Nasopharyngeal Cancer (NPC): Endoscopic View of Post-nasal Space
Nasopharyngeal carcinoma (NPC) is a cancer involving the nasopharynx (hind-part) of the nose. It usually originated at the fossa of Rosenmüller (FOR) area which is a cleft just behind the prominence of tubal opening (torus tubarius). It is classified further into well, moderately, and poorly-differentiated type depending on the degree of differentiation on histopathological examination (WHO classification). Its commoner presenting symptoms include epistaxis (nose bleed) or blood-stained saliva, hearing loss (usually one side- conductive type), nose block, and neck lump (lymph node spread). This video shows cancer involving the nasopharynx with evidence of recent bleeding. Biopsy was taken for confirmation. (Yellow arrow- tumour, curve green arrow- right eustachian tube opening, red arrow- ulcerated tumour with bleeding, S- nasal septum, IT- tail of inferior turbinate, R- roof of nasopharynx)

The Nasopharynx as Seen Using Transoral Nasopharyngoscopy Technique
Nasopharynx or epipharyx is the hindpart of the nose and not easily visualized. Traditionally, it is assessed indirectly by using a postnasal mirror and reflected light. Anterior rhinoscopic examination is limited to mid-nasal cavity. The endoscopic technology advancement has enabled this part of the nose to be examined through the nasal passage or transorally. This video shows an examination of the nasopharynx via transoral nasopharyngoscopic approach using a 70 degrees rigid endoscope looking upward and anteriorly towards the posterior nasal carity. Excellent appreciation of its anatomy can be seen. (W-posterior nasopharyngeal wall, R- roof of nasopharynx, S- posterior nasal septum made by vomeric bone and its overlying mucosa, MT- middle turbinate, IT- inferior turbinate, Greeen arrow- eustachian tube opening, blue arrow leads to sphenoid ostium, red oval- fossa of Rossenmuller). NB: The superior turbinate can be seen partly on the left side. The left side of the video corresponds to left side of the patient and the right side of the video corresoponds to the right side of the patient.

Juvenile Nasopharyngeal Angiofibroma is not an uncommon tumour. It presents with recurrent profuse bleeding from nose. Traditionally it is removed by lateral Rhinotomy / Weber Furguson approach. Lately we have been removing large tumours by sublabial approach, thus avoiding external scar on face. This vedeo shows post embolisation endoscopic removal of JNAF involving the nose, nasopharynx and sphenoid sinus.

The author offers own tactics of surgical treatment of patients with presence of the basicranialy-extending of JNA that according to the unified classification is subdivided into tumors I, II and III categories. Under supervision there were 40 patients with JNA, from which at 28 (70 %) patients there was a basicranialy extending form of a tumor [at 12 (30 %) patients there was intracranialy-extending form]. At 4 patients was available I (the tumor occupied a nasopharynx, nasal cavity and sphenoid sinus), at 9 - II (the form of growth of a tumor corresponded I to a category + a tumor it was involving in sphenopalatinal fossa and sinuses of a ethmoidal bone) and at 15 - III (the form of growth of a tumor corresponded to II category + a tumor was involving in infratemporal fossa, orbit, maxillary sinus and parapharyngeal space) a category of a tumor. The differentiated application of such modified operations as operation on Owens (at removal of tumors I of a category), Denker (at removal of tumors of II category) and to Moure (at removal of tumors of III category) has allowed to provides radical removal of a tumor at the overwhelming majority sick (87,7 %), that, in opinion of the author, specifies high efficiency of tactics of surgical treatment of patients.

Comparatively limited ability of operation with application transpalatinal access (in comparison with the operations, providing a wide operational field) allow to apply this operation only at "small" juvenile nasopharyngeal angiofibroma, occupying a nasopharynx, extending into nasal cavity and sphenoid sinus, ie at tumors of I category/or stage. Transpalatine approach provides a short and direct way to the said anatomic structures that allows carrying out full removal of a tumor, not inflict damage to nearby structures. Advantage transpalatine approaches also appear paunch absence on face, which in the cosmetic relation is that allows carrying out full removal of a tumor, not inflict damage to nearby structures.
