Raleigh, North Carolina

Raleigh (/ˈrɑːli/; RAH-lee) is the capital of the state of North Carolina as well as the seat of Wake County in the United States. It is the second most populous city in North Carolina, after Charlotte. Raleigh is known as the "City of Oaks" for its many oak trees, which line the streets in the heart of the city. The city covers a land area of 142.8 square miles (370 km²). The U.S. Census Bureau estimated the city's population to be 439,896 as of July 1, 2014. It is also one of the fastest-growing cities in the country. The city of Raleigh is named after Sir Walter Raleigh, who established the lost Roanoke Colony in present-day Dare County.

Raleigh is home to North Carolina State University and is part of the Research Triangle area, together with Durham (home of Duke University) and Chapel Hill (home of the University of North Carolina at Chapel Hill). The "Triangle" nickname originated after the 1959 creation of the Research Triangle Park, located in Durham & Wake Counties partway between the three cities and their universities. The Research Triangle region encompasses the U.S. Census Bureau's Raleigh-Durham-Chapel Hill Combined Statistical Area (CSA), which had an estimated population of 2,037,430 in 2013. North Carolina

North Carolina (ⁱ/ˌnɔːrθ kærəˈlaɪnə/) is a state in the southeastern region of the United States. The state borders South Carolina and Georgia to the south, Tennessee to the west, Virginia to the north, and the Atlantic Ocean to the east. North Carolina is the 28th most extensive and the 9th most populous of the 50 United States. North Carolina is known as the Tar Heel State and the Old North State.

North Carolina is composed of 100 counties. Its two largest metropolitan areas are among the top ten fastest-growing in the country: its capital, Raleigh, and its largest city, Charlotte. Daniel Albright

Daniel Albright (October 29, 1945 – January 3, 2015) was the Ernest Bernbaum Professor of Literature at Harvard and the editor of Modernism and Music: An Anthology of Sources. He was born and grew up in Chicago, Illinois and completed his undergraduate studies on a full scholarship at Rice in 1967. He received his MPhil in 1969 and PhD in 1970, both from Yale. Albright is also the author of the book Quantum Poetics which was published by Cambridge University Press in 1997. He held an NEH fellowship from 1973-1974, was a Guggenheim Fellow from 1976-1977, and more recently, he was a 2012 Berlin Prize Fellow at the American Academy in Berlin.

Albright began his undergraduate career as a mathematics major, but changed to English literature. Although trained at Yale as a literary critic, after the publication of his book Representation and the Imagination: Beckett, Kafka, Nabokov, and Schoenberg, he was invited by the University of Rochester to come teach there as a kind of liaison between the department of English and the Eastman School of Music. At Rochester, he studied musicology, which forever changed his career. Much of his subsequent work has been on literature and music, culminating with his recent book, Panaesthetics which addresses many arts and examines to what extent the arts are many or are one. He was hired in 2003 in the Harvard departments of English, but later joined the Comparative Literature department and soon began offering courses in the Music department as well.

Orthopedic surgery

Orthopaedic surgery or orthopaedics (sometimes spelled orthopedic surgery and orthopedics) is the branch of surgery concerned with conditions involving the musculoskeletal system. Orthopaedic surgeons use both surgical and nonsurgical means to treat musculoskeletal trauma, sports injuries, degenerative diseases, infections, tumors, and congenital disorders.

Nomenclature

Nicholas Andry coined the word "orthopædics" in French as orthopédie, derived from the Greek words ὀρθός orthos ("correct", "straight") and παιδίον paidion ("child"), when he published Orthopedie (translated as Orthopædia: or the Art of Correcting and Preventing Deformities in Children) in 1741. Though as the name implies it was initially developed with attention to children, the correction of spinal and bony deformities in all stages of life eventually became the cornerstone of orthopedic practice.

As with many words derived with the "æ" ligature, simplification to either "ae" or just "e" is common. In the US the majority of college, university and residency programs, and even the American Academy of Orthopaedic Surgeons, still use the spelling with the Latinate digraph ae. Elsewhere, usage is not uniform; in Canada, both spellings are acceptable; orthopaedics usually prevails in the rest of the British Commonwealth, especially in the UK.

