Brain tumor
A brain tumor or intracranial neoplasm occurs when abnormal cells form within the brain. There are two main types of tumors: malignant or cancerous tumors and benign tumors. Cancerous tumors can be divided into primary tumors that start within the brain, and secondary tumors that have spread from somewhere else, known as brain metastasis tumors. This article deals mainly with tumors that start within the brain. All types of brain tumors may produce symptoms that vary depending on the part of the brain involved. These may include headaches, seizures, problem with vision, vomiting, and mental changes. The headache is classically worse in the morning and goes away with vomiting. More specific problems may include difficulty in walking, speaking and with sensation. As the disease progresses unconsciousness may occur.
The cause of most brain tumors is unknown.Risk factors that may occasionally be involved include a number of inherited conditions known as neurofibromatosis as well as exposure to the industrial chemical vinyl chloride, the Epstein-Barr virus, and ionizing radiation. While concern has been raised about mobile phone use, the evidence is not clear. The most common types of primary tumors in adults are: meningiomas (usually benign), and astrocytomas such as glioblastomas. In children, the most common type is a malignant medulloblastoma. Diagnosis is usually by medical examination along with computed tomography or magnetic resonance imaging. This is then often confirmed by a biopsy. Based on the findings, the tumors are divided into different grades of severity.

Cushing's syndrome
Cushing's syndrome is a collection of signs and symptoms due to prolonged exposure to cortisol. Signs and symptoms may include: high blood pressure, abdominal obesity but with thin arms and legs, reddish stretch marks, a round red face, a fat lump between the shoulders, weak muscles, weak bones, acne, and fragile skin that heals poorly. Women may have more hair and irregular menstruation. Occasionally there may be changes in mood, headaches, and a chronic feeling of tiredness.
Cushing's syndrome is caused by either excessive cortisol-like medication such as prednisone or a tumor that either produces or results in the production of excessive cortisol by the adrenal glands. Cases due to a pituitary adenoma are known as Cushing's disease. It is the second most common cause of Cushing's syndrome after medication. A number of other tumors may also cause Cushing's. Some of these are associated with inherited disorders such as multiple endocrine neoplasia type 1 and Carney complex. Diagnosis requires a number of steps. The first step is to check the medications a person takes. The second step is to measure levels of cortisol in the urine, saliva or in the blood after taking dexamethasone. If this test is abnormal, the cortisol may be measured late at night. If the cortisol remains high, a blood test for ACTH may be done to determine if the pituitary is involved.

Cushing syndrome is a group of conditions caused by high levels of circulating cortisol or related corticosteroids.
Cortisol is a steroid hormone produced by the adrenal gland. Cortisol production is stimulated by the pituitary hormone ACTH. The secretion of ACTH is, in turn, promoted by the hypothalamic hormone CRH.
Cortisol helps the body deal with stress by mobilizing energy resources. It increases blood glucose level and makes glucose more available to the brain, as compared to other organs. For example, cortisol induces muscle cells to decrease glucose consumption and increase protein degradation, producing amino acids that can be converted into new glucose, a process known as gluconeogenesis. Cortisol also helps maintain high blood pressure to keep the body stay on high alert.
On the other hand, high levels of corticosteroids suppress inflammation and antibody production by B-cells. For this reason, cortisol and related products such as prednisone are used to treat inflammatory conditions and autoimmune diseases. These medications also help suppress the immune system to prevent organ rejection in transplant recipients.
Cushing syndrome usually results from excessive use of corticosteroid drugs, or overproduction of cortisol by the adrenal glands. A tumor in the adrenal gland may produce excess cortisol regardless of ACTH levels. On the other hand, the adrenal glands may be normal, but they are overstimulated by high ACTH levels produced by a tumor in the pituitary gland or another organ. This group of conditions is more specifically known as Cushing disease.
Corticosteroids cause obesity and alter the body fat distribution. Excessive fat develops specifically in the face and torso. Patients commonly have a round face, a hump on the back, and disproportionately slender arms and legs. Other signs include muscle wasting, thin and easily bruised skin, and purple streaks over the abdomen or chest. In the long term, patients may develop high blood pressure, diabetes, weakened bones, and vulnerability to infections.
There are several diagnostic tests for cortisol levels. Because normal cortisol secretion follows a daily rhythm - high in the morning and low at night, a high midnight cortisol level is an indicator of the disease.
ACTH levels are measured to determine the cause. Low ACTH levels suggest a primary adrenal cause, while high levels are indicative of a pituitary cause or an ectopic ACTH source.
Imaging tests can be used to detect tumors.
Supportive treatment includes diets high in proteins and potassium. If corticosteroid drugs are the cause, they may be gradually reduced to optimal levels. Certain medications can be used to block cortisol production, or block the effects of cortisol. Tumors are usually removed surgically or treated with radiation therapy.

