lasikcomplications.com Corneal ectasia is one of the most feared and dreaded complications of LASIK. The rate of ectasia after LASIK is uknown due to failure of LASIK surgeons to report complications and lack of long-term followup. There are likely several thousand patients suffering from post-LASIK ectasia in the United States. The medical literature contains reports of late onset ectasia occuring several years after LASIK. Reported risk factors for ectasia after LASIK include abnormal corneal topography, insufficient stromal bed thickness, high myopia, and age younger than 25 years. Operating on patients with pre-op thin corneas should be avoided. Reported treatments for the management of post-LASIK ectasia include hard contact lenses, intraocular pressure-lowering drugs, INTACS, and corneal collagen crosslinking. A corneal transplant may be required. Patients who suffer from post-LASIK ectasia should report it to the FDA by filing a MedWatch report (www.accessdata.fda.gov LASIK surgeons deliberately hide this serious complication by diagnosing it as "keratoconus", a naturally occuring disease of the cornea.

This endoscopic video is from a patient with gastric antral vascular ectasia, also known as GAVE, performed by Dr. Joseph Galati, is a cause of intestinal bleeding in patients with cirrhosis and portal hypertension. Patients present with anemia and iron deficiency. The diagnosis is made with endoscopy, and one of the treatments is argon plasma coagulation (APC). This is from a 60 year old women with non-alcoholic fatty liver disease awaiting liver transplant at The Methodist Hospital in Houston, Texas.

Corneal ectasia following LASIK surgery causes changes in the shape of the cornea and can lead to decreased vision. In this episode of A State of Sight, Isaac Porter, MD explains post-LASIK ectasia and discusses some of the risk factors that are thought to be associated with it. Laser vision correction procedures thin the cornea with the excimer laser to include the power needed in glasses or contact lenses onto the eye. Corneal ectasia is a change seen after LASIK that results in a progressive steepening and distortion of the cornea. After the procedure, corneas that develop ectasia may not have enough strength to maintain a consistent shape. This can result in a loss of the correction from the laser treatment and distorted or decreased vision. Initially, this could be corrected with glasses or contact lenses, but may require collagen crosslinking or corneal transplantation in severe cases. Risk factors for post-LASIK ectasia that have been identified include thinner corneas, larger laser treatments, younger age patients, and abnormal corneal topography. Pellucid marginal degeneration, PMD, corneal ectasia - A State of Sight #30

In pellucid marginal degeneration (PMD), the cornea progressively thins and changes shape, particularly in the lower cornea. This can cause a large degree of astigmatism and distort the vision. Initially patients with PMD may need glasses to correct their vision, but as the condition advances, rigid gas permeable contact lenses may be required to see clearly. In advanced stages, surgery may be needed to restore vision with a corneal transplant, collagen crosslinking, or other advanced corneal procedures. Capsule Endoscopy - Gastric Antral Vascular Ectasia (GAVE)

Gastric antral vascular ectasiaes is a rare cause of gastrointestinal bleeding and iron deficiency anaemia. This condition was first described by JA Rider, AP Klotz and JB Kirsner in 1953. Becaese of the appearance of this disease it is also called "watermelon stomach". The etiology is unknown. It can be associated with portal hypertension, scleroderma and perhaps liver cirrhosis. Endoscopic argon plasma coagulation is the treatment of choice. This video shows GAVE in the gastric antrum seen during a capsule endoscopy - a standard upper endoscopy is of course sufficient to get a firm diagnose of this disease.
