A perforated eardrum is a hole in the eardrum. We call it a tympanic membrane perforation and there are many different causes. The most common cause is an ear infection that ruptures the eardrum. And such perforations always heal spontaneously. However, if you have repeated ear infections and repeated ruptures the hole might not close. The second most common cause of a hole in the eardrum is trauma. And this can be direct trauma, from using a Q-tip or sticking a foreign body in your ear or from being hit over the ear. So sometimes if someone is slapped over the ear pressure will push through the eardrum. Whenever your eardrum ruptures you should immediately seek medical attention. Particularly if you're having any dizziness associated with this. A ruptured eardrum will usually cause some degree of hearing loss. Many ruptured eardrums will heal spontaneously and they should be given some time to heal. After a period of time if there's no evidence of healing a ruptured eardrum can be repaired surgically.

It happened during snorkelling - diving to about 3m without equalizing properly.
Was painful the day it happened. A little painful with water still coming out the next day. Not painful at all on the third day.
There is a mild hearing loss in the higher frequenzies (2,5kHz and up). Will supposedly heal and go back to normal hearing within 3-6 weeks.

This is a video of one of our patients in Salt Lake City. He has severe ear disease and ruptured ear drums. His ear drum is totally gone in his left ear and when he sniffs or gets excited, you can hear air moving in and out of his ear (from the back of his throat). Dr. Miller flushed out his ears with video-otoscopy equipment and he is currently undergoing a diet trial for possible food allergies. He came in for a recheck 2 weeks after ear flushing and is doing very well. The owners saved him about a year ago from a hoarding situation-he is one lucky dog!!! A perforated eardrum or punctured eardrum is a rupture or perforation (hole) of the eardrum which can occur as a result of otitis media infection, trauma (e.g. by trying to clean the ear with sharp instruments), explosion, loud noise or surgery (accidental creation of a rupture). Flying with a severe cold can also cause perforation due to changes in air pressure and blocked eustachian tubes resulting from the cold. This is especially true on landing.
Perforation of the eardrum leads to conductive hearing loss, which is usually temporary. Other symptoms may include tinnitus, earache or a discharge of mucus.
The perforation may heal in a few weeks, or may take up to a few months. Some perforations require intervention. This may take the form of a paper patch to promote healing (a simple procedure by an ear, nose and throat specialist), or surgery (tympanoplasty). However, in some cases, the perforation can last several years and will be unable to heal naturally. Such cases are usually a result of a perforation being surgically induced during an operation involving the ear.

Retracted Ear Drum : A retracted eardrum refers to a condition wherein the eardrum gets pulled or sucked into the area occurring behind it. This occurs when the pressure in the middle ear area becomes very low. A retracted eardrum is mostly caused due to repeated infections. The eardrum comprises two parts, the pars tensa, which is the main part of the eardrum, and the pars flaccida, which is a smaller part of the eardrum located above the pars tensa. Either or both of these parts may become retracted. The retracted segment of eardrum is often known as a retraction pocket. The terms “atelectasis” or sometimes “adhesive otitis media” can be used to describe retraction of a large area of the pars tensa. Symptoms of a Retracted Ear Drum : Many people with retracted ear drums experience some hearing loss which often goes away with adequate treatment. Other symptoms vary depending on the cause of ear drum retraction. Causes a Retracted Ear Drum- The following conditions are associated with auditory tube dysfunction and may cause a retracted ear drum: 1) middle ear infections 2) fluid in the ears (also called otitis media with effusion or serous otitis media) 3) auditory tube dysfunction from an upper respiratory infection, chronic allergies, sinusitis, or enlarged adenoids or tonsils which may block the tube 4) a history of chronic rupturing of the ear drum or a ruptured ear drum that does not heal properly A retracted ear drum is a sign of auditory tube dysfunction and the underlying cause needs to be found and treated. If left untreated, negative pressure inside the middle ear can lead to other problems including: 1) erosion of the ear canal 2) erosion of the small bones in the ear (specifically the incus and stapes) and possibly permanent hearing loss 3) cholesteatoma -

His ear drum is totally gone in his left ear and when he sniffs or gets excited, you can hear air moving in and out of his ear (from the back of his throat). Dr. Miller flushed out his ears with video-otoscopy equipment and he is currently undergoing a diet trial for possible food allergies. Retracted Ear Drum :
A retracted eardrum refers to a condition wherein the eardrum gets pulled or sucked into the area occurring behind it. This occurs when the pressure in the middle ear area becomes very low. A retracted eardrum is mostly caused due to repeated infections.
The eardrum comprises two parts, the pars tensa, which is the main part of the eardrum, and the pars flaccida, which is a smaller part of the eardrum located above the pars tensa. Either or both of these parts may become retracted. The retracted segment of eardrum is often known as a retraction pocket. The terms “atelectasis” or sometimes “adhesive otitis media” can be used to describe retraction of a large area of the pars tensa.
Symptoms of a Retracted Ear Drum :
Many people with retracted ear drums experience some hearing loss which often goes away with adequate treatment. Other symptoms vary depending on the cause of ear drum retraction.
Causes a Retracted Ear Drum-
The following conditions are associated with auditory tube dysfunction and may cause a retracted ear drum:
1) middle ear infections
2) fluid in the ears (also called otitis media with effusion or serous otitis media)
3) auditory tube dysfunction from an upper respiratory infection, chronic allergies, sinusitis, or enlarged adenoids or tonsils which may block the tube
4) a history of chronic rupturing of the ear drum or a ruptured ear drum that does not heal properly
A retracted ear drum is a sign of auditory tube dysfunction and the underlying cause needs to be found and treated. If left untreated, negative pressure inside the middle ear can lead to other problems including:
1) erosion of the ear canal
2) erosion of the small bones in the ear (specifically the incus and stapes) and possibly permanent hearing loss
3) cholesteatoma

I have to tell you that I've only seen pictures of infected earlobes before. I've never been party to treating one of these until now. Not that they are much more complicated than any of the hundreds of abscesses that I have drained in the past but they are rather rare to show up like this one.
This poor soul was more than a little concerned that this was going to be extremely painful. We went out of our way to use a tuberculin syringe and needle to inject the anesthetic. That coupled with our use of the magic spray made the initial anesthetic virtually pain free.
What happens next you have to see to believe.
Gonzo couldn't help out on this one today. I know there are a lot of Gonzo fans out there so just in case you were worried she is just fine but didn't make it in on this one. Thanks to Jenny who gladly stepped up to help out!
Yes, we did take fluid for culture. No needles were capped. Pain medication and medication for nausea were prescribed. The patient also received a pain shot and an antibiotic shot.

It is not clear whether the test was authentic, and if it was, whether it will be considered a success by the North.