Cushing syndrome occurs due to excess circulating glucocorticoids over a period of time. The most common cause is iatrogenic, including the use of topical and inhaled glucocorticoids. Endogenous causes of Cushing syndrome may be divided into adrenocorticotrophic (ACTH)-dependant and non-ACTH-dependent disease. ACTH-dependent disease arises when there is increased ACTH from either the pituitary (Cushing disease) or from an ectopic ACTH-producing tumor. Non-ACTH-dependent disease occurs due to either adrenal adenomas or adrenal carcinomas.
Cushing disease accounts for two-thirds of all cases of endogenous Cushing syndrome and is caused by excessive secretion of ACTH hormone by a pituitary microadenoma. This leads to increased secretion of cortisol (secondary adrenal hypercortisolism) from the adrenal cortex as well as increased levels of androgens. The typical cushingoid features seen in patients with Cushing syndrome arise because of the glucocorticoid excess. These cushingoid features are due in part to fat redistribution and typical skin changes.
Skin changes in Cushing syndrome are manifold. The skin becomes thin and translucent, and ecchymoses occur following minor trauma. Thinning of the epidermis also leads to the plethoric appearance of the skin. Hyperpigmentation of the skin may be seen but is more common in pituitary or ectopic ACTH production. Although generalized, it tends to occur on sun-exposed areas. Addison disease also results in a similar pattern of hyperpigmentation in sun-exposed areas, but it also occurs in the following areas: in the palmar creases, areas of friction, vermilion border, genital skin, and oral mucosa.
Acne, hirsutism, and menstrual irregularities, either oligomenorrhea or amenorrhea, arise due to the increased levels of androgens.
Cortisol hypersecretion also predisposes to hypertension, peripheral edema, glucose intolerance, gonadal dysfunction, osteoporosis, and proximal muscle weakness. Psychiatric symptoms may be present in a large majority of patients (more than 70%), and severity of symptoms range from anxiety to mania, depression, and frank psychosis. Insomnia presents early in the disease.
Ectopic ACTH production can also lead to Cushing syndrome. In these patients, because the onset of Cushing syndrome is sudden, there is no time to develop the characteristic clinical features. Instead, these patients will have sudden-onset hypertension, edema, glucose intolerance, and hypokalemia. Seventy-five percent of these tumors are small cell lung carcinoma. Ectopic ACTH is produced by bronchial carcinoids, thymic or pancreatic carcinoids, thymic neuroendocrine tumors, medullary carcinoma of the thyroid, or pheochromocytomas.
Patients with carcinoid tumors or pheochromocytomas usually have the typical cushingoid features, as these develop over a longer period of time. Due to the immunosuppressive effects of glucocorticoids, these patients commonly develop cutaneous staphylococcal, candidal, and superficial fungal infections. The incidence of pituitary-dependent adrenal hyperplasia is more common in women by 3-fold, with the average age of onset being in the 3rd and 4th decade.
Mortality is increased 4-fold compared with the general population, with cardiovascular disease being the major cause of death.
Although uncommon in children, obesity along with retarded growth may be a presenting feature.

Cushing's syndrome is caused by either excessive cortisol-like medication such as prednisone or a tumor that either produces or results in the production of excessive cortisol by the adrenal glands.[10] Cases due to a pituitary adenoma are known as Cushing's disease, which is the second most common cause of Cushing's syndrome after medication.[3] A number of other tumors may also cause Cushing's.[3][11] Some of these are associated with inherited disorders such as multiple endocrine neoplasia type 1 and Carney complex.[7] Diagnosis requires a number of steps.[4] The first step is to check the medications a person takes.[4] The second step is to measure levels of cortisol in the urine, saliva or in the blood after taking dexamethasone.[4] If this test is abnormal, the cortisol may be measured late at night.[4] If the cortisol remains high, a blood test for ACTH may be done.[4]
Most cases can be treated and cured.[6] If due to medications, these can often be slowly decreased if still required or slowly stopped.[5][12] If caused by a tumor, it may be treated by a combination of surgery, chemotherapy, and/or radiation.[5] If the pituitary was affected, other medications may be required to replace its lost function.[5] With treatment, life expectancy is usually normal.[6] Some, in whom surgery is unable to remove the entire tumor, have an increased risk of death.[13]

