Accident Case Study: Hazardous Attitudes
It’s an overcast afternoon on February 3, 2019, when the pilot of a Cessna 414 Chancellor departs Fullerton Municipal Airport in California. He plans to fly VFR to Minden, Nevada, 320 nautical miles to the north.
Join the AOPA Air Safety Institute as we follow the flight that four minutes after departure encounters instrument meteorological conditions - weather air traffic control had warned the pilot about. Within another two minutes things turn from bad to ugly, in microburst conditions, turbulence, and rain showers. Learn how hazardous attitudes can betray pilots who don’t heed warnings.

Accident Case Study: Faulty Assumptions
In the early morning light on April 22, 2019, the pilot of a Beechcraft Baron 58 preflights his aircraft at West Houston Airport in Texas, before he heads to the terminal to greet his five passengers.
The flight will take just over one hour. But the pilot’s mistaken beliefs about existing onboard fuel will prove disastrous and clinch the flight’s outcome.

Early Analysis: Midair Collision March 7, 2023 Winter Haven, FL
On March 7, 2023, a Piper Warrior and a Piper J-3 Cub on floats collided over Lake Hartridge, just east of Winter Haven Regional Airport in Winter Haven, Florida. Sadly, all four people on board the airplanes perished in the accident. The AOPA Air Safety Institute gives an early look into the factors surrounding the crash and addresses notable portions of the flight and highlights areas the NTSB will likely probe.
