Nd:YAG laser
Nd:YAG (neodymium-doped yttrium aluminium garnet; Nd:Y3Al5O12) is a crystal that is used as a lasing medium for solid-state lasers. The dopant, triply ionized neodymium, Nd(III), typically replaces a small fraction (1%) of the yttrium ions in the host crystal structure of the yttrium aluminium garnet (YAG), since the two ions are of similar size. It is the neodymium ion which provides the lasing activity in the crystal, in the same fashion as red chromium ion in ruby lasers.
Laser operation of Nd:YAG was first demonstrated by J. E. Geusic et al. at Bell Laboratories in 1964.
Technology
Nd:YAG lasers are optically pumped using a flashtube or laser diodes. These are one of the most common types of laser, and are used for many different applications. Nd:YAG lasers typically emit light with a wavelength of 1064 nm, in the infrared. However, there are also transitions near 940, 1120, 1320, and 1440 nm. Nd:YAG lasers operate in both pulsed and continuous mode. Pulsed Nd:YAG lasers are typically operated in the so-called Q-switching mode: An optical switch is inserted in the laser cavity waiting for a maximum population inversion in the neodymium ions before it opens. Then the light wave can run through the cavity, depopulating the excited laser medium at maximum population inversion. In this Q-switched mode, output powers of 250 megawatts and pulse durations of 10 to 25 nanoseconds have been achieved. The high-intensity pulses may be efficiently frequency doubled to generate laser light at 532 nm, or higher harmonics at 355 and 266 nm.

Yagō
Yagō (屋号), literally meaning "house name", is a term applied in traditional Japanese culture to names passed down within a guild, studio, or other circumstance other than blood relations. The term is synonymous with iena (家名) and kadona (角名). The term most often refers to the guild names of kabuki actors, but is also applicable to the names artists take from their masters or studios, names taken from one's business, and a few other similar circumstances.
The ya (屋) of yagō, also often seen at the end of a yagō name, means "house", "roof", or "shop", and helps to illuminate the origins and meanings of the term.
History
Originally, yagō were place names of homes or buildings taken on by the inhabitants. Even when the house changed hands entirely from one family to another, the new family would take on the name of the house. However, the previous owners would frequently keep the house name when they moved. Often, a family (or individual) would come to be better known by their yagō than by their actual family name.

Ayahuasca
Ayahuasca (UK /ˌaɪjəˈwæskə/; US /ˌaɪjəˈwɑːskə/), also commonly called yagé (/jɑːˈheɪ/ or /jæˈheɪ/), is an entheogenic brew made out of Banisteriopsis caapi vine, often in combination with various other plants. The brew is used as a traditional spiritual medicine in ceremonies among the Indigenous peoples of Amazonian Peru, many of whom say that they received the instructions in its use directly from the plants and plant spirits themselves.
Ayahuasca was first described outside of Indigenous communities in the early 1950s by Harvard ethnobotanist Richard Evans Schultes, who became aware of the Native communities who use it for divinatory and healing purposes.
It can be mixed with the leaves of Chacruna or Chagropanga, dimethyltryptamine (DMT)-containing plant species. It has been reported that some effects can be felt from consuming the caapi vine alone, but that DMT-containing plants (such as Psychotria) remain inactive when drunk as a brew without a source of monoamine oxidase inhibitor (MAOI) such as B. caapi.

Yttrium aluminium garnet
Yttrium aluminium garnet (YAG, Y3Al5O12) is a synthetic crystalline material of the garnet group. It is also one of three phases of the yttrium-aluminium composite, the other two being yttrium aluminium monoclinic (YAM, Y4Al2O9) and yttrium aluminium perovskite (YAP, YAlO3). YAG is commonly used as a host material in various solid-state lasers. Rare earth elements such as neodymium and erbium can be doped into YAG as active laser ions, yielding Nd: YAG and Er:YAG lasers, respectively. Cerium-doped YAG (YAG:Ce) is used as a phosphor in cathode ray tubes and white light-emitting diodes, and as a scintillator.
Gemstone YAG
YAG for a period was used in jewelry as a diamond and other gemstone simulant. Colored variants and their doping elements include: green (chromium), blue (cobalt), red (manganese), yellow (titanium), purple (neodymium), pink, and orange. As faceted gems they are valued (as synthetics) for their clarity, durability, high refractive index and dispersion. The critical angle of YAG is 33 degrees. YAG cuts similar to natural garnet, with polishing being performed with alumina or diamond (50,000 or 100,000) on common polishing laps. YAG has low heat sensitivity.