Brain tumor
A brain tumor or intracranial neoplasm occurs when abnormal cells form within the brain. There are two main types of tumors: malignant or cancerous tumors and benign tumors. Cancerous tumors can be divided into primary tumors that start within the brain, and secondary tumors that have spread from somewhere else, known as brain metastasis tumors. This article deals mainly with tumors that start within the brain. All types of brain tumors may produce symptoms that vary depending on the part of the brain involved. These may include headaches, seizures, problem with vision, vomiting, and mental changes. The headache is classically worse in the morning and goes away with vomiting. More specific problems may include difficulty in walking, speaking and with sensation. As the disease progresses unconsciousness may occur.
The cause of most brain tumors is unknown.Risk factors that may occasionally be involved include a number of inherited conditions known as neurofibromatosis as well as exposure to the industrial chemical vinyl chloride, the Epstein-Barr virus, and ionizing radiation. While concern has been raised about mobile phone use, the evidence is not clear. The most common types of primary tumors in adults are: meningiomas (usually benign), and astrocytomas such as glioblastomas. In children, the most common type is a malignant medulloblastoma. Diagnosis is usually by medical examination along with computed tomography or magnetic resonance imaging. This is then often confirmed by a biopsy. Based on the findings, the tumors are divided into different grades of severity.

Cushing syndrome is a group of conditions caused by high levels of circulating cortisol or related corticosteroids.
Cortisol is a steroid hormone produced by the adrenal gland. Cortisol production is stimulated by the pituitary hormone ACTH. The secretion of ACTH is, in turn, promoted by the hypothalamic hormone CRH.
Cortisol helps the body deal with stress by mobilizing energy resources. It increases blood glucose level and makes glucose more available to the brain, as compared to other organs. For example, cortisol induces muscle cells to decrease glucose consumption and increase protein degradation, producing amino acids that can be converted into new glucose, a process known as gluconeogenesis. Cortisol also helps maintain high blood pressure to keep the body stay on high alert.
On the other hand, high levels of corticosteroids suppress inflammation and antibody production by B-cells. For this reason, cortisol and related products such as prednisone are used to treat inflammatory conditions and autoimmune diseases. These medications also help suppress the immune system to prevent organ rejection in transplant recipients.
Cushing syndrome usually results from excessive use of corticosteroid drugs, or overproduction of cortisol by the adrenal glands. A tumor in the adrenal gland may produce excess cortisol regardless of ACTH levels. On the other hand, the adrenal glands may be normal, but they are overstimulated by high ACTH levels produced by a tumor in the pituitary gland or another organ. This group of conditions is more specifically known as Cushing disease.
Corticosteroids cause obesity and alter the body fat distribution. Excessive fat develops specifically in the face and torso. Patients commonly have a round face, a hump on the back, and disproportionately slender arms and legs. Other signs include muscle wasting, thin and easily bruised skin, and purple streaks over the abdomen or chest. In the long term, patients may develop high blood pressure, diabetes, weakened bones, and vulnerability to infections.
There are several diagnostic tests for cortisol levels. Because normal cortisol secretion follows a daily rhythm - high in the morning and low at night, a high midnight cortisol level is an indicator of the disease.
ACTH levels are measured to determine the cause. Low ACTH levels suggest a primary adrenal cause, while high levels are indicative of a pituitary cause or an ectopic ACTH source.
Imaging tests can be used to detect tumors.
Supportive treatment includes diets high in proteins and potassium. If corticosteroid drugs are the cause, they may be gradually reduced to optimal levels. Certain medications can be used to block cortisol production, or block the effects of cortisol. Tumors are usually removed surgically or treated with radiation therapy.