Transcript of video: What are the three approaches for hip surgery for hip replacement operation? Three approaches. My name is Doctor Dan Albright. I'm an orthopedic surgeon in Raleigh, North Carolina. I specialize in hip replacements. I did a fellowship in hips and devote a large part of my practice to replacing the hip. There are three approaches to the hip and as a surgeon we're, of course, focused on anatomy. That's one reason we all became surgeons because we love anatomy. Anatomy is crucial to what we do and doing it well. This is a model of the hip. This plastic outside is the skin. We'll take that out - it represents the skin. And then there are three parts of the hip that we explore with surgery. We go from the front which is my preferred way. From the front, I'll tell you why in a minute. We also go from the side which I do sometimes with tougher surgery, revision surgery. If someone's had three, four, five surgeries, cancer or just some unusual situation, I'll go lateral. It's a good way. And then there's posterior and that's what most of the country does. Our whole culture opens the gludeus maximus, your buttock muscle and that muscle is split with the posterior approach and that's what our culture is centered around - the posterior approach to the hip. That's what's most common. That's what you'll hear about and it works. I have a lot of colleagues across the country that do the posterior approach. It's a good operation. There's controversy amongst us surgeons we all think that the way we do it is the best. In my hands, and I've done all three approaches, in my hands, the anterior approach is what I believe works the best. One reason is you split muscle (in the other approaches) and you don't stitch muscle back to the bone and it's my belief that there's less dislocations after. There's some debate on that topic, but I have no restrictions after surgery. That's a real big deal with a hip replacement operation. You could tie your shoes first day, if you want. With posterior approach there are some real restrictions for some surgeons and that's held true over decades. Though not dislocating is a big deal, less muscle damage is a big deal, in my opinion. I don't sew anything back. I just close the skin that's pretty much what I do, and the fascia, which is the covering over the muscle. So it's a very, pretty quick procedure going from the front and quicker recovery, in my opinion. Patients are out of bed within a couple hours, usually, and then home by the next day, most times. So one night in the hospital. It's possible to go home same day, but that's not something I'm trying to encourage right now, but one night in the hospital, home the next day is the norm. That's pretty good, especially compared to the old days. Years ago when I trained, people were in the hospital for a week, ten days - in traction. It was quite different. So we made a lot of really nice gains with technology. The evolution of hip replacement surgery has been very good. I encourage you to Google "anterior hip replacements." There is a lot of good information out there. I trained at Yale years ago and we were a center for anterior hips and really got some good training. Did a fellowship at Harvard in hips, so I really like to focus on it because people are happy after they get this operation. They have a better life, a healthier life. Transcription of video: Hello. My name is Dr. Dan Albright. I'm an orthopedic surgeon in Raleigh, North Carolina. I'd like to talk to you about why smoking tobacco is bad for you and how it affects, negatively, your surgery. It is pretty much obvious that we all known smoking is bad for you. It gives you cancer. And I am amazed how many patients of mine don't really recognize that. Smoking does give you cancer, lung cancer, but it also affects badly, your heart, heart disease, your brain, your kidneys, but I'm an orthopedic surgeon, so I'm doing muscle and skeletal bone surgery, bone and joint surgery so why do I care? Well, one, I care about your health. But number two, it affects my surgery. Nicotine constricts blood flow and when my patients walk in the office I can just tell from their skin. Nicotine cuts down the blood to your skin and people look gray. Your skin looks unhealthy and I am immediately thinking, oh their skin is not going to heal very well when I do surgery. And when I'm doing surgery on a smoker the bleeding is less. Nicotine constricts blood flow so patients don't bleed as well in surgery and that means they don't heal as well after surgery that means they can get more infections. So smoking is a big deal for us with surgery. And sometimes we won't do surgery if people keep smoking. Particularly with the spine fusion. It's been proven that nicotine hurts fusions. So you don't fuse your back as well or your neck when you have spine surgery if you are smoking. There are surgeons who won't operate on smokers for that reason. I just wanted to get out into the open that the many effects of smoking are just hurting you and particularly for us as surgeons we know our smokers are going to heal slower and have more risks for surgery. So stopping smoking at least a month before surgery will help you heal quicker. You will heal quicker and you'll heal better. Guaranteed. Have a safer surgery if you stop smoking. So the sooner you get the nicotine out of your body, the better. I'm happy to talk to you more about other aspects of this. Using a model, Dr. Albright shows the type of device used and explains how it relieves nerve pain. Transcription of video: What is a cervical fusion? Hello, my name is Dr. Dan Albright. I'm an orthopedic spine surgeon in Raleigh, North Carolina. So