Log into tumblr.com/customize. Click here to open that page in a new window.

In your customization settings, find the option to add your Disqus Shortname.

Save your theme and you're done. If you can't find this Disqus option in your customization settings, it's because some Tumblr themes do not have built-in Disqus support. It is still easy to manually add Disqus to your theme by reading the following steps.
Manual instructions (advanced)

These instructions are for manually adding Disqus to your Tumblr theme. Select "Edit HTML" under the thumbnail of your current theme.

Copy and paste the following embed code into the HTML.

Where should you paste this? Every theme is a little different.

First, check to see if your theme already has a specific spot to paste this code.
Try searching the HTML for the words disqus
or comments
to find that spot.

Didn't find anything? No problem. You can just paste the code immediately after this line in the HTML:
{/block:Posts}

This next step is to add a link to each post that displays the number of comments, such as "3 Comments."
Copy and paste the following code so that it is at the bottom of each post type in your HTML.

With most themes, you can just paste this immediately before this line in the HTML:
{/block:Posts}