Cushing syndrome occurs due to excess circulating glucocorticoids over a period of time. The most common cause is iatrogenic, including the use of topical and inhaled glucocorticoids. Endogenous causes of Cushing syndrome may be divided into adrenocorticotrophic (ACTH)-dependant and non-ACTH-dependent disease. ACTH-dependent disease arises when there is increased ACTH from either the pituitary (Cushing disease) or from an ectopic ACTH-producing tumor. Non-ACTH-dependent disease occurs due to either adrenal adenomas or adrenal carcinomas.
Cushing disease accounts for two-thirds of all cases of endogenous Cushing syndrome and is caused by excessive secretion of ACTH hormone by a pituitary microadenoma. This leads to increased secretion of cortisol (secondary adrenal hypercortisolism) from the adrenal cortex as well as increased levels of androgens. The typical cushingoid features seen in patients with Cushing syndrome arise because of the glucocorticoid excess. These cushingoid features are due in part to fat redistribution and typical skin changes.
Skin changes in Cushing syndrome are manifold. The skin becomes thin and translucent, and ecchymoses occur following minor trauma. Thinning of the epidermis also leads to the plethoric appearance of the skin. Hyperpigmentation of the skin may be seen but is more common in pituitary or ectopic ACTH production. Although generalized, it tends to occur on sun-exposed areas. Addison disease also results in a similar pattern of hyperpigmentation in sun-exposed areas, but it also occurs in the following areas: in the palmar creases, areas of friction, vermilion border, genital skin, and oral mucosa.
Acne, hirsutism, and menstrual irregularities, either oligomenorrhea or amenorrhea, arise due to the increased levels of androgens.
Cortisol hypersecretion also predisposes to hypertension, peripheral edema, glucose intolerance, gonadal dysfunction, osteoporosis, and proximal muscle weakness. Psychiatric symptoms may be present in a large majority of patients (more than 70%), and severity of symptoms range from anxiety to mania, depression, and frank psychosis. Insomnia presents early in the disease.
Ectopic ACTH production can also lead to Cushing syndrome. In these patients, because the onset of Cushing syndrome is sudden, there is no time to develop the characteristic clinical features. Instead, these patients will have sudden-onset hypertension, edema, glucose intolerance, and hypokalemia. Seventy-five percent of these tumors are small cell lung carcinoma. Ectopic ACTH is produced by bronchial carcinoids, thymic or pancreatic carcinoids, thymic neuroendocrine tumors, medullary carcinoma of the thyroid, or pheochromocytomas.
Patients with carcinoid tumors or pheochromocytomas usually have the typical cushingoid features, as these develop over a longer period of time. Due to the immunosuppressive effects of glucocorticoids, these patients commonly develop cutaneous staphylococcal, candidal, and superficial fungal infections. The incidence of pituitary-dependent adrenal hyperplasia is more common in women by 3-fold, with the average age of onset being in the 3rd and 4th decade.
Mortality is increased 4-fold compared with the general population, with cardiovascular disease being the major cause of death.
Although uncommon in children, obesity along with retarded growth may be a presenting feature.

Cushing's syndrome is the collection of signs and symptoms due to prolonged exposure to glucocorticoids such as cortisol.[3][8][9] Signs and symptoms may include high blood pressure, abdominal obesity but with thin arms and legs, reddish stretch marks, a round red face, a fat lump between the shoulders, weak muscles, weak bones, acne, and fragile skin that heals poorly.[2] Women may have more hair and irregular menstruation.[2] Occasionally there may be changes in mood, headaches, and a chronic feeling of tiredness.[2]
Cushing's syndrome is caused by either excessive cortisol-like medication such as prednisone or a tumor that either produces or results in the production of excessive cortisol by the adrenal glands.[10] Cases due to a pituitary adenoma are known as Cushing's disease, which is the second most common cause of Cushing's syndrome after medication.[3] A number of other tumors may also cause Cushing's.[3][11] Some of these are associated with inherited disorders such as multiple endocrine neoplasia type 1 and Carney complex.[7] Diagnosis requires a number of steps.[4] The first step is to check the medications a person takes.[4] The second step is to measure levels of cortisol in the urine, saliva or in the blood after taking dexamethasone.[4] If this test is abnormal, the cortisol may be measured late at night.[4] If the cortisol remains high, a blood test for ACTH may be done.[4]
Most cases can be treated and cured.[6] If due to medications, these can often be slowly decreased if still required or slowly stopped.[5][12] If caused by a tumor, it may be treated by a combination of surgery, chemotherapy, and/or radiation.[5] If the pituitary was affected, other medications may be required to replace its lost function.[5] With treatment, life expectancy is usually normal.[6] Some, in whom surgery is unable to remove the entire tumor, have an increased risk of death.[13]
