This is an example of small-incision cataract and intra-ocular lens implant surgery I filmed about four years ago, as viewed through the operating microscope. It uses a one-handed technique which I find very helpful in certain cases, especially for "soft" cataracts.

This 3D medical animation depicts the phacoemulsification and extracapsular removal of a cataract (cloudy lens), and the placement of an artificial lens. The lens of the eye is responsible for focusing images onto the back of the eye. It is normally transparent. As a normal part of aging, the lens begins to cloud and causes a gradual, painless loss in vision. Cataract removal is most often performed to better examine the back of the eye when monitoring for damage from certain diseases such as diabetes or glaucoma and to improve vision. There are two main types of cataract removal. The large majority of cataract surgeries are performed using the phacoemulsification technique. During the phacoemulsification technique an ultrasound probe breaks the cloudy lens into tiny fragments. The fragments are vacuumed out through a tiny incision. An intraocular lens implant is then inserted to replace the natural lens that was removed. Because the incision is tiny, stitches are often not necessary and visual improvement is usually noted relatively soon after surgery. During the extracapsular technique the cataract is removed as one entire piece. This requires a larger incision and stitches. An intraocular lens implant (artificial lens) is inserted to replace the natural lens that was removed. Recovery is usually slower, due to the larger incision. This 3D medical animation shows the formation of a cataract- the clouding of the eye's lens that leads to decreased vision and its removal by two different processes; phacoemulsification and extracapsular removal. In phacoemulsification a tiny probe is inserted into the eye. The probe emits ultrasound waves that break up the cloudy lens into small fragments, then a suction device removes these fragments. This is the most common form of cataract removal surgery. This procedure usually requires no stitches. In extracapsular removal of the cataract an incision is made in the eye, and the hard center of the lens is removed. At the conclusion of each procedure an artificial lens is insert into the eye. Eleanor Blakely, radiation biologist of the Life Sciences Division at Lawrence Berkeley National Laboratory, has been a scientist at Berkeley Lab since 1975. She is studying the effect of radiation on cataracts which concerns not only cancer patients, but also astronauts. An estimated 150000 people in Nepal are blind because of cataracts, but at the same time, Nepal has one of the highest rates of curable blindness anywhere in the world. One doctor in particular has been making remarkable progress in treating cataracts - and setting the example for many other developing countries, where 90 per cent of the world's blind reside. Sanduk Ruit, the Nepalese surgeon some call the "God of Sight", has removed 100000 cataracts over his 30-year career. As the surgeon's mobile eye camp received patients at a monastery in Dharding, hundreds of people, most of whom had never seen a doctor before, had travelled for days by bicycle, motorbike, bus and even on their relatives' backs to get an appointment. Al Jazeera's Subina Shrestha visits Ruit's clinic at Phulahari Monastery in remote Dharding, where at the end of the day, it is individual human experience that may be most rewarding of all.