I would use a 1064 long pulse wavelength in the treatment of dare skin hair removal. If you’re going to do a good job in hair removal now, you need to be able to treat all skin types well. Many times, people try to get one system and make it work, but 1064 is really the best wavelength for skin type 4, 5, and 6. I would also use it in a fair skinned person with course black hair.
In your laser manual there are settings for laser hair removal for thick hair or thin hair and brown hair versus black hair. Usually with a person’s first laser treatment for hair removal, you want to start with the settings for thick hair and as you move on to subsequent treatments the hairs that you’ll be treating get thinner and thinner, so you’ll change your settings to the thinner hair settings. With the thick hair settings, because the thick hair has a larger diameter you want to use a longer pulse duration so that the energy is delivered over a longer period of time versus a short energy or short pulstation where you’re delivering the energy too quickly.
With laser hair removal treatments, you always want to use some kind of cooling, so, we have the Zimmer cold air blower connected to our hand piece here, so that at the same time of the treatment, there’s also cold air blowing onto the skin. And the cooling is for two reasons; one for patient comfort but two, more importantly, is to protect the surrounding tissue, in other words the skin or the epidermis that surround the hair follicles that we’re treating.
As we do the treatment, I’m going to cover up the entire surface area of the skin that we want to treat. A little bit of overlapping is ok, but generally speaking overlapping is not necessary.
The clinical endpoint to look for with laser hair removal is a little bit of erythema, which we see here, as well as some perifollicular edema, which is just a little bit of swelling directly around the hair follicles and that’s very normal and actually shows a great result. Also, laser hair removal for Fitzpatrick types 4, 5, and 6 can actually be more painful than laser hair removal for Fitzpatrick types 1, 2, or 3 and that’s just due to the amount of pigment that’s in darker skin colors and that’s very normal as well.
“Grant, how does your skin feel after the treatment?”
“It feels good.”
“Does it feel warm at all?”
“A little bit, but not too bad.”
Ok, so a little bit of heat sensation like a mild sunburn is natural and normal. That usually lasts about two to three hours and then it should dissipate. If it’s bothersome or uncomfortable, you can take some Tylenol or some ibuprofen to help with that. You can also put ice packs on your skin to help with that as well. Your next treatment should be in roughly six weeks or so and then you can also use any of your regular products. For instance, when you shower, wash your face with soap, that’s fine, and any aftershaves or lotions that you normally put on your face is totally ok to use as well. But, you do want to use a good SPF on your skin for any time you’re in the sun for good sun protection after having a laser treatment.

I would use a 1064 long pulse wavelength in the treatment of dare skin hair removal. If you’re going to do a good job in hair removal now, you need to be able to treat all skin types well. Many times, people try to get one system and make it work, but 1064 is really the best wavelength for skin type 4, 5, and 6. I would also use it in a fair skinned person with course black hair.
In your laser manual there are settings for laser hair removal for thick hair or thin hair and brown hair versus black hair. Usually with a person’s first laser treatment for hair removal, you want to start with the settings for thick hair and as you move on to subsequent treatments the hairs that you’ll be treating get thinner and thinner, so you’ll change your settings to the thinner hair settings. With the thick hair settings, because the thick hair has a larger diameter you want to use a longer pulse duration so that the energy is delivered over a longer period of time versus a short energy or short pulstation where you’re delivering the energy too quickly.
With laser hair removal treatments, you always want to use some kind of cooling, so, we have the Zimmer cold air blower connected to our hand piece here, so that at the same time of the treatment, there’s also cold air blowing onto the skin. And the cooling is for two reasons; one for patient comfort but two, more importantly, is to protect the surrounding tissue, in other words the skin or the epidermis that surround the hair follicles that we’re treating.
As we do the treatment, I’m going to cover up the entire surface area of the skin that we want to treat. A little bit of overlapping is ok, but generally speaking overlapping is not necessary.
The clinical endpoint to look for with laser hair removal is a little bit of erythema, which we see here, as well as some perifollicular edema, which is just a little bit of swelling directly around the hair follicles and that’s very normal and actually shows a great result. Also, laser hair removal for Fitzpatrick types 4, 5, and 6 can actually be more painful than laser hair removal for Fitzpatrick types 1, 2, or 3 and that’s just due to the amount of pigment that’s in darker skin colors and that’s very normal as well.
“Grant, how does your skin feel after the treatment?”
“It feels good.”
“Does it feel warm at all?”
“A little bit, but not too bad.”
Ok, so a little bit of heat sensation like a mild sunburn is natural and normal. That usually lasts about two to three hours and then it should dissipate. If it’s bothersome or uncomfortable, you can take some Tylenol or some ibuprofen to help with that. You can also put ice packs on your skin to help with that as well. Your next treatment should be in roughly six weeks or so and then you can also use any of your regular products. For instance, when you shower, wash your face with soap, that’s fine, and any aftershaves or lotions that you normally put on your face is totally ok to use as well. But, you do want to use a good SPF on your skin for any time you’re in the sun for good sun protection after having a laser treatment.

