Physician Concerns with ICD-10
Much of the anxiety on the part of physicians really relates to concerns that the conversion to ICD-10 will, in some way, cause a disruption in their payment. Now, AMA is arguing that because of that, the whole system should be postponed. The focus should not be on postponing ICD-10, but instead ensuring that all the parties, the payers, CMS, and so on, actually have contingency plans in place to ensure that ICD-10 will not cause a disruption in the payment to physician offices. If we look at the typical practice of the small physician, they use a super bill, a standard form that's a check-off form in which they have all the diagnoses, all the procedures that they typically deal with. So the conversion to ICD-10, for a typical physician's office, pretty much involves their vendor taking their super bill, as it's called, converting all the check-off boxes, maybe adding a few new check-off boxes that are reflective of the additional specificity in I-10, and doing that one-time conversion. AHIMA has done a number of studies where they looked at the level of difficulty in converting a super bill, and what they've found, that it would typically take less than one day's worth of effort to convert a typical physician office super bill to ICD-10.

I think so many payers and providers have already started down the path, that I think they're going to continue. They've made huge investments in ICD-10 and I suspect that technologies will emerge where they can start to begin to use ICD-10, while ICD-9 is still in place. And I think what they'll find is that the granularity in ICD-10 allows them to improve their business operations and their clinical operations. To lose the discrimination between different types of conditions and procedures really ends up impairing our ability to track, monitor, understand the different types of conditions and their prevalence, as well as how we're treating them. Right now, our vocabulary for conditions is limited by the number of ICD-9 codes we have, and so as we want to better understand procedures, for example, ICD-10 gives us that granularity, if there is a artery repair, to actually know whether it's in your arm or your aorta, or in your neck, starts to be able to match the resources it takes to actually trace it to the event that occurred; also helps us understand what procedures and events are more common, or less common, so that we can plan. Do we need more or less of the supplies, more or less of the room? How do we staff for the different needs? So you can kind of play out how extra granularity just starts to make insights much more available.
