This is a video performed by Joseph S. Galati, Houston, Texas, on a young patient with chronic abdominal pain. After an evaluation with blood work and an ultrasound of the abdomen, an upper endoscopy was performed that revealed a large gastric ulcer in the antrum/pylorus of the stomach. Several smaller erosions were also noted in the stomach.

Stomach Ulcer: An Overview
Up to 100 million individuals worldwide are affected with stomach ulcer at a given time. It is most commonly caused by an infection with Helicobacter pylori. Other causes include pain medications and smoking. Stomach ulcer is most commonly diagnosed with an upper endoscopy examination. The treatment of stomach ulcer includes eradiction of Helicobacter, discontinuation of pain medication, and cessation of smoking. Additionally, medications that suppress acid production in the stomach are advised.

Gastric Ulcers Overture
A peptic ulcer is a hole in the gut lining of the stomach, duodenum, or esophagus. A peptic ulcer of the stomach is called a gastric ulcer; of the duodenum, a duodenal ulcer; and of the esophagus, an esophageal ulcer. An ulcer occurs when the lining of these organs is corroded by the acidic digestive juices which are secreted by the stomach cells. Peptic ulcer disease is common. For many years, excess acid was believed to be the major cause of ulcer disease. Accordingly, treatment emphasis was on neutralizing and inhibiting the secretion of stomach acid. While acid is still considered significant in ulcer formation, the leading cause of ulcer disease is currently believed to be infection of the stomach by a bacteria called "Helicobacter pyloricus" (H. pylori). Another major cause of ulcers is the chronic use of anti-inflammatory medications, commonly referred to as NSAIDs (nonsteroidal anti-inflammatory drugs), including aspirin. Cigarette smoking is also an important cause of ulcer formation and ulcer treatment failure. H. pylori bacteria is very common, infecting more than a billion people worldwide. It is estimated that half of the United States population older than age 60 has been infected with H. pylori. Infection usually persists for many years, leading to ulcer disease in 10 % to 15% of those infected. H. pylori is found in more than 80% of patients with gastric and duodenal ulcers. Endoscopy of two Giant Gastric Ulcers
This video endoscopy shows two ulcers in a cirrhotic patient. This 82 year old woman who had had severe ascites and previously in the paracentesis procedure had drained 6 liters of ascites liquid, at endoscopy two giant ulcers were found. Liver cirrhosis is frequently associated with complicated peptic ulcer disease. It has been showed that peptic ulcer is more frequent in patients with liver cirrhosis, is associated with the severity of cirrhosis, and occurs without upper abdominal pain in up to 70% of patients and with complications in 29%. This giant ulcer is found at the lesser curvature of the gastric body, macroscopically has suspected, some parameters of malign but the biopsies were negative, however in six week will need a new follow up endoscopy that showed that the ulcer of the antrum is in the scar phase as well as the scar of the ulcer of the body., a six week with proton pump inhibitor (PPI) treatment.

Peptic Ulcers - Part 1
Gastric ulcers (those in the stomach) and duodenal ulcers (those in a tube connected to the stomach, called the duodenum) are usually referred to as peptic ulcers. Peptic ulcers can cause a variety of symptoms, and these vary from patient to patient. Some patients with ulcers have minimal, unusual, or even no symptoms at all. Others may have every symptom. It is also important to remember that the symptoms listed below can occur as a result of other conditions, not just duodenal or gastric ulcers. These include gastroesophageal reflux disease (GERD), chronic dyspepsia without the presence of ulcers (often called non-ulcer or functional dyspepsia), gallbladder disease, liver disease and other disorders. Once again, if you are concerned about your symptoms, it is important that you see your doctor about your stomach problems.