•เป็นโรคที่มีการสะสมของสารโปรตีนชนิดหนึ่ง คือ Amyloid สะสมใต้ผิวหนังในชั้นหนังแท้ ทำให้เกิดอาการเป็นตุ่มแข็งๆ สีน้ำตาลแดง มีอาการคัน พบได้บ่อยที่หน้าแข้งทั้ง 2 ข้าง และอาจพบได้ที่เท้า, ต้นขา
•โรคนี้เกิดจากมีการ สะสมของสารอะมัยลอยด์ (Amyloid) ที่บริเวณหนังแท้ส่วนบน ทำให้เกิดผื่นมักเกิดบริเวณหน้าแข้ง และมีอาการคัน
•มีลักษณะ ผิวหนังพบบ่อยบริเวณหน้าแข้งมีการหนาตัวขึ้น ลักษณะเป็นตุ่มนูน ***เรียงกันคล้ายกับสร้อยลูกปัด rippled pattern***
•โดยตุ่มนูนจะอยู่ระหว่างรูขุมขน มีอาการคันบ้าง แต่ไม่รุนแรง ลูบๆก็ทุเลาลง
🔬ชื่อโรค Lichen amyloidosis อาจจะฟังดูไม่คุ้นแต่ในความเป็นจริงสามารถ พบได้บ่อย ในชาวเอเชีย เกิดจากความผิดปกติของเซลล์ผิวของเรา การเกิดขึ้นของโรคนี้บางครั้งมีปัจจัยกระตุ้น บางครั้งอาจไม่พบ
👉แต่ส่วนใหญ่เหตุกระตุ้นที่พบได้บ่อยคือการเสียดสีที่บริเวณนั้นบ่อยๆ เช่นการ เกา การถู ดังนั้นน บริเวณที่พบโรคนี้ได้บ่อยมักเป็นที่หน้าแข้งเนื่องจากเราต้องใส่การเกง ถุงเท้า ถุงน่อง กันเป็นประจำ
🧬การเสียดสีซ้ำไปซ้ำมาจะเป็นตัวการทำให้เกิดการทำลายของเซลล์ผิวหนัง(keratinocyte) เมื่อเกิดการตายของเซลล์ผิวหนัง มันจะไปกระตุ้นให้เกิดการสร้างเส้นใยโปรตีนออกมา (Amyloid protein) คล้ายกับที่เวลาเราล้มเป็นแผลถลอกแล้วมีการสร้างสะเก็ดแผล แต่กระบวนการสร้างโปรตีนนั้นไม่สมบูรณ์ เป็นโปรตีนผิดรูป เส้นใยโปรตีนจึงไม่สามารถนำไปใช้งานได้ ต้องทิ้งสถานเดียว แต่เนื่องจากเป็นโปรตีนที่ไม่ได้มาตรฐาน กลไกการขับออกจากร่างกายจึงบกพร่อง กลับกลายเป็นสะสมไว้ใต้ผิวหนังเป็นตุ่มนูนหนา คล้ายผื่นขึ้นมาแทน การสะสมของโปรตีนใต้ผิวหนังจะไปรบกวนปลายประสาททำให้เกิดอาการคันขึ้นได้ด้วย
-โดยส่วนใหญ่คนไข้ที่เป็นโรคนี้มักจะรำคาญและอาการคันและลักษณะผื่นที่ดูเหมือนสกปรก
-โดยคนไข้จะพยายามขัดถูผิวบริเวณนี้เป็นประจำเพื่อให้ผื่นหลุดออกไป แต่กลับกลายเป็นการกระตุ้นให้เกิดการเสียดสีมากขึ้น ผื่นจึงเป็นมากขึ้น
✅ดังนั้นการดูแลรักษาจึงจำเป็นต้องปรับความเข้าใจในการดูแลรักษาผิว ครับโดยงดขัดถูแกะเกา***
✅และต้องทาครีมให้ความชุ่มชื้น moisturizer ผิวเยอะๆดลยครับ***
•หากใครไม่แน่ใจว่าเป็นโรคนี้หรือไม่ แนะนำให้พบแพทย์เฉพาะทางผิวหนังเพื่อตรวจวินิจฉัยเพื่อให้การรักษาได้อย่างถูกวิธีครับ

