing 727 Advanced, registration number N473DA. It was delivered in 1973, the 992nd Boeing 727 to be delivered. Two facts were primarily blamed for the tragedy of flight 1141: The crew had not ensured that the wing's flaps were properly positioned for take-off, and the plane's unprepared take-off horn was not functioning. As a result, the plane struck a nearby object immediately after taking off, causing the accident. In an unexpected sequel to the investigation, the broadcast of the cockpit voice recorder (CVR) by the media demonstrating why the crew mistakenly mispositioned the flaps, provoked such an outcry by pilots, that subsequent releases of CVR data are protected by law and are carefully vetted by the NTSB. FAA regulations require a sterile cockpit before takeoff. This means there is to be no conversation outside of talk pertaining to the plane and pending flight.
