Republican presidential candidate Mitt Romney raised eyebrows yesterday when he said he would "not get rid of all of healthcare reform." Romney's statement made headline news because it differed from his previous rhetoric of "repealing ObamaCare." We would rather hear politicians explain the real reasons for why a trip to the ER for a headache can amount to a bill of $10000. Could it be that healthcare isn't actually that expensive? Over the years healthcare costs in the US have increased from 5% of GDP in the 60s to 17.4% in 2009, according to an OECD report published in 2011. A recent report from the Institute of Medicine calculated systemic waste in the US healthcare system at $765 billion, representing 30% of total expenditures. We ask Doctor Keith Smith, Managing Partner and Medical Director for the Surgery Center of Oklahoma, about the role of insurance companies, wasteful administration costs, and hospital inefficiencies. Keith Smith runs an independent surgical clinic and brings free market competitive prices to surgery. He offers customers lower prices and patients fly in from all over the world, even from countries with universal healthcare systems, to get surgery at his center. Three years ago, Dr. Keith Smith, co-founder and managing partner of the Surgery Center of Oklahoma, took an initiative that would only be considered radical in the health care industry: He posted online a list of prices for 112 common surgical procedures. The 51-year-old Smith, a self-described libertarian, and his business partner, Dr. Steve Lantier, founded the Surgery Center 15 years ago, after they became disillusioned with the way patients were treated at St. Anthony Hospital in Oklahoma City, where the two men worked as anesthesiologists. In 1997, Smith and Lantier bought the shell of a former surgical center with the aim of creating a for-profit facility that could deliver first-rate care at a fraction of what traditional hospitals charge. The major cause of exploding US heath care costs is the third-party payer system, a text-book concept in which A buys goods or services from B that are paid for by C. Because private insurance companies or the government generally pick up most of the tab for medical services, patients don't have the normal incentive to seek out value. The Surgery Center's consumer-driven model could become increasingly common as Americans look for alternatives to the traditional health care market—an unintended consequence of Obamacare. Weight Bias in Health Care
Overweight and obese patients frequently feel stigmatized in health care settings, and face stereotypes and prejudice from health care providers. These stigmatizing experiences (also called 'weight bias') jeopardize patients' emotional and physical health. The Rudd Center for Food Policy and Obesity at Yale University (www.yaleruddcenter.org) has released this new video in response to a growing concern about weight bias in health care. The video, hosted by celebrity and activist Emme and featuring Rudd Center experts including Dr. Rebecca Puhl and Dr. Kelly Brownell, uses expert commentary and dramatic representation to increase awareness of bias and stigma that overweight and obese patients encounter in health care. Equally importantly, the video presents a range of practical strategies to help providers reduce bias in their clinical practice, and to optimize the health care experience for their overweight and obese patients.
