Diabetes Mellitus is also simply known as diabetes. It is the disease characterized by a malfunctioning metabolism and a high blood sugar level. The result can be low levels of insulin or abnormal insulin resistance. This mixed with inadequate levels of insulin secretion results in diabetes. Symptoms of diabetes mellitus include increased urine production, excessive thirst, extreme fatigue, and excessive thirst and weight loss. These symptoms though may not be present in those people with only mildly elevated sugar levels. Diabetes mellitus includes type 1, type 2 and gestational diabetes, which occur only during pregnancy. ABSTRACT: Rates of fructose consumption continue to rise worldwide, and have been linked to rising rates of obesity, type-2 diabetes mellitus, and metabolic syndrome. Elucidation of fructose metabolism in liver and fructose action in brain demonstrate three parallelisms with ethanol. First, hepatic fructose metabolism is similar to ethanol in that by accelerating the process of de novo lipogenesis, both promote hepatic insulin resistance, dyslipidemia, and hepatic steatosis. Second, fructosylation of proteins with resultant superoxide formation can result in inflammation similar to acetaldehyde, an intermediary metabolite of ethanol. Lastly, by stimulating the “hedonic pathway” of the brain both directly and indirectly, fructose creates habituation, and possibly dependence; also paralleling ethanol. On a societal level, the treatment of fructose as a commodity on the open market exhibits similarities to ethanol. Fructose induces alterations in both hepatic metabolism and central nervous system energy signaling, leading to a “vicious cycle” of excessive consumption and disease consistent with metabolic syndrome. These dose-dependent actions of fructose on the liver and on the hedonic pathway of the brain recapitulate the effects of ethanol.

Bio:
Lynda Frassetto, MD, is a Professor of Medicine and Nephrology at the University of California San Francisco. She is the director of a clinical research center at UCSF, supervises patient care at three of the University’s hospitals, and helps teach courses on improved communications and behavioral stress modification techniques.
Abstract:
Obesity is one of the major health problems in the world today, in both developed and third world countries. Being overweight or obese increases the risk of developing insulin resistance and diabetes; about 85-90% of people with type 2 diabetes are overweight or obese.
Nutritional guidelines for diabetes rely on moderating carbohydrate intake and promoting weight loss. Recent meta- analysis suggest that weight loss, and not any particular diet, is the most important intervention for improving glucose levels (1,2). A recent Cochrane review of dietary advice for diabetes control concluded, “There are no high quality data on the efficacy of the dietary treatment of type 2 diabetes”(3).
There is, however, evidence in short term studies, that dietary composition can affect glucose control independent of weight loss. Thus studies of isocaloric Paleolithic-type diets demonstrate improvement of glucose levels and insulin sensitivity. Which component(s) of the “paleo-diet” is responsible for these improvements are not known. The diet is high in fiber and very large increases in fiber in the diet can lower glucose levels. Another possibility is that the quantity or quality of foods can alter neuro-hormonal control of appetite and satiety. In our presentation, we will review evidence available from clinical trials comparing dietary treatments for diabetes.

Abstract:
The Food Pyramid, established by the USDA in 1982, recommended ingestion of 55-70% of calories as carbohydrate. The subsequent 30 years showed a growing epidemic of obesity, metabolic syndrome, type 2 diabetes mellitus, as well as identifiably increased risks for cancer, arthritis and cardiovascular disease. The Food Pyramid has recently been abandoned. Recommendations for carbohydrate intake now are in the range of 40-55% of total calories. Some propose that we should increase consumption of whole grain foods, natural fruits, etc. Other approaches recommend reducing total carbohydrate consumption further than current recommendations on grounds that 90% of the carbohydrates ingested nowadays are sugars or starches that digest to sugars.These are points of discussion, but regardless of specific dietary recommendations, we believe that a new dietary paradigm can be identified that calls for reducing overall insulin signaling. Benefits include reduced risks for type 2 diabetes, obesity, dyslipidemias, cardiovascular disease, arthritis as well as cancers: in short, many of the chronic diseases of our time.The panel and audience will discuss risk factors for chronic diseases and their inter-relations, mechanisms of insulin signaling that appear likely to contribute to these relationships challenges and possible suggestions toward improving dietary patterns.
