Metastasis or Metastaseis ("dialectic transformations"), is an orchestral work by Iannis Xenakis, a Greek composer-architect and a major figure in the postwar development of musical modernism worldwide. He is particularly remembered for the pioneering use of stochastic mathematical techniques in his compositions, including probability (Maxwell-Boltzmann kinetic theory of gases, aleatory distribution of points on a plane, minimal constraints, Gaussian distribution, Markov chains), game theory, group theory, Boolean algebra and Brownian motion. Metastasis was inspired by Einstein's view of time (a function of matter & energy) and structured on mathematical ideas by Xenakis's colleague Le Corbusier. The 1st and 3rd movements don't have a melodic theme to hold them together, but rather depend on the strength of this conceptualization of time. The 2nd movement does have some sort of melodic element. A fragment of a 12-tone row is used, with durations based on the Fibonacci sequence (1,1,2,3,5,8,13,21,34...) The preliminary sketch for Metastasis was in graphic notation looking more like a blueprint than a musical score, showing graphs of mass motion and glissandi like structural beams of the piece, with sound frequencies on one axis and time on the other. Skin involvement occurs in 24% of breast cancer cases. It is the presenting sign 3.5% of the time. Several distinct clinical patterns of metastatic breast cancer are recognized 1. Inflammatory metastatic breast carcinoma resembles erysipelas in the anterior chest but without fever or tenderness. It is caused by capillary congestion. 2. En cuirasse metastatic breast carcinoma is a diffuse morphea-like induration of skin that begins as scattered, firm, lenticular papulonodules that coalesce. Local lymphatic spread is possible. 3. Telangiectatic metastatic carcinoma, the violaceous papulovesicles resemble lymphangioma circumscriptum. There is local lymphatic spread. It may be pruritic and may resemble vasculitis. 4. Nodular metastatic carcinoma, multiple firm papules or nodules appear on the anterior chest. They may be ulcerated and may suggest melanoma or pigmented basal cell carcinoma. 5. Alopecia neoplastica has asymptomatic, non-inflammatory, circular areas of alopecia. There is distant hematogenous spread. 6. Paget's disease of the breast, there is a sharply defined plaque of erythema and scaling on the breast, suggestive of eczema, but the eruption is persistent. Over 100000 new cases of colon cancer are diagnosed in the United States alone annually. For patients who are surgically receptable, meaning they can undergo an operation for their colon cancer, we might not catch small clusters of cancer cells called micrometastases that might be around at the time of surgery, but are not detectable by current methods. For patients who have high risk tumor features or positive lymph nodes, the goal of postoperative systemic therapy which is delivered via the bloodstream is to try and get rid of some of these small clusters of cells, to really increase the cure rate. For patients who have non operable, metastatic colon cancer systemic therapies like chemotherapy and targeted therapies are used.

Sylvia De Vries was diagnosed with Stage III-C Ovarian Cancer in October 2003. She had major surgery to remove her cancer-infected reproductive organs and her appendix, followed by 6 rounds of Carboplatin Taxol chemotherapy. She went into remission until December 2009, and commenced Carboplatin Taxol chemotherapy once again for her metastases. After 3 failed treatments, she was put on 6 rounds of Calyx, then Gemzar, which ended up attacking her bone marrow, for which she had a hard time. On August 2, 2010, her Canadian oncologist deemed her terminal and gave her approximately 2 months to live, but she and her husband were not prepared to accept that. Her husband began researching for other alternative cures and was impressed by the Issels Treatment. They also talked to another woman from Windsor, Ontario, who had experienced the treatment. In October 14, 2010 Sylvia came to the Issels Treatment Center in a very frail state, in a wheelchair, and unable to walk. In her own words: "In less than 3 weeks I was able to walk 6 blocks to the ocean from here...The doctors are very caring, they're very aggressive with their treatment, they're very hands-on. You're not a number." Her tumor marker Ca-125 was 141 when she started the Issels Treatment, and in less than 3 weeks it dropped down to 77.9. She feels stronger, more vibrant, and feel alive again. "I know I'm going to be healed. This is the place to be."
