Cysts
Cysts are harmless, sac-like growtsh in the deeper layers of the skin. They form from the lining of a hair follicle that gets blocked. It is not known why cysts appear, nor why some persons get many of them.
Types of cyst include:
Acne comedones (whiteheads)
Acne cysts (large uninflamed acne lumps)
Dermoid cyst (a developmental inclusion cyst)
Milia (tiny surface white balls often found on the cheeks after sun exposure or following an injury). Milia can easily be squeezed out.
Epidermal cysts (soft cheese-like contents)
Trichilemmal cysts (scalp cysts, often multiple and familial, arising from hair root sheath)
Pilar cysts (firm white content)
Steatocystoma multiplex (multiple inherited cysts on trunk)
Digital myxoid cyst (cyst at the base of a nail)
Pseudocyst of auricle (cyst on the external ear)
Labial mucous cyst (cyst in the lip)
Apocrine hidrocystoma (clear jelly-like cyst of eyelid)
Bartholin's cyst (vulval swelling)
Benign cysts may sometimes be confused with skin cancers, especially a nodular or 'cystic' basal cell carcinoma.
The epidermal cyst sac is filled with a soft, whitish brown material that sometimes oozes out onto the skin's surface. This material, which is keratinous debris (dead skin cells), smells like rotten cheese.

Ruptured cysts
Cysts can get inflamed if the contents of the cyst rupture into the surrounding skin. This makes them red and painful, and they may discharge yellow pus.
Occasionally bacteria enter the cyst and cause an infection which resembles a boil. When this happens, antibiotics such as flucloxacillin taken by mouth and minor surgery may be needed to relieve the pressure and pain.
Treatment
If you have a cyst, ask your dermatologist's or doctor's advice.
Small cysts (eg. less than 5 mm) don't usually need treatment, but can be readily removed by a minor surgical procedure. Larger ones are usually removed because they are unsightly or because they have been inflamed.
Cysts are treated by making a small surgical opening into the skin and removing the sac (excision biopsy). This is done under local anaesthetic and may require stitches, removed a few days later.
Very occasionally the cyst recurs and needs further treatment.

Huge Infected Sebaceous Cyst Back Drainage .
This 74 year old man had a huge sebaceous cyst on his back which increased and become extremely tender for last one month. patient given a course of antibiotics but no benefit found.so incision and drainage of pus planned.
i usually do these procedures under local anesthesia.
2 % lignocaine with adrenaline is used for field block before pus drain.
with a no 11 blade stab incision given over the cyst.
pus drained with gentle pressure from surroundings.
proper drainage is essential .
in inflammed cases the wall starts oozing with sudden decompression which is seen in this video , pus mixed with blood coming out.
i put my little finger inside the cavity to ensure all loculi broken with no residual pus inside the cavity.
as it is a inflammed cavity with inflammed wall ,complete removal of cyst wall not possible.
so i use a scoop to scrape the wall to remove the cyst wall as much as possible.
after the procedure i just pack the cavity with betadine soak gauge piece which i remove after 24 hours.
then patient needs daily dressing, course of antibiotics .
if patient is diabetic ,blood sugar control is essential.

Large Inflamed Sebaceous Cyst Back
This Man is an astrologer who visited me with complain of painful swelling over his back for one week.On examination I found it was a large red , extremely tender cystic swelling over his back.It was painful,elevated red with local rise temperature.All the signs of inflammation was there.These inflamed cysts also described as angry looking cysts because of its red swelling.One can see the punctum in the center of the swelling.It indicates it is an inflamed(Angry looking) Sebaceous Cyst(epidermoid cyst) . There is fluctuation present ,which indicates liquid pus inside the cyst as the content.This cyst needs definitely incision and drainage of pus.
Before pus drainage , a course of antibiotics reduces the inflammation .If the patient is diabetic ,control of blood sugar also essential before the procedure.
After drainage of pus the inflammatory changes reduces very fast and the swelling subsides.The wound heals within one week time.
