Dr. Supriya Mohile Discusses Prostate Cancer in Elderly Patients
Supriya G. Mohile, MD, MS, associate professor, Department of Medicine, Hematology/Oncology, University of Rochester Medical Center, discusses the prevalence and treatment options for elderly patients with prostate cancer. Mohile describes prostate cancer as primarily a disease of older people. In general, the median age for patients with prostate cancer is 79, compared to the early 70's in colon and lung cancer. Information that is usually uncommon for older populations is more prevalent in prostate cancer because of the higher median age. Major trials investigating new therapeutic agents have enrolled more geriatric patients because of the abundance of this patient type. This has provided more data on the risks and benefits of treatment for elderly patients with prostate cancer than other types of tumors. As the population ages and people live longer with prostate cancer it becomes more challenging to treat the disease. There is a need to be careful with aggressive treatment in earlier asymptomatic stages of prostate cancer, once the disease becomes more advanced or symptomatic an aggressive treatment makes more sense.

By Faris Mokhtar | SingaporeScene -- Thu, Jun 16, 2011 National Development Minister Khaw Boon Wan has hinted that there might be a need to revise the current income ceiling for Singaporeans applying for the Build-To-Order HDB flats. In his latest blog post on Thursday, Minister Khaw gave a breakdown of the profile of BTO flat applicants for the month of May. The data showed that among the number of first-timer applicants, more than half had applied under the HDB's Fiancé-Fiancée Scheme with the median age at 27. The median age of those in the remaining portion which include married couples is around 34. Given the rising age of applicants, Minister Khaw said that there is justification in revising the HDB income ceiling. In his first BTO launch last month, Minister Khaw said that nearly 14000 Singaporeans applied for 4000 units. Of those who applied, 65 per cent are first-timers, meaning those who have never bought a flat directly from HDB or taken a CPF housing grant. The minister also noted that 40 per cent of the applicants had applied under the Married Child Priority Scheme, to live near their parents. While the ministry will be "mindful" of their intentions, he pointed out that it will not be easy considering that mature estates have limited scope for further BTO development. SINGAPORE: Marriages registered in Singapore last year suffered their first annual decline since 2003. According to the latest publication on marriage and divorce statistics, 24363 marriages were registered, 6.6 per cent lower than in 2009. The Department of Statistics (DOS) said the number of non-Muslim marriages fell, which offset the slight increase in Muslim marriages over the same period. In tandem with the fall in number of registered marriages, the general marriage rates declined to a historic low in 2010. Among unmarried male residents, the general marriage rate declined from 43.6 marriages per thousand unmarried males in 2009 to 39.4 in 2010. Among unmarried female residents, the rate dropped from 41.1 to 37.2 marriages per thousand unmarried females during the same period. DOS said marriage rates fell across the younger age groups below 30 years in 2010, with males and females aged 25-29 years experiencing the largest fall compared to 2000. The peak age group for men marrying shifted from 25-29 years in 1990 and 2000 to 30-34 years in 2010. The peak age group for women marrying remained at 25-29 years in 2010. About 74 per cent of total marriages in 2010 were first marriages where neither party had previously been married. A further 18 per cent were remarriages for one of the partners. Not all professional organizations agree with these conclusions and heated debates continue even though, in a 12-year study, treating localized prostate cancers in men 65 years old showed no benefit over watchful waiting. In autopsy studies, prostate cancer has been reported to occur in 15-60% of men 60 years old. In one autopsy study of men (median age 64 years) who had died of other causes with no evidence of prostate cancer, careful tissue sections detected cancer in 29%. Such a high frequency of undetected prostate cancers in older men suggests that most of these cancers tend to be of the innocent type. Although Prostate Specific Antigen (PSA) screening has been associated with a decrease in prostate cancer death rates, detection of clinically unimportant cancers continues to be a problem. It is estimated that the annual incidence of spinal cord injury (SCI), not including those who die at the scene of the accident, is approximately 40 cases per million population in the US or approximately 12000 new cases each year. Since there have not been any incidence studies of SCI in the US since the 1990′s it is not known if incidence has changed in recent years. The number of people in the United States who are alive in 2010 who have SCI has been estimated to be approximately 265000 persons, with a range of 232000 to 316000 persons. SCI primarily affects young adults. From 1973 to 1979, the average age at injury was 28.7 years, and most injuries occurred between the ages of 16 and 30. However, as the median age of the general population of the United States has increased by approximately 9 years since the mid-1970, the average age at injury has also steadily increased over time. Since 2005, the average age at injury is 40.7 years. Other possible reasons for the observed trend toward older age at injury might include changes in either referral patterns to model systems, the locations of model systems, survival rates of older persons at the scene of the accident, or age-specific incidence rates. Overall, 80.7% of spinal cord injuries reported to the national database have occurred among males. Just look at what candidates said during the Republican primaries: "Mitt Romney will complete construction of a high-tech fence," the candidate announced on his campaign website. Michele Bachmann promised to "build a double-walled fence." And Herman Cain said: "We're going to have a fence; it's going to be 20 feet high; it's going to have barbed wire on the top." The fence in question guards a third of America's 2000 mile-long border with Mexico. Supporters of harsher laws argue that 3 out of every 5 illegal immigrants are from Mexico. But just as American hostility is reaching a crescendo, the problem might be disappearing. I was struck by a Pew report I saw this week. It says a historic pattern of migration has been reversed. Comparing two recent five-year blocks, the report finds that not only has the number of Mexicans immigrating to the US declined by 50 percent (from 3 million to about 1.4 million), the number of Mexicans going the other way back home has doubled. December 10th 2010 Gila Bend was founded in 1872 and is a town located in Maricopa County, Arizona. As of the census of 2000, there were 1980 people, 659 households and 492 families residing in the town. Gila, (pronounced "hee-la") Bend, was named for the sharp bend in the Gila River. It is located near a prehistoric Hohokam Indian Village which Farther Eusebio Francisco Kino visited in 1699. Kino found the fertile banks of the Gila River had been abandoned by an early Indian tribe call the Opas, who had established a rancheria and raised two grain crops annually, irrigating from the Gila River. This same rancheria was visited by Spanish Captain Juan Bautista de Anza, commander of the Presidio at Tubac, and founder of the City of San Francisco, and Father Francisco Tomas Garces in 1774. In recognition of historical routes that pass through the area, the town's website refers to Gila Bend as "The Crossroads of the Southwest". The nickname the "Crossroads of the Southwest" stems from the area having been part of an important transportation route in the settling, development and growth of the Great Southwest. Interrupting the blood flow for more than 20 to 25 minutes during kidney cancer surgery leads to a greater risk for patients developing chronic kidney disease, a Mayo Clinic and Cleveland Clinic collaborative research team has found. The study was published today in the journal European Urology. For the retrospective study, researchers analyzed outcomes of 362 patients with only one kidney who underwent surgery for renal cortical tumors at Mayo Clinic and Cleveland Clinic between 1990 and 2008. Using a technique called warm ischemia, surgeons kept the patient's kidneys at body temperature during the partial nephrectomy. Ischemia involves cutting off the blood supply to the kidney with clamps in order to control bleeding and to keep blood from obscuring the surgeon's view of the kidney. It also allows for precise closure of the urine collecting system and the surgical opening. The median ischemia time was 21 minutes in the study and the median age of patients was 62 years. Ischemia can cause tissue damage from a lack of oxygen and nutrients. Researchers found that each additional minute of warm ischemia is associated with a 5 to 6 percent increase in the odds of developing acute renal failure or reduced kidney functioning and is associated with a 6 percent increased risk of new onset Stage IV chronic kidney disease during long-term follow-up.