what is the anterior cervical fusion? It's called an ACDF. If one googles ACDF you'll see Anterior (which means from the front), Cervical (which means neck), Decompression and Fusion - ACDF. It's done around the world. Maybe a model, visual aid is the easiest way to understand. This is a fusion device, right here. What we do is we take the disc out. Why do we take the disc out? To get pressure off the nerves. Often times people have nerve pain which is pain going from your neck to your shoulder, shoulder blade, down your arm. It's awful neck pain oftentimes. And so we get pressure off by going through the front of the neck which is the easiest way really to get to the spine. And the disc comes out. The disc is the mobile part between the bones. The disc comes out, the pressure comes off the spinal cord, pressure comes off the nerves and then you put a block in there. In this case it's a metal cage. It can be a piece of bone from your hip bone. I don't do that much any more that way. This is better. Cage and your bones fuse to the cage. To lock it in, we put two screws above, two screws below and a plate. In over six weeks these two bones become one bone. There's no motion any more. This cures arm pain. It can help neck pain - not always cures neck pain, but we can cure the nerve pain, and it's a great operation. It's been around a long, long time - decades. I've done thousands of these since the 1980s when I started my training. It's the gold standard for cervical spine surgery. There are other ways to do neck surgery.sometimes from the back, which I'll do occasionally. And sometimes we'll do a disc replacement, but the traditional, tried-and-true gold standard for cervical spine surgery is this device right here. And no one argues it's a terrific operation. Dr. Dan Albright, an orthopedic surgeon, discusses the difference between a partial knee replacement and a total knee replacement and when he would use a partial knee replacement over a total knee replacement. Transcript of video: Why is a partial knee replace better than a total knee replacement? That's a question I'd like to address in this segment. I'm Dan Albright, MD. I'm an orthopedic surgeon in Raleigh, North Carolina. So why are partial knees is better than total knees? There's some debate and controversy there, but I'll give you my opinion. There are many times - yesterday, in fact, I operated on a woman's knee and only the inner part of the knee was worn out and so I only replaced the inner part of the knee. And I think that's better than a total knee. I have several patients where I have done a total knee replacement, which you have to do some times. And then on that same patient I've done a partial knee replacement in a different year. Without exception they always like their partial knee replacements better. Partial knee replacements move more. They flex more and they feel more natural. That sounds like, perhaps, a cliché, but when you replace only part of the knee, the inner part that's worn out and you preserve the outer part and you preserve the kneecap it feels more natural. Patients tell me that and there's literature to support that. I'll show you some anatomy here just to demonstrate. On the model here are several knees. The first one is a knee with no arthritis. The blue is the cartilage on the femur and that's crucial there is also cartilage on the tibia. Tibia is the leg bone right here. Anyway, when the cartilage wears out is when you need a knee replacement. On the very end here that brownish discoloration - that patient needs a total knee. The whole knee's shot. Total knees are great in the right situation. Where there are many patients where arthritis just starts on the inner part, right there where the brown is, but the rest is pretty good. So I almost feel guilty when I replace somebody's knee, like a total knee like some patients just want a total knee, no partial, but they're only worn out on the inside. So I'm taking out pristine cartilage on the outside and taking out a pristine kneecap cartilage and it's overkill. To do a total knee, a lot of people don't need a total knee. A lot of people just need the inner part replaced - that's all that's worn out and the benefit is, I believe, better motion, it feels more like your normal knee because you have one third of it replaced - the rest is good. And it is untouched by surgery. This is an example, two examples, one the total knee. Here's an example of a knee that's wearing out, just another example where you see the cartilage is being lost there on the end of the bone. And it's like walking on sandpaper. There's no cushion there. So I'm a big believer in total knees. I do a lot of them when the whole knee is worn out. This is a total knee. Its got a femur, and you have a tibia, and then you also have a kneecap - a plastic button glued on the kneecap. The partial is much smaller. Here is a partial. I use the Oxford knee which I think is better - we'll go into the reasons in a minute, but it's just the inner part of the femur's replaced, a glued on piece of metal. You have a glued on piece of metal on the tibia. Then the magic of the oxford is you have this mobile piece of plastic - it moves around in between. There are good twenty years success rates on this specific type of partial knee replacement. So, yes, I do a lot of total knee replacements, but I try to do a partial whenever I can if only the inner part of the knee is worn out. The patients love it. It's a smaller surgery, it's a safer surgery, it has less risks than a total knee, and it works really nicely. So if you having knee pain, knee arthritis, I'm happy to go into all this with you. Dr. Albright discusses