Totally Laparoscopic Pancreaticoduodenectomy (Laparoskopik Whipple)
Dr. Mehmet Kaplan, Gaziantep, Turkey Although the rapid development in laparoscopic surgery, pancreaticoduodenectomy is still one of the most discussed and technically demanding surgery performed only at few centers in the world. This report describes a totally laparoscopic pylorus-preserving pancreaticoduodenectomy for a tumor of ampulla of Vater with a successful outcome, representing the first description of this laparoscopic procedure in Turkey. The patient was a 58-year-old male patient with the diagnosis of ampullary tumor. The operation was performed with 5 ports. Following resection, all anastomoses were made intracorporeally. A double layer pancreaticogastrostomy, an end-to-side choledochojejunostomy, a two layer gastrojejunustomy and drain placements completed the operation. The operating time was 510 minutes, and estimated blood loss was 350 ml. Surgical margins were negative and number of retrieved lymph nodes was 16. The patient was discharged on postoperative day 6 without complication. This case demonstrates that laparoscopic pancreaticoduodenectomy is a feasible operative procedure in carefully selected patients. This technique can achieve adequate margins and follow oncological principles. However randomized comparative studies are needed to establish the superiority of minimal invasive surgery over traditional open surgery. A team led by Dr. Song-Cheol Kim from AMC has performed the laparoscopy-assisted pancreaticoduodenectomy procedure over one hundred times and set a world record in this field. In the period between May 2007 and February 2012, AMC surpassed the remarkable record of one hundred such procedures. This number is significantly higher than the second-highest institutional record, held by the Mayo Clinic with sixty cases. Along with attaining this high record, an amazing success rate of 99% was also achieved at AMC. Even well-established institutions overseas have performed no more than fifty or sixty. Pancreatic Cancer Whipple Surgery Pancreaticoduodenectomy Heidelberg Hospital Recovery From Pancreas Removal Pancreatitis Cancer Survival Liz, 48 was diagnosed in the UK with unresectable locally advanced pancreatic cancer on the 30/03/12. The UK surgeons considered it too advanced for surgery so we turned to Germany's leading cancer centre in Heidelberg. Liz underwent a pancreaticoduodenectomy in Heidelberg carried out by Professor Buchler on the 30/04/12. We have nothing but praise for the surgical and medical care she received, meaning we left Heidelberg free of all viewable cancer to convalesce in Liemen and then to fly back to the UK for Chemotherapy. We remain deeply thankful for the emotional, financial and practical help we received from family and friends. This is her story so far in pictures.

For surgeons, pancreatic cancer is the most challenging of the gastrointestinal malignancies. The tumor has spread in 80% of cases even before the diagnosis is made, so that surgical resection is an option in only about 20%. The operation (Whipple procedure) is the most complex of general surgical procedures, and cures less than 20% of those who undergo it. Nevertheless, some patients are cured by surgery (and additional medical therapy), and all patients who have ever been cured have had the tumor surgically removed. Thus, surgical resection will remain a central feature in the management of this disease for the foreseeable future.

PLEASE MIRROR THIS VIDEO My friends and I may have fun with Kevin, like shooting bottle rockets at him while he does the Moonwalk, but he does have a very sobering story to tell about his father, Chris. About two years ago, Chris Hanley was diagnosed with chronic pancreatitis, which is widespread inflammation of the pancreas. Apart from being excruciatingly painful, it is more often than not fatal. He has had several surgeries in the past two years--a pancreaticoduodenectomy (the Whipple procedure), and several surgeries to install drains throughout his gastrointestinal tract. He is currently taking about 30 different prescriptions every day. The reason why Chris is still alive today is because of his $1100-per-month health insurance policy. However, not all Americans are so lucky. Nearly 50 million Americans are without any health coverage at all. I could not begin to fathom what the struggle would be for anyone who has to go through what Chris has to go through, let alone going through it without adequate health coverage.
