This video shows the surface marking of the upper border of the thyroid cartilage as seen by using a 70 rigid endoscope. The overlying mucosa is very transparent enabling it visualization as marked. There is also left vocal fold palsy.

Crico-Laryngeal Separation: Complete Tracheal Transection
A young lady got accidentally strangulated while she was pillion riding with her husband. Her "churni/dupatta" (Indian ladies scarf) got entangled in the back wheel dragging her quite some distance before her husband could detect it. She made a grunting sound and immediately collapsed. She was cyanosed (blue) and had extensive swelling (surgical emphysema) of her face and neck, her pupils were dilated (significant brain hypoxia), subconjunctival haemorrhage and multiple face and neck petechiae. There was a dramatic ligature mark in the neck as seen in hanging cases. The emergency nurse could intubate her instantly on reception at the hospital (CSSH). A CT Scan could not give much details to the extent of her injury. As there was a suspicion of tracheal injury clinically she was taken up for exploratory surgery. A midline longitudinal neck incision almost immediately led to the diagnosis of complete transection between Cricoid and the Thyroid cartilage. The anterior surface of trachea was exposed and a trachoestomy done. A cuffed trachesotomy tube was inserted and connected to a sterile Bainbridge circuit so that the endotracheal tube can be removed for facilitating the anastomosis between the cricoid and laryngeal box. The cricoid cartilage and the thyroid cartilage is anastomosed using 2-0 prolene continuous suture. The wound is closed and neck kept in flexed position by few retention stitches between the chin and the anterior chest wall. The patient was gradually weaned <b>...</b>

To relax abdomen, throat is a good reflexion point. Which part of the throat is the best position? The answer is not only one place, and move around as time goes. On thyroid cartilage or thyroid gland can be good. To push throat aside is another good idea. Which side is better? Try it. Don't push with strong power.