an advanced procedure to alleviate neck pain in this video on cervical disk replacement. Transcript of video: Hello. When you do a cervical disc replacement? That's the question I'd liked the address today. My name is Doctor Dan Albright. I'm an orthopedic spine surgeon at the Raleigh Orthopedic Clinic in Raleigh, North Carolina. And total disc replacement for the cervical spine is what I would like to talk about. It's also called arthroplasty. A total disk arthroplasty. What is it and when do you do it? Well, the cervical spinal - I have a model here. The discs are the soft shock absorbers between the hard bones of the vertebrae. We most commonly would do disc surgery in the neck for a rupture, a herniation. A herniation is when - if the disc is like a jelly donut, it's when the jelly squirts out from the middle, out to the outside and pinches a nerve. You get nerve pain into your arm, into your shoulder, into your shoulder blades, scapula. It's awful pain. It usually gets better without surgery, but not always. So you need, if you've got awful nerve pain, not just neck pain, but pain shooting into the nerve and into the arm surgery is done commonly. And for decades we've done ACVFs. ACVF is a fusion. You going in - it's a great operation - you go in from the front of the spine from the neck. Take out the whole disc. Take the pressure off the nerves. Then we've got to fill the desk with something. Recently, the science has become very strong to fill that disc not with a fusion material, but with a replacement. A replacement - this is a magnified view - this is actually the lumbar spine, but a disc replacement is mobile. You put it in the empty disc and you can move. So here's the neck, it's much smaller and you preserve motion. So why do you want to preserve motion? Because that's natural. Your body is made to move. Hip joints are made...we do hip replacements, knee replacements, shoulder and elbow replacements because motion is what your body wants. So while fusions have been shown to work, it appears that disc replacements may work better. At least when you do two disc levels. There's recent science saying that two disc levels, when you do a replacement, can work better than a fusion. How long have replacements been out? Well, 2004 is when I used the Mobi-C disc replacement and that was started in 2004 and tens of thousands of patients have that disc replacement across the world. Our government, the FDA, approves of the disc replacement, but I'm a big proponent of preserving motion. So, it's an excellent operation, cervical spine surgery, fusion or disc replacement. My preference is the disc replacement when possible and I highly recommend that operation. There are some other nuances as to what type of surgery you get and when. What are Cervical Disc Herniation Symptoms and Treatments?
Dr. Daniel Albright, an orthopedic surgeon in Raleigh, North Carolina, describes a the symptoms and treatments for a cervical disc herniation. A cervical disc herniation can cause pain down your arm. Treatment usually does not require surgery, but if the nerve pain cannot be relieved, it can be a last option. Watch the video for more information. Transcription of video: Hello. Cervical disc herniation symptoms and treatment. My name is Dr. Dan Albright. I'm an orthopedic spine surgeon in Raleigh, North Carolina. So, cervical disc herniation. First of all cervical means the neck, the cervical spine. The disc is the mobile piece between the bone (the vertebrae). It's like a tire between the hard bones. A herniation is a rupture, it means a blowout. Think of the disc like a jelly doughnut and it's the soft part between the bones. There's the outer part and then the inner part is the jelly. Occasionally that jelly squirts out, that's a disc rupture. That's a disc herniation. It's very inflammatory. It hurts. If you have a disc rupture on your nerve you get an awful, toothache-like pain down your shoulder blade. It can be pretty tough. So that's the symptom. A symptom is what you feel. A sign I look for is weakness and reflex changes. First thing we want to know is what are your symptoms. Typically for a disc rupture it's neck pain, shooting to the shoulder, the shoulder blades, the scapula, then down the arm. Then again many people say it's like a toothache. A toothache is nerve pain. That's what a disc rupture does, it causes nerve pain. Pressed nerves, pinched nerves all the same thing. Our medical term is radiculopathy. You might see that if you are reading about this. So, what do you do about it? What's the treatment for this? Usually it's not surgical. Surgery can be terrific, but not the first treatment. For a ruptured disc exercise is bad. This is not a muscle pain it's nerve pain. Nerve pain gets worse with exercise. You can walk. Walk a lot, but don't do any upper body exercises. Don't lift weights - nothing over the weight of a gallon of milk. Take some Advil. Take some Aleve. (If you are allowed to from a medical viewpoint.)Stay out of a car. Riding or driving a car can aggravate it (the vibrations can get to you). Therapy can help sometimes. Sometimes a traction can help. A cervical epidural steroid shot can help. We'd send you to a pain clinic for that. A ruptured disc will heal with time. With these non-surgical treatments we're trying to buy time. If we can get the patient through a week, two weeks, three weeks. Usually nature just calms this down. Nature cures disc ruptures. They absorb. They dissolve. They go away. Those are the main things for treatment. Not everyone gets better with non-surgical treatments. Surgery takes pressure off the nerve. You wake up and go "aaah" the pressure is just gone.