-ที่ทำให้อาการดีขึ้นได้ (คันลดลง ผื่นเรียบลง) มีหลายวิธี การตอบสนองขึ้นกับแต่ละบุคคลและควรปรึกษาแพทย์ก่อนการรักษา
-แต่ผลการรักษา รักษาค่อนข้างยากและมีโอกาสเป็นซ้ำบ่อยถ้ายังมีการแกะเก่าอยู่ครับ***
•การรักษามีทั้งที่ใช้ยาทา ผลัดเซลล์ผิว Peeling ฉายแสง ไปจนถึงการใช้เลเซอร์หลายชนิดครับ
และที่สำคัญคืองดเกาขัดถูครับ
-การทายา topical steroid หรือยาทาสเตียรอยด์ จะช่วยลดอาการคันได้รวดเร็ว แต่การทาควรอยู่ในดุลยพินิจของแพทย์ผู้รักษา เพื่อปรับความแรงตามความหนาหรืออาการของโรค หากทายาผิดวิธีจะมีผลข้างเคียงได้ครับ
-การทายา กลุ่ม calcinurin inhibitors
-การใช้ยาทากลุ่มที่ทำให้เกิดการผลัดผิว เช่น salicylic acid

Nd:YAG lasers are optically pumped using a flashtube or laser diodes. These are one of the most common types of laser, and are used for many different applications.
Nd:YAG dental lasers are used for soft tissue surgeries in the oral cavity, such as gingivectomy, periodontal sulcular debridement, LANAP, pulpotomy, frenectomy, biopsy, and coagulation of graft donor sites.
Nd:YAG lasers are used in manufacturing for engraving, etching, or marking a variety of metals and plastics, or for metal surface enhancement processes like laser peening.
Nd:YAG lasers can be used for flow visualization techniques in fluid dynamics .
Nd:YAG lasers are frequently used to build optical tweezers for biological applications.
The Nd:YAG laser is the most common laser used in laser designators and laser rangefinders.
Nd:YAG laser
Nd:YAG (neodymium-doped yttrium aluminium garnet; Nd:Y3Al5O12) is a crystal that is used as a lasing medium for solid-state lasers. The dopant, triply ionized neodymium, Nd(III), typically replaces a small fraction (1%) of the yttrium ions in the host crystal structure of the yttrium aluminium garnet (YAG), since the two ions are of similar size. It is the neodymium ion which provides the lasing activity in the crystal, in the same fashion as red chromium ion in ruby lasers.
Laser operation of Nd:YAG was first demonstrated by J. E. Geusic et al. at Bell Laboratories in 1964.
Technology
Nd:YAG lasers are optically pumped using a flashtube or laser diodes. These are one of the most common types of laser, and are used for many different applications. Nd:YAG lasers typically emit light with a wavelength of 1064 nm, in the infrared. However, there are also transitions near 940, 1120, 1320, and 1440 nm. Nd:YAG lasers operate in both pulsed and continuous mode. Pulsed Nd:YAG lasers are typically operated in the so-called Q-switching mode: An optical switch is inserted in the laser cavity waiting for a maximum population inversion in the neodymium ions before it opens. Then the light wave can run through the cavity, depopulating the excited laser medium at maximum population inversion. In this Q-switched mode, output powers of 250 megawatts and pulse durations of 10 to 25 nanoseconds have been achieved. The high-intensity pulses may be efficiently frequency doubled to generate laser light at 532 nm, or higher harmonics at 355 and 266 nm.
